BACKGROUND: Hindfoot varus deformity is a known risk factor for chronic lateral ankle instability (CLAI). The impact of this deformity on clinical results following arthroscopic lateral ankle ligament repair (ALLR) for CLAI has not been studied.
METHODS: Sixty-three ankles from 62 patients who received ALLR for CLAI were retrospectively examined. Preoperative plain radiographs were used to measure tibial articular surface (TAS) angles, and long axial hindfoot alignment radiographs were used to measure tibiocalcaneal angles (TCAs) pre- and postoperatively. Results included ratings on the Self-

BACKGROUND: A musculoskeletal model for the ankle complex is vital in order to enhance the understanding of neuro-mechanical control of ankle motions, diagnose ankle disorders and assess subsequent treatments. Motions at the human ankle and foot, however, are complex due to simultaneous movements at the two joints namely, the ankle joint and the subtalar joint. The musculoskeletal elements at the ankle complex, such as ligaments, muscles and tendons, have intricate arrangements and exhibit transient and nonlinear behaviour.
METHODS: This paper develops a musculoskeletal model of the ankle com

Ankle arthrodesis remains an important treatment option for patients with ankle arthritis. Many methods have been described; however, no consensus has been reached regarding the best technique to achieve both successful fusion and a good position for optimal foot mechanics. Furthermore, as arthroplasty has become more popular, preservation of the fibula to allow for future arthroplasty has become critical. The present report describes an innovative technique in which temporary external fixation at operative fixation is used, along with internal fixation, to achieve both an optimal foot positio

Primary ankle arthrodesis used to treat a neglected open ankle fracture dislocation is a unique decision. A 63-year-old man presented to the emergency department with a 5-day-old open fracture dislocation of his right ankle. After thorough soft tissue debridement, primary arthrodesis of the tibiotalar joint was performed using initial Kirschner wire fixation and an external fixator. Definitive soft tissue coverage was later achieved using a latissimus dorsi free flap. The fusion was consolidated to salvage the limb from amputation. The use of primary arthrodesis to treat a compound ankle fract

We present the outcomes in 38 consecutive patients who had total ankle replacement using the Ankle Evolution System with a minimum follow-up of four years. Pain and function were assessed using the American Orthopaedic Foot and Ankle Society (AOFAS) score and regular standardised anteroposterior and lateral weight-bearing radiographs were obtained. Patient satisfaction and complications were recorded and the survival of the implants was demonstrated by the Kaplan-Meier method. The mean follow-up was for 57.8 months (48 to 80). The cumulative survival rate at six years was 94.7% (95% confidence

To compare two methods of ankle evertor muscle reinforcement after ankle sprain: concentric vs eccentric reinforcement. Eighteen subjects practising sport with first episode of ankle sprain. The first randomized group underwent conventional physical therapy including concentric reinforcement of the evertor ankle muscles [concentric group (CG)]. The second randomized group underwent eccentric reinforcement [eccentric group (EG)]. At the end of the physical therapy, the strength of the evertor muscles was tested using an isokinetic dynamometer. The measurements were peak torques in the concentri

We performed this study to monitor changes in ankle eversion strength and sensorimotor control functions after acute ankle inversion injury. Forty-four patients with clinical grade II to III first-time ankle inversion sprains were tested for 1) pathologic talar tilt and anterior talar translation at 1 and 12 weeks after injury; 2) isometric eccentric ankle eversion strength and 3) peroneal reaction time to sudden ankle inversion at 3, 6, and 12 weeks after injury; and 4) accuracy of inversion position assessment 1, 3, 6, and 12 weeks after injury. Not all patients could perform all tests at th

Anterior ankle impingement syndrome (AAIS) has been reported to account for a high percentage of complications following ankle fracture surgery. The soft tissue etiology of AAIS is thought to be thickening and inflammation of the anterior ankle soft tissues intervening anteriorly at the tibiotalar joint, causing pain and functional limitation during dorsiflexion. However, the effects of anterior ankle soft tissue dynamics and stiffness on AAIS have yet to be clarified. This study aimed to determine the relationship between AAIS and the anterior ankle soft tissue thickness change ratio and shea

BACKGROUND: Chronic ankle instability commonly develops after lateral ankle sprain, but differences in movement patterns between subtypes of ankle instability have not been determined. We hypothesized mechanically and functionally unstable ankle subjects would demonstrate different kinematics and kinetics compared to a group of ankle sprain injury "copers".
METHODS: Sixty-three recreational athletes, 21 in each of 3 groups (11 males, 10 females) matched for gender, age, height, mass, and limb dominance were tested. Knee and ankle sagittal and frontal plane motion, peak ground reaction forces,

BACKGROUND: Acute lateral ankle ligament injuries are very common problems in present health care. Still there is no hard evidence about which treatment strategy is superior. Current evidence supports the view that a functional treatment strategy is preferable, but insufficient data are present to prove the benefit of external support devices in these types of treatment. The hypothesis of our study is that external ankle support devices will not result in better outcome in the treatment of acute ankle sprains, compared to a purely functional treatment strategy. Overall objective is to compare

