[Binge eating disorder].

In 1994 binge eating disorder (BED) was included into the DSM-IV as provisional diagnostic category requiring further study. The prevalence in the general population is 2%; BED is 1.5 times more common in women than men. Up to 30% of participants in weight loss programs meet criteria for BED. As opposed to patients with bulimia nervosa (BN), restraint or restrictive eating is not a necessary antecedent in the development of BED. Eating-related as well as general psychopathology is significantly more common in obese patients with BED compared to obese patients without BED. In treating obese pat

[Coping strategies in laryngectomy patients].

BACKGROUND: Over the last few years, paradigms for judging the results of treatment of tumor patients have shifted. In addition to characteristics such as the complete excision of the tumor and the duration of patient survival, restoring the quality of life has become an equally important aim. There is still a lack of systematic (and sufficient) research on coping strategies as a means of dealing with malignant tumors of the head and neck. PATIENTS AND METHOD: In a cross-sectional study, 25 laryngectomees, whose voices had been successfully restored with a Provox voice prosthesis, were examine

[Successful surgical repair of composite graft detachment occurred 5 months after combined Bentall's operation and graft replacement with active aortitis syndrome].

Ninteen-year-old male with annuloaortic ectasia and resultant massive aortic valvular regurgitation, along with aortic aneurysmal dilatation extending from the root to the distal arch, underwent surgical repair at our hospital. Operative procedure comprised composite graft replacement of aortic valve-ascending aorta (Bentall's operation with Piehler's modification), combined with the aortic arch replacement with reconstruction of the neck vessels. Physical, laboratory, and histopathological findings revealed active aortitis syndrome as the etiology of this aortic disease. Five months after ope

Delirium in the elderly.

Delirium, an acute confusional state, is an organic brain syndrome that manifests deficits in attention, irrelevant or rambling speech, and other cognitive deficits. Its symptoms often fluctuate over the course of the day, and patients may be hyperactive--for example, restless and screaming--or hypoactive--for example, quiet, inactive, and stuporous. Occurring in approximately 20% of hospitalized elderly patients, delirium is the most common psychiatric syndrome in acutely ill general medical and surgical patients. Fifteen to 30% of delirious patients expire, and others are prone to a variety

Combined nicotinamide riboside and magnesium-l-theanine protects against chronic stress-induced gut-liver injury by modulating NAD⁺ and metabolic pathways.

BACKGROUND: Chronic variable stress (CVS) has been associated with increased intestinal permeability and hepatic dysfunction, primarily through oxidative imbalance and metabolic dysregulation. This study evaluated the protective effects of nicotinamide riboside (NR) and magnesium-L-theanine (MgT) supplementation against CVS-induced intestinal and hepatic damage in rats. METHODS AND RESULTS: A total of 28 rats were randomly divided into four groups: Control, MgT + NR, Stress, and Stress + MgT + NR. NR (600 mg/kg/day) and MgT (400 mg/kg/day) were administered orally throughout the experimental

Interrelationships between stress, eating attitudes, and metabolism in endurance athletes across a competitive season.

A competitive sport season represents a multidimensional stressor where physical and psychological stress may render an athlete susceptible to energy deficiency (ED). Downstream effects of ED can include a reduction in measured-to-predicted resting metabolic rate (RMRratio), indicating metabolic compensation. A pathway linking stress, eating attitudes, and metabolic compensation has not been explored. To test if sport-specific stress is associated with eating attitudes and metabolism in endurance athletes (18-22 years) at different phases of a competitive season, we assessed two groups of athl

Hippocampal injection of the exercise-induced myokine irisin suppresses acute stress-induced neurobehavioral impairment in a sex-dependent manner.

Stress disrupts a variety of neural processes, including reducing levels of brain-derived neurotrophic factor (BDNF) in the hippocampus. In contrast, exercise increases BDNF and is beneficial for health and cognition. Irisin is a myokine that is released into circulation during exercise. Although its main known functions are browning white adipose tissue and improving glucose homeostasis, Irisin also mediates the activation of an exercise-induced BDNF-mediated neuroprotective pathway in the hippocampus. Therefore, we tested the hypothesis that Irisin can counteract the deleterious effects of a

Patient safety in inpatient mental health settings: a systematic review.

OBJECTIVES: Patients in inpatient mental health settings face similar risks (eg, medication errors) to those in other areas of healthcare. In addition, some unsafe behaviours associated with serious mental health problems (eg, self-harm), and the measures taken to address these (eg, restraint), may result in further risks to patient safety. The objective of this review is to identify and synthesise the literature on patient safety within inpatient mental health settings using robust systematic methodology. DESIGN: Systematic review and meta-synthesis. Embase, Cumulative Index to Nursing and A

Impact of a nurse engagement intervention on pain, agitation and delirium assessment in a community intensive care unit.

