Complications of shoulder arthroscopy.

Shoulder arthroscopy can be a safe and effective tool to perform a variety of diagnostic and therapeutic procedures about the shoulder. Careful attention to positioning, fluid management, use of traction, and appreciation of shoulder anatomy to select portal placement will help decrease the common complications associated with shoulder arthroscopy. The awareness of uncommon complications, such as deep venous thrombosis, pneumothorax, and iatrogenic rotator cuff tears will help the orthopedic surgeon to promptly diagnose and treat these problems.

Posterior superior glenoid impingement: expanded spectrum.

Posterior superior glenoid impingement is a recently recognized mechanism of injury producing rotator cuff injury in athletes. Usually the mechanism is repetitive overhand activity such as throwing. A survey of the author's practice was undertaken to show a wider spectrum of this mechanism both in the activity that caused it and the number of structures at risk of injury from this mechanism. The survey revealed 11 patients who had a clear recollection of their mechanism of injury and an objective documentation of the injury by arthroscopy or imaging studies. The majority of shoulders had damag

Imaging of shoulder instability.

Multiple imaging modalities are used in the evaluation of the patient with known or suspected instability. Plain films are often the initial study and are useful in detecting fractures and assessing alignment. Magnetic resonance imaging, with its multiplanar capabilities, such as that of visualizing soft-tissue structures, is rapidly becoming the study of choice for shoulder instability. Ultrasonography and computed tomography arthrography have limited roles in the evaluation of the patient with instability.

[Factors affecting the surgical results of treatment of rotator cuff tears in the shoulder].

The work presents the outcomes of clinical, radiological and mathematical analyses of 42 patients with the rupture of the whole width of the cuff of shoulder rotators treated operatively. The group comprised 34 male and 8 female patients, the average age 55.5 years. The mean period between the operation and final clinical evaluation was 23 months. According to the method chosen for the evaluation of the results by means of 100-point score acceptable result was achieved in 90 per cent of cases (excellent in 64 per cent, good in 26 per cent). Double t-test showed that significantly participating

Clinical presentation of complete tears of the rotator cuff.

To determine whether the presence and extent of a tear of the rotator cuff could be predicted on the basis of a patient's history, physical examination, and radiographic findings, detailed data from the histories and physical examinations of 103 patients who were known to have a tear of the rotator cuff were correlated with the radiographic and operative findings on these patients. An age-matched control group of fifty-one patients who had similar symptoms, but whose arthrograms showed normal results, was used to establish a baseline incidence of ten specific radiographic findings in the shoul

Impingement syndrome in athletes.

Athletes, particularly those who are involved in sporting activities requiring repetitive overhead use of the arm (for example, tennis players, swimmers, baseball pitchers, and quarterbacks), may develop a painful shoulder. This is often due to impingement in the vulnerable avascular region of the supraspinatus and biceps tendons. With the passage of time, degeneration and tears of the rotator cuff may result. Pathologically the syndrome has been classified into Stage I (edema and hemorrhage), Stage II (fibrosis and tendonitis), and Stage III (tendon degeneration, bony changes, and tendon rupt

Medial dislocation of the tendon of the long head of the biceps brachii.

Medial displacement of the long tendon of the biceps brachii muscle is a rare condition associated with degenerative or traumatic ruptures of the rotator cuff. This condition was recorded in nine shoulders during 45 reconstructive procedures on the rotator cuff. Five of the displacements were complete, leaving the tendon medially displaced in a fascial sling; four were incomplete, allowing a to-and-fro medial displacement of the tendon out of the intertubercular groove. Anatomical dissections on eight shoulders showed that the coracohumeral ligament is the key ligament which keeps the biceps t

Biomechanical risk factors for rotator cuff syndrome in high-risk occupations: A prospective study protocol.

BACKGROUND: Rotator cuff syndrome (RCS) is the most common upper limb musculoskeletal disorder worldwide. RCS negatively impacts quality of life and comes with high costs to the individual and society through time loss of work or healthcare usage. Identifying modifiable risk factors for RCS is a critical avenue for exploration to improve prevention and treatment of RCS. OBJECTIVE: The overarching goal of this research is to explore the connection between shoulder kinematics and RCS in high-risk occupations and determine if pre-injury shoulder kinematics during a standardized overhead reaching

Location of brachial plexus birth injury affects functional outcomes in a rat model.

