Severe Maternal Morbidity by Disability Status and Type in the United States.

OBJECTIVE: To estimate the risk of severe maternal morbidity (SMM) among women with disabilities compared with those without disabilities in a nationally representative sample of U.S. delivery hospitalizations. METHODS: We conducted a retrospective cohort analysis using the 2016-2021 Healthcare Cost and Utilization Project's Nationwide Inpatient Sample. We identified delivery hospitalizations and disability status using International Classification of Diseases, Tenth Revision diagnosis codes. The primary outcome was SMM, which was determined using 21 indicators specified by the Centers for Di

Prevalence and materno-fetal outcomes of preeclampsia/eclampsia among pregnant women in Nigeria: a systematic review and meta-analysis.

BACKGROUND: Preeclampsia and eclampsia (PE/E) are hypertensive disorders of pregnancy with significant morbidity and mortality for both mothers and fetuses. This study aimed to investigate the prevalence of PE/E, associated complications, and mortality rates in pregnant women in Nigeria using a systematic review and meta-analysis approach. METHODS: A search strategy was employed to identify relevant studies published in English from electronic databases like PubMed, Google Scholar, Science Direct, AJOL, DOAJ and Cochrane Library. Studies investigating the prevalence of PE/E, associated compli

Protracted postpartum urinary retention-a long-term problem or a transient condition?

INTRODUCTION AND HYPOTHESIS: Protracted postpartum urinary retention (P-PUR) is a rare puerperal complication of overt urinary retention that proceeds beyond the 3rd postpartum day. Long-term consequences of P-PUR are poorly reported. The objective of the study was to compare the long-term outcome of patients with P-PUR with a matched control group, using a validated pelvic floor distress questionnaire. METHODS: All medical files of women diagnosed with P-PUR between 2005 and 2016 were reviewed. The control group was comprised of women who had a consecutive birth, matched in a 1:2 ratio, by m

Pediatric subaxial cervical spine injuries: origins, management, and outcome in 51 patients.

Periods of intense hormonal fluctuations have been associated with heightened prevalence and exacerbation of underlying psychiatric illness, particularly the occurrence of premenstrual dysphoria, puerperal depression and depressive symptoms during perimenopause. It has been speculated that sex steroids such as estrogens, progestogens, testosterone and dehydroepiandrosterone (DHEA) exert a significant modulation of brain functioning, possibly through interactions with various neurotransmitter systems. It is therefore intuitive that abrupt alterations of these hormones would interfere with mood

Prenatal beta-endorphin as an early predictor of postpartum depressive symptoms in euthymic women.

BACKGROUND: After delivery, many women experience symptoms of postpartum depression (PPD), and early identification of women at risk is therefore important. The opioid peptide beta-endorphin has been implicated in non-puerperal depression but its role in the development of PPD is unknown. METHODS: Three hundred and seven women with a singleton, full-term (>37.0 weeks' GA) pregnancy were recruited early in pregnancy and followed up into the postpartum period. Blood samples were obtained at 15, 19, 25, 31 and 37 weeks' gestational age (GA) and at 9 weeks postpartum for assessment of beta-endorp

Postpartum obsessive compulsive symptom onset: Modeling a pathway of risk.

The perinatal period represents uniquely elevated risk for the development of obsessive-compulsive symptoms compared to any other period in the lifespan but the variables of risk and the specific pathways that lead to symptom development are not well understood. The goal of the current study was to evaluate an integrated, theoretically-derived model of risk focusing on a pathway from genes through personality vulnerability to dysfunctional beliefs to symptom development in the postpartum period. It was hypothesized that women with elevated stress reactivity, via genetic and personality vulnera

Assessing Factors Affecting Postpartum Post-Traumatic Stress Disorder and Development of Risk Prediction Nomogram Model.

BACKGROUND: Currently, the factors impacting postpartum post-traumatic stress disorder (PP-PTSD) remain unclear. Therefore, this study aimed to screen the PP-PTSD risk factors and to develop an effective and user-friendly column chart prediction model (nomogram), thereby providing a basis for early clinical diagnosis and prompt intervention. METHODS: This retrospective study collected 180 postpartum women between January 2023 and December 2023. Based on the occurrence of PP-PTSD, study participants were divided into two groups: a control group (No-PP-PTSD) and an observation group (PP-PTSD).

Self-harm in women with postpartum mental disorders.

BACKGROUND: Women suffering from first onset postpartum mental disorders (PPMD) have a highly elevated risk of suicide. The current study aimed to: (1) describe the risk of self-harm among women with PPMD and (2) investigate the extent to which self-harm is associated with later suicide. METHODS: We conducted a register-based cohort study linking national Danish registers. This identified women with any recorded first inpatient or outpatient contact to a psychiatric facility within 90 days after giving birth to their first child. The main outcome of interest was defined as the first hospital-

Ventral Spontaneous Durotomy Following Vaginal Delivery.

BACKGROUND: Dural breaches have a diverse etiology, including spontaneous rupture and trauma. Most cases resolve with bed rest; in refractory cases, an epidural blood patch can be placed to obstruct further leakage. We discuss a unique case of a spontaneous ventral durotomy following vaginal delivery that was managed with injections of autologous blood through bilateral transforaminal needles. CASE DESCRIPTION: A previously healthy, 36-year-old pregnant woman presented to the inpatient maternity ward with positional occipital headaches and neck pain 24 hours after normal spontaneous vaginal d

A pharmacological mouse model suggests a novel risk pathway for postpartum psychosis.

Postpartum psychosis (PP) is a severe psychiatric disorder affecting a small proportion of new mothers shortly after childbirth. The molecular pathophysiology underlying the disorder is currently poorly understood, and there are no amenable animal models for the condition; maternal deficiency for the enzyme steroid sulfatase has been proposed as a potential risk mechanism. Here we show that inhibition of steroid sulfatase with 667-COUMATE (10mg/kg p.o.) in new mouse mothers results in behavioural abnormalities that can be partially alleviated by the administration of the clinically-efficacious

History of childhood abuse is accompanied by increased dissociation in young mothers five months postnatally.

BACKGROUND: Dissociation has been recognized as a relevant factor within the context of traumatization. Since childhood maltreatment as well as child birth can be regarded as a potential trauma, this study examined dissociation in a sample of 58 young mothers with a history of abuse in comparison to a control group. METHODS: All women with newborn children were contacted by mail and presented with the Childhood Trauma Questionnaire. Women who reached a cutoff for moderate or severe sexual and/or physical abuse and whose children were term babies with Apgar scores >7 were included in the study

[Psychosomatic aspects of pregnancy, parturition and puerperium (author's transl)].

Pregnancy, delivery and post-partum period represent not only a special physical strain, but also an extra-ordinary psychic stress. Defective developments and neuroses, especially in the sexual field, as well as improperly adjusted current conflicts, can lead to somatic disturbances. The present-day situation is particularly characterized by the dual role played by the modern woman in society, and the resulting ambivalence of her attitude toward maternity. Over and above this, the technicalization of modern medicine can be associated with a loss of the human relationship between the patient an

Factors that influence maternal mortality in eclampsia.

Three hundred sixty-five cases of eclampsia, including 49 women who died, were analyzed in order to determine factors which led to death. The age of the patient was clearly the most important factor. Older women tended to have coexisting renal and vascular disease and also manifested more hematologic abnormalities, in particular, disseminated intravascular coagulation. Other important factors leading to death were twin pregnancies, delay in hospitalization, failure to terminate the pregnancy, and the physician's unawareness of the severity of the mother's disease. The mortality after cesarean