BACKGROUND: The postexercise ankle-brachial index (ABI) is useful in patients with suspected peripheral arterial disease (PAD) and a normal resting ABI. Our objective was to determine the independent predictors of an abnormal postexercise ABI.
HYPOTHESIS: We hypothesized that the lowest ankle systolic pressure to calculate the resting ABI would be associated with an abnormal post-exercise ABI.
METHODS: Among 619 consecutive patients referred for suspected PAD, we calculated the postexercise ABI in patients with a normal resting ABI. An ABI <0.90 at rest was considered abnormal. We investigat

OBJECTIVES: The American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot Scale is among the most commonly used instruments for measuring outcome of treatment in patients who sustained a complex ankle or hindfoot injury. It consists of a patient-reported and a physician-reported part. A validated, Dutch version of this instrument is currently not available. The aim of this study was to translate the instrument into Dutch and to determine the measurement properties of the AOFAS Ankle-Hindfoot Scale Dutch language version (DLV) in patients with a unilateral ankle fracture.

Background: End-stage ankle osteoarthritis (OA) severely limits function and quality of life. Total ankle arthroplasty (TAA) and ankle arthrodesis (AA) are key surgical interventions when conservative treatment fails. This study compares TAA and AA outcomes using a national dataset to inform patient-centered care. Methods: A retrospective analysis of 27,595 patients undergoing TAA or AA from 2016-2019 was conducted using the National Inpatient Sample. Propensity score matching addressed baseline differences. Results: Primary OA was more prevalent in TAA (85.9%) than in AA (55.4%). TAA utilizat

BACKGROUND: We aimed to compare the clinical effectiveness, cost-effectiveness and complication rates of total ankle replacement with those of arthrodesis (i.e. ankle fusion) in the treatment of end-stage ankle osteoarthritis.
METHODS: This was a pragmatic, multicentre, parallel-group, non-blinded randomised controlled trial. Patients with end-stage ankle osteoarthritis who were aged 50-85 years and were suitable for both procedures were recruited from 17 UK hospitals and randomised using minimisation. The primary outcome was the change in the Manchester-Oxford Foot Questionnaire walking/stan

BACKGROUND: Ankle arthrodesis (AA) has historically been the gold standard surgical treatment for end-stage ankle arthritis, providing reliable pain relief at the expense of motion. Total ankle arthroplasty (TAA) has emerged as a motion-preserving alternative with improving survivorship and functional outcomes. However, large-scale comparative data evaluating complications, reoperation rates, opioid utilization, and health care costs remain limited. This study compared outcomes following AA and TAA using a national administrative database.
METHODS: A retrospective cohort study was performed u

CASE: A 37-year-old man developed chronic ankle instability because of the complete absence of the anterior talofibular and calcaneofibular ligaments after previous ankle arthroscopy, confirmed by magnetic resonance imaging. Arthroscopic lateral ligament reconstruction with a gracilis tendon autograft yielded excellent outcomes, with the patient achieving stability, being pain-free, and attaining normal functional scores at 12 months postoperatively.
CONCLUSION: This case highlights the importance of surgical expertise to prevent complications of the arthroscopy procedure, describing a rare c

BACKGROUND: Foot and ankle injuries are common among dancers. Full plantar flexion places stress on the lateral aspect of the ankle, increasing the risk of ankle sprains. Arthroscopic lateral ankle ligament repair has shown favorable clinical outcomes in elite athletes with chronic lateral ankle instability (CLAI); however, its effectiveness in dancers remains unclear. This study was performed to evaluate the clinical outcomes and efficacy of arthroscopic lateral ankle ligament repair for CLAI in dancers.
METHODS: From July 2015 to March 2021, arthroscopic lateral ankle ligament repair was pe

This report describes a case in which ankle-preserving surgery was performed for secondary ankle instability with good results in a patient with residual deformity after triple arthrodesis. A 42-year-old woman had undergone triple arthrodesis at the age of 16 years to correct a club foot deformity that had caused paralysis of the lower extremities. However, her left ankle instability worsened over time. Eventually, she visited our hospital, where she was found to have ankle instability and club foot. The ankle joint appeared to have a ball-and-socket shape, and plain radiographs showed a talus

CONTEXT: Recurrent trauma and altered biomechanics in those with chronic ankle instability (CAI) have been linked to altered joint loading. Previous studies revealed that patients with CAI exhibit altered joint contact force (JCF) profiles relative to uninjured individuals during walking and landing. Identifying more easily obtainable outcomes that are associated with ankle JCF in those with CAI would reduce the knowledge gap between loading profiles at the ankle joint and outcomes related to CAI.
OBJECTIVE: To quantify how ankle JCF, structural measures, postural control, and walking biomech