BACKGROUND: In 2013, the Society of Critical Care Medicine published a revised version of the ICU Pain, Agitation, and Delirium (PAD) guidelines. Immobility and sleep were subsequently added in 2018. Despite the well-established advantages of implementing these guidelines, adoption and adherence remain suboptimal. This is especially true in community settings, where PAD assessment is performed less often, and the implementation of PAD guidelines has not yet been studied. The purpose of this prospective interventional study is to evaluate the effect of a multifaceted nurse engagement interventi

Impact of a comanaged Geriatric Fracture Center on short-term hip fracture outcomes.

BACKGROUND: Hip fractures are associated with substantial morbidity and mortality for older adults. Patients sustaining hip fractures usually have comorbid conditions that may benefit from comanagement by geriatricians and orthopedic surgeons. METHODS: The Geriatric Fracture Center (GFC) is part of a community teaching hospital. Patients are comanaged daily by a geriatrician and orthopedic surgeon, emphasizing total quality management, timely treatment, and standardized care. We reviewed medical records to compare process and outcome measures in the GFC with a local institution that did not h

Normal lactational environment restores nephron endowment and prevents hypertension after placental restriction in the rat.

Uteroplacental insufficiency in the rat restricts fetal growth, impairs mammary development, compromising postnatal growth; and increases adult BP. The roles of prenatal and postnatal nutritional restraint on later BP and nephron endowment in offspring from mothers that underwent bilateral uterine vessel ligation (restricted) on day 18 of pregnancy were examined. Sham surgery (control) and a group of rats with reduced litter size (reduced; litter size reduced at birth to five, equivalent to restricted group) were used as controls. Offspring (control, reduced, and restricted) were cross-fostere

Effects of medetomidine in dogs with mitral regurgitation.

Thirteen small breed dogs with heart murmurs due to mitral regurgitation of varying severity received medetomidine as a sedative drug for teeth cleaning procedures. The dose was 30 micrograms/kg as an intramuscular injection. The drug caused a considerable reduction of the body temperatures and the heart rates over the 6 hrs of monitoring. The respiratory rates increased over the first 30 min and later decreased below control values. Systolic arterial blood pressures did not change significantly. Arterial oxygen tensions were significantly lower at 30 min after drug administration, but the art

Hypoxanthine-guanine phosophoribosyltransferase (HPRT) deficiency: Lesch-Nyhan syndrome.

Deficiency of hypoxanthine-guanine phosphoribosyltransferase (HPRT) activity is an inborn error of purine metabolism associated with uric acid overproduction and a continuum spectrum of neurological manifestations depending on the degree of the enzymatic deficiency. The prevalence is estimated at 1/380,000 live births in Canada, and 1/235,000 live births in Spain. Uric acid overproduction is present inall HPRT-deficient patients and is associated with lithiasis and gout. Neurological manifestations include severe action dystonia, choreoathetosis, ballismus, cognitive and attention deficit, and

FTO alleviates chronic restraint stress-induced cognitive deficits and depressive-like behaviors via regulating PI3K/Akt signaling, autophagy, and synaptic plasticity.

OBJECTIVE: This study aimed to investigate whether Fat mass and obesity-associated protein (FTO) alleviated chronic restraint stress (CRS)-induced neurobehavioural deficits and elucidated the potential mechanisms. METHODS: C57BL/6 mice were randomly assigned to five groups: Control, CRS, CRS + AAV-mediated FTO overexpression, CRS + AAV-empty, CRS + FTO-AAV + LY294002. Depressive-like behaviors and cognitive function were assessed followed by CRS procedure. Western blot was used to analyze PI3K/Akt signaling, autophagy markers (LC3, Beclin-1), and synaptic proteins (PSD-95, Synaptophysin). Imm

Melatonin protects against chronic stress-induced oxidative meiotic defects in mice MII oocytes by regulating SIRT1.

Chronic stress which is common in the current society can be harmful to female reproduction and is associated with oocyte defects. However, the underlying mechanisms remain largely unknown. Herein, by using a mouse model of chronic restraint stress, we demonstrated that chronic stress could induce meiotic spindle abnormalities, chromatin misalignment, mitochondrial dysfunction and elevated ROS levels in oocytes in vivo, all of which were normalized by the administration of melatonin. Consistently, melatonin treatment during in vitro maturation also attenuated the meiotic defects induced by H2O

Does chronic stress enhance the risk of diseases?

OBJECTIVE: In the everyday life, stress is deemed as something unfavorable that may enhance the risk for the development or worsen a disease. However, in its nature, stress is adaptive reaction of the body. Its main characteristic is the activation of the hypothalamic-pituitary-adrenocortical (HPA) axis. Previously, we have shown that activation of the HPA axis plays a gastroprotective role during acute stress. The aim of our study was to clarify the effects of chronic stress and chronically elevated basal corticosterone levels on the gastric ulceration and cardiovascular vulnerability in rats
