Calcium channel blockers for anxiety disorders?

The role of calcium in the etiology of anxiety has been proposed for several decades. Calcium channel blockers profoundly influence calcium metabolism and the transport of calcium. Even though the evidence for the role of calcium remains weak, drugs affecting calcium might be useful in the treatment of anxiety disorders. One of these compounds, verapamil, has been used to treat mood disorders. Calcium channel blockers have also been tried in other indications such as premenstrual syndrome, irritable bowel syndrome, schizophrenia, tardive dyskinesia, and Tourette's syndrome. However, the number…

Psychosocial correlates of childhood anxiety disorders.

OBJECTIVE: This study explored psychosocial and "environmental" correlates of childhood anxiety disorders. The study examined relationships among parental psychiatric symptomatology, perceived family environment, temperament, and self-competence in children with a DSM-III-R anxiety disorder. METHOD: A community sample of third through sixth graders was screened initially for symptoms of test anxiety. Those with high and low scores were administered the Anxiety Disorders Interview Schedule for Children. Three groups (childhood anxiety disorder, test-anxious only, and normal controls) were iden…

Aetiology and mechanisms of anxiety disorders.

Theories on the aetiology of pathological anxiety abound. A critical review is given of the most influential approaches (e.g. psychoanalytical, "somatogenic", behavioural, psychophysiological and "biological" theories), with emphasis on panic and agoraphobic disorders. Evidence is presented from epidemiological and genetic research and from observations on the course of illness. A surprisingly constant prevalence of anxiety syndromes, and a familial distribution suggesting a pivotal importance of genetic factors, speak in favour of a biological model. The course of illness, however, points to

Genetics of fear and anxiety disorders.

From protozoa to mammals, organisms have been selectively bred for genetic differences in defensive behaviour which are accompanied by differences in brain and other biological functions. Studies of twins indicate some genetic control of normal human fear from infancy onwards, of anxiety as a symptom and as a syndrome, and of phobic and obsessive-compulsive phenomena. Anxiety disorders are more common among the relatives of affected probands than of controls, especially among female and first-degree relatives; alcoholism and secondary depression may also be over-represented. Familial influence…

State-Trait Anxiety Inventory (STAI) for detecting anxiety disorders in adults.

BACKGROUND: Anxiety disorders are highly prevalent mental health conditions, yet frequently remain undiagnosed. The State-Trait Anxiety Inventory (STAI) is one of the most widely used self-report questionnaires for assessing both momentary (state) and general (trait) anxiety severity. As it has also been considered for screening, it is essential to evaluate its accuracy in identifying undiagnosed anxiety disorders. OBJECTIVES: To assess the diagnostic test accuracy (DTA) of the State-Trait Anxiety Inventory (STAI) for detecting anxiety disorders in adults. We specifically aimed to determine a…

[Anxiety and anxiety disorders--differential diagnosis, etiology and medico-legal assessment].

Psychiatric disorders including anxiety disorders, are becoming more and more important in medico-legal assessment. Anxiety disorders are frequent in the general population. However, they may not be reliably diagnosed even by psychiatric experts because the disorders' symptoms may fail to manifest themselves during the examination. Functional impairments related to anxiety disorders are similarly problematic to assess. Finally, given the existence of effective therapies, it is hard to explain why patients with anxiety disorders have to be assessed at all. This report discusses the various and …

Generalized Anxiety Disorder.

Generalized anxiety disorder (GAD) is a common and disabling illness that is often underdiagnosed and undertreated. Patients with GAD are at increased risk for suicide as well as cardiovascular-related events and death. Most patients can be diagnosed and managed by primary care physicians. Symptoms include chronic, pervasive anxiety and worry accompanied by nonspecific physical and psychological symptoms (restlessness, fatigue, difficulty concentrating, irritability, muscle tension, or sleep disturbances). Effective treatments include psychotherapy (often cognitive behavioral therapy) and phar

[Generalized anxiety disorder].

Generalized anxiety disorder (GAD) is a prevalent mental condition with substantial impact on psychosocial functioning and quality of life. There is also an increased risk of comorbidity with several other mental and somatic diseases. Clinical symptomatology is characterized by excessive and uncontrollable worrying about distinct issues of daily living which is frequently associated with somatic symptoms of stress and anxiety. Neurobiological and psychological research provide evidence for alterations in (para) limbic areas, a disturbed monoaminergic transmission as well as for dysfunctional l

Social anxiety disorder.

