Fifteen to 20 percent of patients with major facial trauma suffer vision-threatening injuries. Not only does the early identification and management of such ophthalmic insults frequently carry a better visual prognosis, but manipulations during facial fracture repair may exacerbate unrecognized eye trauma. BACKGROUND: Despite reported successes with progressive addition lenses, lined multifocals remain the lens of choice for previous lined multifocal wearers.
METHODS: To explore the feasibility of switching lined multifocal wearers to PALs, in a prospective clinical trial we interviewed 265 lined multifocal wearers, prescribed a new PAL (Varilux Comfort) and then interviewed them again to compare the lens types and patient preferences.
RESULTS: PALs ranked significantly higher than lined multifocals for all questions and were preferred by 92 percent of the patients.

We evaluated the distance and near vision of 33 presbyopic patients who wore the Anterior Constant Focus (ACF) rigid gas permeable (RGP) bifocal lens. Twenty-seven patients (82%) wore the lenses for one year; the six patients who quit the study complained of unacceptable vision (four patients) and discomfort (two patients). Twenty-five patients (76%) obtained 20/25 or better distance vision, and 28 patients (85%) obtained J2 or better near vision. Contrast sensitivity was within normal limits in 24 of 33 (73%) patients. A literature survey of amplitudes of accommodation in various countries is presented. Results of push-up and minus-lens determination of 447 patients in Nigeria are compared with previous results. A smaller decrement in amplitude due to temperature than previously accepted is suggested.

A number of scientifically substantiated indications of the application of placental suspension in man of higher age, the positive results of the studies of the effectiveness of placental therapy on metabolic processes and physiological functions of the aging organism, and the experience won in the application of placental suspension in the complex therapy of some diseases frequently occurring at old age allow us to consider this kind of treatment an effective therapy and thus to recommend tissue therapy with a placental suspension for the introduction into geriatric practice.
