Extensor pollicis longus tendon rupture in an adult after intramedullary nailing of a radius fracture: case report.

This case report details the late rupture of an extensor pollicis longus tendon in a 21-year-old woman who had intramedullary nailing of her radius fracture. We identify possible pitfalls with this particular patient and with other intramedullary upper extremity techniques and discuss how to avoid them in future cases.

[Treatment of post-traumatic swan-neck deformities of the fingers. Apropos of a series of 43 patients].

Thirty three patients with 51 swan neck deformities were reviewed with a mean follow up of 35 months (6 to 86 months). 8 fingers presenting some stiffness necessitated a first step of splinting before operation giving nevertheless 5 excellent and good results. Among the other fingers with full range of passive motion surgery was performed in 38 cases with 81.5 per cent of excellent and good results. According to etiology, articular or extrinsic, excellent and good results were respectively 71 per cent and 94 per cent. The most frequently used operations were Littler type II (12 cases) and Litt

Four-bone ligament reconstruction for treatment of chronic complete scapholunate separation.

This study reviews the results of a four-bone ligamentous weave reconstruction for treatment of chronic complete scapholunate separation. This reconstruction between the radius, lunate, capitate, and scaphoid employs a dorsal and palmar approach weaving a long strip of the extensor carpi radialis brevis through the four bones reducing and stabilizing the scaphoid and lunate with a wire loop. Thirty-six consecutive cases of operated chronic complete scapholunate separation were evaluated in a 2- to 10-year follow-up. Average length of follow-up was 4.8 years. Average age was 34 years. Average p

Upper and lower trapezius muscle transfer to restore shoulder abduction and external rotation in longstanding upper type palsies of the brachial plexus in adults.

Lesions affecting the upper roots of the brachial plexus result in paralysis of shoulder abduction and external rotation. In longstanding lesions, neurological surgery is not recommended in which case muscle transfers become an option to improve shoulder function. We describe the surgical treatment of seven adult patients with longstanding lesions of the upper roots of the brachial plexus, in whom the upper trapezius muscle was transferred to the humeral head, whereas the lower trapezius muscle was sutured to the infraspinatous muscle tendon. Within an average of 11.7 months after surgery, pat

Treatment of residual instability and extensor lag in polydactyly of the thumb.

Extensor indicis tendon transfers were used in six cases with residual deformity of the thumb in polydactyly. The indication was extensor lag and instability of the interphalangeal joint. At mean follow-up of three and a half years, normal stability was improved. Full extension was achieved in three cases and two cases also derived some benefits from this operation. Alignment was improved in five cases.

Upper-limb function in Brachial Plexus birth palsy: Does the Pediatric Outcomes Data Collection Instrument correlate with the Brachial Plexus Outcome Measure?

Functional outcome assessment for children with brachial plexus birth palsy (BPBP) is essential. We aimed to investigate the association between the Brachial Plexus Outcome Measure (BPOM) and the Pediatric Outcomes Data Collection Instrument (PODCI) in children with BPBP and healthy matched controls, and to explore upper-limb function and the discriminative properties of the two tests. Thirty children, aged 6-14 years, diagnosed with BPBP and undergoing shoulder tendon transfer and 20 age-matched healthy controls were included. In both groups, the Turkish version of the PODCI, used to evaluate…

After a general presentation of the geographical and social aspects of Vietnam, the authors describe the conditions of their surgical work d … amination, brachial plexus injuries cannot be treated, except the simplest cases needing one or two tendon transfers. Concerning skin coverage and reconstructive surgery, our experience has shown that pedicled local and locoregional flaps, and even microsurgical transfers are adapted and reliable techniques due to the imperative of single stage procedure. Free transfer of a large sensory flap from the first web space and dorsum of the foot including the second toe for reconstruction of a mutilated hand.

A 19-year-old man sustained a mutilating crush injury in August, 1978, in an 80-ton punch press. The injury resulted in transmetacarpal amputations of the index through little fingers. The thumb was revascularized by microvascular anastomoses. The replanted index to the little finger position failed. A free transfer of the patient's second toe to the fifth metacarpal was done to provide a post to oppose the thumb. The first web space of the toe was used in expanding the contracted single cleft of the hand. The innervated dorsal foot flap provided coverage for the dorsal surface of the hand. An

Semitendinosus Tendon Reconstruction for Recurrent Acromioclavicular Joint Instability Following Repeated Fixation Failure and Postoperative Trauma: A Case Report.

