PURPOSE: Ethnic and area-level deprivation disparities in psychiatric inpatient outcomes amongst patients with psychotic disorders are known. However, how these two variables intersect to influence features of inpatient care is unclear. We investigated this intersection.
METHODS: Using de-identified electronic health data from inpatient services at a large south London mental healthcare provider, we identified a sample of 6767 working-age patients with non-affective psychotic disorders who were admitted between 2016 and 2019. Logistic and negative binomial regressions were used to examine the

The insula is a heterogeneous cortical region, comprised of three cytoarchitecturally distinct sub-regions (agranular, dysgranular, and granular), which traverse the anterior-posterior axis and are differentially involved in affective, cognitive, and somatosensory processing. Smaller insula volume is consistently reported in psychosis-spectrum disorders and is hypothesized to result, in part, from abnormal neurodevelopment. To better understand the regional and diagnostic specificity of insula abnormalities in psychosis, their developmental etiology, and clinical correlates, we characterized i

The main subject of criminal proceedings is that of criminal responsibility, from this point of view bipolar disorders sometimes seem to be a neglected subject in legal scholarship. Yet they may affect decision-making across the spectrum of the law, especially when manic and psychotic symptoms are implicated. This case studies a 37-year-old woman, diagnosed with bipolar affective in disorder, who attacked the neighbour of her ex-husband during a manic episode with psychotic symptoms. Two groups of those psychotic symptoms are especially remarkable: delusions and experiences of influences playi

In three patients, two men aged 33 and 43 years receiving treatment for epilepsy, and one woman aged 31 years with mild mental retardation and psychotic episodes, psychotic symptoms emerged soon after the introduction of topiramate. These symptoms were associated with agitation/aggressive behaviour and significant weight loss. Psychotic symptomatology gradually disappeared after discontinuation of topiramate without the administration of an antipsychotic. Over the past years several novel anticonvulsants, including topiramate, have become available for the treatment of refractory partial seizu

IMPORTANCE: Less than 10% of research on psychotic disorders has been conducted in settings in the Global South, which refers broadly to the regions of Latin America, Asia, Africa, and Oceania. There is a lack of basic epidemiological data on the distribution of and risks for psychoses that can inform the development of services in many parts of the world.
OBJECTIVE: To compare demographic and clinical profiles of cohorts of cases and rates of untreated psychoses (proxy for incidence) across and within 3 economically and socially diverse settings in the Global South. Two hypotheses were teste

OBJECTIVE: Schizophrenia and other non-affective psychoses (NAP) are associated with an increased prevalence of both type 2 diabetes (DM2) as well as a family history of DM2. We performed a systematic review and meta-analysis of the association between comorbid DM2 and a family history of DM2 in patients with NAP.
METHOD: We searched major electronic databases from inception until August 2018 for studies of comorbid DM2 in patients with non-affective psychosis and family history of DM2 status. Random effects meta-analysis calculating odds ratios (ORs) and 95% confidence intervals (CI) and met

Genital self-mutilation is a rare severe form of self-injurious behaviour usually described in psychotic disorders with delusions & hallucinations. It has been ascribed to sexual conflicts, body image distortion, internalized aggression and suicidal intent. This phenomenon has been described in schizophrenia, affective psychosis, alcohol intoxication and personality disorders. In the present case genital self-mutilation in a case of Major-Depressive Disorder in an 82 yrs old male is being reported and psychosocial factors are discussed. This case has some unusual features.

The present paper shows that the short-term prognosis of patients with paranoid disorders hospitalized nowadays is poor for the majority. The best prognosis is attached to patients with affective or other not specified psychotic disorders. Socio-vocational variables at the time of first admission contain more predictive value than clinical variables. The term reactive psychosis as a term with good prognosis is questioned and the findings of a mortality study are presented in support of this view when concerned with hospital populations.

