Impact of insurance coverage on access to assisted reproductive technology: A nationwide survey in Japan (the IZANAMI project).

AIM: Assisted reproductive technology (ART) is a well-established infertility treatment. However, financial and geographical barriers can hinder patient access. The Japanese government implemented insurance coverage in April 2022 for specific ART treatments. This study assessed the effects of insurance coverage on ART utilization and accessibility. METHODS: An online survey was administered to 427 ART facilities that participated in a pre-policy assessment from December 1, 2023, to January 30, 2024. RESULTS: A total of 283 facilities responded (response rate: 66.3%). The number of ART patien

Implication of the Vaginal Microbiome in Female Infertility and Assisted Conception Outcomes.

The rise in infertility rates has prompted research into the impact of vaginal microbiota on female fertility and the success of assisted reproductive technology (ART). Our study aimed to compare the vaginal microbiome between fertile and infertile women and explore its influence on ART outcomes. Vaginal secretion samples were collected from 194 infertile women and 100 healthy controls at Shanghai Changzheng Hospital. The V3-V4 region of the 16S rRNA gene was amplified using polymerase chain reaction (PCR). A machine learning model was applied to predict infertility based on genus-level abunda

Epidemiology of falling fertility: the contribution of social, environmental, and genetic forces.

The past half century has witnessed a dramatic decline in human fertility as reflected in the total fertility rate. This decline in total fertility rate is thought to have been triggered by an increase in resources and knowledge that precipitated a significant decline in infant mortality. This, in turn, led to a reduction in the desire of couples to have large families, supported in recent times by a series of factors including a delay in childbearing as women acquired the education and autonomy to enter the paid workforce, the progressive urbanization of advanced societies, and, for many, a s

Special considerations in assisted reproductive technology for patients with obesity.

Obesity rates are increasing, and patients with obesity may disproportionately seek fertility preservation and fertility treatment because of a higher risk of related disease. At the same time, in vitro fertilization centers across the United States are increasingly setting body mass index (BMI) cutoffs above which treatment will not be provided. Patients experience these limits as another example of stigma in healthcare as well as an access challenge, exacerbating the distress of infertility. A healthy lifestyle in the long term should be the focus rather than an arbitrary BMI, which is a fla

Time-series clustering of the migration speed of nucleolus-precursor bodies in human zygotes and live birth following assisted reproductive technology.

PURPOSE: Nucleolus-precursor body (NPB) migration speed in human zygotes predicts live birth after assisted reproduction. However, the impact of changes in NPB migration speed on live births was unclear. This study investigated the relationship between patterns of change in NPB migration speed over time and clinical outcomes. METHODS: The central coordinates of NPBs were tracked at multiple time points to assess changes in NPB migration speed from 1 to 7 h before the disappearance of the pronucleus. Time-series clustering was performed. Cluster centroids were calculated using the time-series

The Utility of Sperm DNA Fragmentation as a Diagnostic Tool for Male Infertility and Its Predictive Value for Assisted Reproductive Technology Outcomes.

Standard semen parameters remain the cornerstone of male infertility evaluation, though they often poorly reflect the likelihood of success in assisted reproductive technology (ART). This study evaluates sperm DNA fragmentation (SDF) as a diagnostic tool for male infertility and predictive biomarker for ART success. Semen samples were collected from 20 fertile donors and 40 infertile patients with abnormal semen parameters. A fraction of each sample was used for SDF assessment via TUNEL assay and flow cytometry, while the remaining portion was processed for conventional semen analysis and ART.

Pharmacogenomic of LH and its receptor: are we ready for clinical practice?

Luteinizing hormone (LH) is fundamental to support development and reproduction. It acts through a receptor expressed in the gonads, modulating mitogenic, anti-apoptotic, and steroidogenic signals. LH is also marketed as a drug for controlled ovarian stimulation (COS), where it is administered to women to support the action of follicle-stimulating hormone and can lead to specific responses, depending on the individual genetic background. These concepts underline the relevance of a pharmacogenetic approach to COS, in the attempt to optimize clinical outcomes and avoid adverse events. However, k

Viral infection and its impact on fertility, medically assisted reproduction and early pregnancy - a narrative review.

Viral infections can significantly affect the physiopathology of reproductive organs, leading to fertility problems, reducing the success rates of assisted reproductive technologies, and negatively impacting pregnancy. This review aims to summarize the current evidence on viral pathogens that are either suspected or confirmed to play a role in reproductive medicine and their effects on early pregnancy. For instance, viral hepatitis and human immunodeficiency virus can decrease sperm quality. Human papilloma virus infection in men appears to cause infertility, while herpesviruses pose a greater

Bioengineered 3D ovarian model for long-term multiple development of preantral follicle: bridging the gap for poly(ε-caprolactone) (PCL)-based scaffold reproductive applications.

BACKGROUND: Assisted Reproductive Technologies (ARTs) have been validated in human and animal to solve reproductive problems such as infertility, aging, genetic selection/amplification and diseases. The persistent gap in ART biomedical applications lies in recapitulating the early stage of ovarian folliculogenesis, thus providing protocols to drive the large reserve of immature follicles towards the gonadotropin-dependent phase. Tissue engineering is becoming a concrete solution to potentially recapitulate ovarian structure, mostly relying on the use of autologous early follicles on natural or