Brachial plexus birth injury (BPBI) results in shoulder and elbow paralysis with shoulder internal rotation and elbow flexion contracture as frequent sequelae. The purpose of this study was to develop a technique for measuring functional movement and examine the effect of brachial plexus injury location (preganglionic and postganglionic) on functional movement outcomes in a rat model of BPBI, which we achieved through integration of gait analysis with musculoskeletal modeling and simulation. Eight weeks following unilateral brachial plexus injury, sagittal plane shoulder and elbow angles were

Self-assessment and Modulation of Traction During Shoulder Dystocia Simulation Training.

OBJECTIVE: The aim of the study is to determine diagnostic traction for shoulder dystocia and to assess whether applied traction is modifiable with force training. METHODS: We tethered a force-measuring fetal mannequin (PROMPT, Limbs & Things) within a simulated pelvis such that it would not deliver. We asked participants to apply traction to diagnose shoulder dystocia then stop. Blinded from participants' view, we recorded the peak traction. We then asked them to apply what they perceived to be 20 lb (89 N) traction. Each participant estimated the traction s/he applied. The actual force appl

Posttraumatic Neurogenic Thoracic Outlet Syndrome and Immediate Effects of Decompressive Surgery: A Case Report.

Neurogenic thoracic outlet syndrome (TOS) results from a combination of trauma and congenital anatomical predisposition. Although trauma is recognized as a significant contributor to neurogenic TOS, it is predominantly linked to injuries such as whiplash-type neck injuries in individuals with predisposing congenital anatomical structures. Reports on neurogenic TOS resulting from major trauma, including fractures and dislocations near the brachial plexus pathway, are rare. We report a rare case of a patient with persistent paralysis in the right shoulder, preventing abduction, extension, and el

Rotator cuff rehabilitation: current theories and practice.

A fully functioning, painless shoulder joint is essential to maintain a healthy, normal quality of life. Disease of the rotator cuff tendons (RCTs) is a common issue that affects the population, increasing with age, and can lead to significant disability and social and health costs. RCT injuries can affect younger, healthy patients and the elderly alike, and may be the result of trauma or occur as a result of chronic degeneration. They can be acutely painful, limited to certain activities or completely asymptomatic and incidental findings. A wide variety of treatment options exists ranging fro

[Middle third clavicle fractures--diagnosis, complications and treatment].

Clavicle fractures compose 2.5% to 5% of all fractures, and usually occur due to a direct blow to the shoulder. The incidence of these fractures persistently increases. Most of the fractures are in the middle third--a result of the unique structure of the clavicle and its ligamentous and muscular attachments. The fracture diagnosis includes focused physical examination, radiographic evaluation, and comprehensive evaluation of the patient, considering the fact that the fracture can be the result of significant energy absorption and can hide other injuries. Usually treatment is non-operative, an

Rotator cuff tendinosis in an animal model: role of extrinsic and overuse factors.

The rat shoulder animal model has been used previously to study the role of intrinsic injury (modeled as an acute insult to the tendon), extrinsic injury (modeled as external subacromial impingement), and overuse factors on rotator cuff tendinosis. These studies demonstrated that it is possible to produce rotator cuff tendinosis with any one of these factors in isolation. The current study uses the rat shoulder model to study the roles of extrinsic compression, overuse, and overuse in combination with extrinsic compression, on the development of rotator cuff tendinosis. The results of this stu

Traumatic shoulder dislocation with combined bankart lesion and humeral avulsion of the glenohumeral ligament in a professional basketball player: three-year follow-up of surgical stabilization.

Traumatic anterior shoulder instability has been well documented to have associated lesions such as a Bankart tear, humeral avulsion of the glenohumeral ligament (HAGL), Hill-Sachs lesion, fracture, and nerve injury. To our knowledge, the combined Bankart and HAGL injury in a single acute anterior shoulder dislocation has not yet been reported. We describe a traumatic first-time anterior-inferior shoulder dislocation in a professional basketball player with a combined Bankart and HAGL lesion. The patient underwent arthroscopic Bankart repair followed by open repair of the HAGL lesion with an o