Electroencephalographic (EEG) sleep patterns were examined in nine unmedicated patients meeting DSM-III-R criteria for a current manic episode (four men and five women) for two to four consecutive nights. Compared with age- and sex-matched normal control subjects, manic patients exhibited significantly decreased total recording period, decreased time spent asleep, increased time awake in the last two hours of recording, shortened rapid eye movement (REM) latency, increased REM activity, and increased REM density. These results suggest that mania is associated with marked disturbances of sleep

BACKGROUND: Family members of individuals with schizophrenia suffer from elevated rates of schizophrenia-spectrum disorders (SSD) and other forms of psychopathology. However, few studies have examined familial psychopathology in probands with a first episode of psychosis (FEP). We systematically evaluated family history in patients experiencing an affective or non-affective FEP.
METHODS: The Family Interview for Genetic Studies was used to obtain diagnostic information on all first- and second-degree relatives of probands admitted to a specialized FEP program. Probands were 94 previously untr

Psychotic disorders and schizophrenia-spectrum personality disorders (PD) with psychotic/psychotic-like symptoms are considerably linked both historically and phenomenologically. In particular with regard to schizotypal and schizotypal personality disorder (SPD), this is evidenced by their placement in a joint diagnostic category of non-affective psychoses in the InternationaI Classification of Diseases 10th Revision, (CD-10) and, half-heartedly, the fifth edition of Diagnostic and Statistical Manual of Mental Disorders, (DSM-5). Historically, this close link resulted from observations of pecu

BACKGROUND: Reduction of positive symptoms is often the focus of psychiatric treatment for patients with psychosis; however, it is becoming increasingly clear that some patients experience ambivalence towards positive symptoms or may even experience them as pleasant. The present study extends upon work from online studies of patients without verified diagnoses. The first aim was to examine the frequency with which patients report negative, neutral or even positive appraisals of positive symptoms. A second aim was to identify whether these appraisals were associated with medication adherence.

OBJECTIVE: Individuals with severe mental illnesses (SMI), including schizophrenia spectrum illnesses and affective disorders, may be at increased risk for negative mental health outcomes related to the COVID-19 pandemic. This study compared the severity of pre-pandemic symptoms and affective experiences to current symptoms to evaluate this possibility.
METHODS: 148 individuals with SMI (92 with schizophrenia spectrum illnesses and 56 with affective disorders) were recruited from ongoing ecological momentary assessment studies that sampled day-to-day experiences and symptom severity prior to

BACKGROUND: This study aims to describe the prevalence of catatonia in a population of older acute psychiatric inpatients according to different diagnostic criteria. Secondary objectives are: to compare the catatonic symptom profile, prevalence, and severity, in respect to the underlying aetiology, and to evaluate the association between catatonic and somatic comorbidity.
METHODS: The study included 106 patients admitted to an acute geriatric psychiatry ward. Catatonia was assessed using the Bush Francis Catatonia Rating Scale (BFCRS).
RESULTS: Catatonia was highly prevalent (n=42; 39.6%), e

The hippocampus, an integral component of the corticolimbic circuitry of the brain, has been recently implicated in the pathophysiology of schizophrenia. This article has employed quantitative morphometric techniques to determine whether abnormalities of posterior hippocampal cross-sectional area, as well as the number, size, and degree of disarray of pyramidal neurons were present in 9 control and 14 schizophrenic subjects. Seven schizophrenic patients showed evidence of superimposed mood disturbance (schizoaffective type), while the remaining seven were a mixture of paranoid, undifferentiate

Utilization of positive and negative feedback was investigated in a sample of 20 depressed and 20 nondepressed psychiatric inpatients. Patients, selected on the basis of Minnesota Multiphasic Personality Inventory scores and clinical diagnoses, received primarily positive or negative feedback after completing a moral judgment task. Results suggest that depressed patients are likely to utilize positive, personally relevant information to enhance their low self-esteem, whereas nondepressed patients are more likely to counteract negative feedback that is inconsistent with their self-evaluations.

Within psychiatry, no treatment is as effective - or controversial - as electroconvulsive therapy (ECT), and ECT is the only non-pharmacological treatment in widespread clinical psychiatry. The history of ECT as a psychiatric treatment is outlined in this review, and the efficacy in four diagnostic entities is reviewed along with side effects. The evidence shows compelling effect of ECT when administered within affective disorders, delirium and psychotic states.

Extensive experience with the diagnosis of childhood-onset schizophrenia indicates a high rate of false positives. Most mislabeled patients have chronic disabling, affective, or behavioral disorders. The authors report the cases of three children who passed stringent initial childhood-onset schizophrenia "screens" but had no chronic psychotic disorder. For two, the European literature yielded more fitting diagnoses: psychosis not otherwise specified (e.g., reactive or psychogenic psychosis, paranoid schizophrenia), single episode in full remission (e.g., anxiety psychosis), and factitious diso