Recurrent instability following surgical treatment of Rockwood type III acromioclavicular (AC) joint injuries is uncommon but represents a challenging clinical problem, particularly after repeated fixation failure. A 67-year-old man presented with a Rockwood type III AC joint separation following a same-level fall. Initial suture-button fixation failed 45 days postoperatively. Revision hook plate fixation subsequently failed after a further traumatic event, and a third procedure involving rigid AC stabilization with dual plating also resulted in early mechanical failure. Definitive treatment w

Rupture of the extensor pollicis longus tendon.

Rupture of the extensor pollicis longus (EPL) tendon after nondisplaced fracture of the distal radius is a known complication, but can also occur in nonrheumatoid patients without a history of fracture. This study is a retrospective analysis of seven patients treated between 1985 and 1992. Five EPL ruptures occurred with nondisplaced Colles' fractures. Two involved a chronic tenosynovitis of the second dorsal extensor compartment of the wrist. The age of the patients ranged from 29 years to 68 years (mean, 42 years). Length of follow-up ranged from 6 weeks to 62 months (mean, 30 months). Ruptu

Surgery in advanced Charcot-Marie-Tooth disease.

Crippling equinocavovarus position of the foot in advanced Charcot-Marie-Tooth disease with its attendant metatarsalgia tender calluses and rigidity creates painful, limited, hobbling ambulation with lowered work and general activity tolerance. The object of the surgeries is to convert this foot into a plantigrade , more flexible, painless unit. Surgery consists of plantar fasciotomies off the tubercle of the os calcis and also transversely through the middle of the longitudinal arch. Dorsally angulated green-stick osteotomies are performed at the bases of all the metatarsals. A Jones operatio

Hand Extension Robot Orthosis (HERO) Glove: Development and Testing With Stroke Survivors With Severe Hand Impairment.

The hand extension robot orthosis (HERO) glove was iteratively designed with occupational therapists and stroke survivors to enable stroke survivors with severe hand impairment to grasp and stabilize everyday objects, while being portable, lightweight, and easy to set up and use. The robot consists of a batting glove with artificial tendons embedded into the glove's fingers. The tendons are pulled and pushed by a linear actuator to extend and flex the fingers. The robot's finger extension and grasp assistance are automated using inertial measurement unit signal thresholds. Five stroke survivor

Reconstruction of defects involving the Achilles tendon and local soft tissues: a quick solution for a lingering problem.

A total of seven patients (six men and one woman) with a defect in the Achilles tendon and overlying soft tissue underwent reconstruction using either a composite radial forearm flap (n = 3) or an anterolateral thigh flap (n = 4). The Achilles tendons were reconstructed using chimeric palmaris longus (n = 2) or tensor fascia lata (n = 2) flaps or transfer of the flexor hallucis longus tendon (n = 3). Surgical parameters such as the rate of complications and the time between the initial repair and flap surgery were analysed. Function was measured objectively by recording the circumference of th

The cross-digital dorsal adipofascial flap.

The "homodigital distally based" dorsal adipofascial flap has been used by various authors to cover small, complex, dorsal, digital defects. In 2004, Al-Qattan reported on 3 cases in which a "distally based" dorsal adipofascial flap was used in a "cross-digital" fashion. In the current report, we expand on the concept of cross-digital adipofascial flaps in which the flaps are based not only distally but also proximally or laterally located. A total of 14 patients with complex dorsal digital defects were reconstructed with cross-digital adipofascial flaps. The main indication for using the cros

Midfoot ulcers treated with gastrocnemius-soleus recession.

BACKGROUND: Foot ulcers in patients with neuropathy are a common cause of hospital admission for infection sometimes resulting in amputation in patients with neuropathy. Tendon lengthening alone has been reported to be successful in treating neuropathic forefoot ulcers. Tendon lengthening has also been recommended as an adjunct to bony procedures (exostectomy or fusion) for treating midfoot ulcers. The author reports the results of gastrocnemius-soleus recession as the sole treatment of diabetic midfoot ulcers. MATERIALS AND METHODS: This study evaluated the results of 11 patients with 11 neu