Our understanding of social anxiety disorder (also known as social phobia) has moved from rudimentary awareness that it is not merely shyness to a much more sophisticated appreciation of its prevalence, its chronic and pernicious nature, and its neurobiological underpinnings. Social anxiety disorder is the most common anxiety disorder; it has an early age of onset--by age 11 years in about 50% and by age 20 years in about 80% of individuals--and it is a risk factor for subsequent depressive illness and substance abuse. Functional neuroimaging studies point to increased activity in amygdala and

Generalised anxiety disorder.

Generalised anxiety disorder is a persistent and common disorder, in which the patient has unfocused worry and anxiety that is not connected to recent stressful events, although it can be aggravated by certain situations. This disorder is twice as common in women than it is in men. Generalised anxiety disorder is characterised by feelings of threat, restlessness, irritability, sleep disturbance, and tension, and symptoms such as palpitations, dry mouth, and sweating. These symptoms are recognised as part of the anxiety syndrome rather than independent complaints. The symptoms overlap greatly w

Generalized anxiety disorder.

This article discusses generalized anxiety disorder in terms of its defining characteristics, its relationship to other psychiatric disorders, its psychobiology, and its treatment. Although generalized anxiety disorder is frequently encountered in general practice, it has been less systematically investigated than the more dramatic forms of anxiety disorders such as panic disorder and phobias. It is concluded that although not as sharply defined as implied by DSM-III, generalized anxiety disorder appears to be a useful diagnostic category.

Depression and anxiety among patients with somatoform disorders, panic disorder, and other depressive/anxiety disorders in Taiwan.

The aim of this study is to compare the severity of depression and anxiety in individuals with somatoform disorders, panic disorder, other depressive/anxiety disorders, and healthy controls in a Han Chinese population. According to the DSM-IV-TR-based diagnostic interviews, we recruited 152 subjects with somatoform disorders (SG), 56 with panic disorder (PG), 85 with other depressive/anxiety disorders (OG), and 179 without any psychiatric disorder (NG). The four groups reported on the Beck Depression Inventory-II (BDI-II) and Beck Anxiety Inventory (BAI) for depressive and anxiety symptoms, re…

Hospital Anxiety and Depression Scale Anxiety subscale (HADS-A) for detecting anxiety disorders in adults.

BACKGROUND: Despite being highly prevalent mental health conditions, anxiety disorders frequently go undiagnosed, prompting the use of questionnaires for anxiety screening as a potential solution. This review summarises the test accuracy of the Hospital Anxiety and Depression Scale Anxiety subscale (HADS-A) for screening purposes. OBJECTIVES: To assess the test accuracy of the HADS-A in screening for any anxiety disorder (AAD), generalised anxiety disorder (GAD) and panic disorder in adults, and to investigate how the test accuracy varies by sources of heterogeneity and across all cutoffs. Affective disorders, anxiety disorders and psychological distress in non-drinkers.

BACKGROUND: Non-drinkers have elevated levels of psychological distress but a recent study reported no elevation in prevalence of diagnosed disorders. We aimed to determine the prevalence of affective and anxiety disorders (from the CIDI-A) in current abstainers and contrast results with findings for psychological distress (K10) in the same sample. METHODS: Cross-sectional, representative household survey of adult Australians. RESULTS: Non-/occasional drinkers had higher levels of psychological distress than light drinkers, and distress in heavy drinkers was even higher. Heavy drinkers also …

Cultural aspects in social anxiety and social anxiety disorder.

To examine cultural aspects in social anxiety and social anxiety disorder (SAD), we reviewed the literature on the prevalence rates, expressions, and treatments of social anxiety/SAD as they relate to culture, race, and ethnicity. We further reviewed factors that contribute to the differences in social anxiety/SAD between different cultures, including individualism/collectivism, perception of social norms, self-construal, gender roles, and gender role identification. Our review suggests that the prevalence and expression of social anxiety/SAD depends on the particular culture. Asian cultures t
