A neonate had a globular mass bulging through the eyelids of the left eye. Computed tomography revealed a large ectatic corneal lesion and the absence of a lens. The enucleated globe revealed that the posterior surface of the ectatic cornea was lined by iris tissue, indicating corneal staphyloma.

While prolonged wear of contact lenses is possible for the aphakic patient, careful consideration must be given to patient selection and to the potential complications of such use. Currently available lenses are far from ideal and, if used at all, must be carefully monitored. The newer lenses, specifically designed for extended wear, more closely fit our needs. Several manufacturers are testing their devices under strict FDA supervision. FDA guidelines dictate a minimum of two years follow-up before a determination will be made as to which lenses are safe and effective enough for prolonged wea

The monocular and binocular vision will always be disturbed after unilateral cataract extraction in childhood. The treatment therefore not only means operation, but also correction of the optic error, pleoptics and orthoptics. In 40 cases of unilateral aphakia (12 congenital, 28 traumatical) the prognosis depends on the age of the individual and maintenance of treatment. The older the child and the sooner therapy is started the better are the functional results.

The results of investigations on the twilight vision of aphakic eyes are given. In the daylight seeing range all the subjects had satisfactory visual acuity, whereas in the mesopic range, on corresponding tests, definite difficulties were recognisable for a large proportion of the subjects.

The visual outcomes for infants 18 months or younger with cataracts have improved dramatically over the past couple of decades. Earlier detection of infantile cataract and prompt surgical removal-with subsequent visual rehabilitation with contact lenses-mean that these patients now have a much better visual prognosis. Advances in contact lens technology have led to a significantly higher success rate with contact lenses and this has been a major factor in improving the visual outcomes for aphakic infants. This review outlines the contact lens management of infantile cataract, including a detai

BACKGROUND: Exposure to ultraviolet radiation (UVR) and blue light has been shown to cause retinal damage in animals. In humans, it has been argued that the loss of short-wavelength (S) cone sensitivity during normal aging may be related to chronic light exposure.
METHODS: We measured sensitivity to blue and yellow lights presented on bright yellow backgrounds at several retinal eccentricities in both eyes of an observer with long-standing unilateral aphakia.
RESULTS: For yellow lights, where media absorption is not a major factor, there was a 2-fold loss in sensitivity of long (L) and mediu…

We retrospectively studied secondary intraocular lens (IOL) implantation in 165 aphakic patients (162 eyes) from May 1983 to August 1989. Seventy-five eyes (46.3%) had secondary IOL implantation; these included seven cases of trans-sulcus scleral fixation of the posterior chamber lens. The remaining 87 eyes could not have secondary IOL implantation because of the ocular conditions. The most common reason for secondary implantation was to relieve the discomfort caused by spectacles or contact lenses (56.2%). An anterior chamber lens was used in 43 eyes (57.3%) and a posterior chamber lens in 32

The presence of cystoid macular edema (CME) after cataract extraction in the pediatric age group could adversely affect rapid visual rehabilitation of the eye by contributing to irreversible amblyopia. Several previous studies, including one by our group, have addressed this problem. Hoyt's observations of CME in ten of 27 eyes undergoing lensectomy-vitrectomy procedures have stirred renewed interest in this topic. The current study includes 12 eyes undergoing lensectomy-vitrectomy procedures, five eyes undergoing manual extracapsular procedures (ECCE) with discission of the posterior capsule,
