The cul-de-sac approach for appendectomy.

Appendectomy through the peritoneal cul-de-sac by a low transverse incision as described herein is perfectly safe and practical. It is also easier and esthetically superior to appendectomy through other surgical approaches and could be routinely used in patients suspected of having acute appendicitis. It is particularly valuable in female patients in whom the pelvic organs may need exploration during appendectomy and for whom the postoperative esthetic result may be most gratifying.

[Causes of development of peritonitis after appendectomy].

According to the authors' data peritonitis developed after appendectomy in 122 of 18347 patients, operated upon for acute appendicitis, was an extremely grave complication of this lesion and the related surgical intervention. The main causes for development of postoperative peritonitis were local and diffuse peritonitis (101 cases), more rarely--technical and tactical errors made during the operation. Due to diagnostic difficulties a purposeful treatment for postoperative peritonitis was undertaken with a delay, reoperations were performed in late terms and not in every case either. The mortal…

Appendectomy versus antibiotic treatment for acute appendicitis.

BACKGROUND: Acute appendicitis is one of the most common emergency general surgical conditions worldwide. Uncomplicated/simple appendicitis can be treated with appendectomy or antibiotics. Some studies have suggested possible benefits with antibiotics with reduced complications, length of hospital stay, and the number of days off work. However, surgery may improve success of treatment as antibiotic treatment is associated with recurrence and future need for surgery. OBJECTIVES: To assess the effects of antibiotic treatment for uncomplicated/simple acute appendicitis compared with appendectomy…

Inferior Epigastric Artery Pseudoaneurysm Following Laparoscopic Appendectomy.

An inferior epigastric artery pseudoaneurysm is an exceptionally rare occurrence. Formation of an inferior epigastric artery pseudoaneurysm can be seen following surgical intervention and is more common after laparoscopy. A sixty-eight-year-old male presented with a right upper quadrant bulge at his incision site two months following laparoscopic appendectomy. The patient reported sudden onset of a non-reducible bulge at a 5 mm trocar incision site with minimal pain and without obstructive symptoms. Computed tomography of his abdomen and pelvis with intravenous contrast revealed a 4.2 cm pseud…

Children's Pain at Home After Laparoscopic Appendectomy.

BACKGROUND: Hospitalized children experience moderate-to-severe pain after laparoscopic appendectomy, but knowledge of children's pain experiences after discharge home is limited. Accurate pain assessments are needed to guide appropriate pain treatment. AIMS: To describe children's pain at home after laparoscopic appendectomy. DESIGN: Prospective exploratory and descriptive METHODS: A convenience sample of 100 patients, aged 10-17 years, who spoke or wrote in English or Spanish, volunteered to complete 14-day pain diaries at home after laparoscopic appendectomy. Visual analytic techniques we…

Negative-pressure pulmonary edema after laparoscopic appendectomy.

Negative-pressure pulmonary edema (NPPE) is acute-onset bilateral pulmonary interstitial edema. This condition can be caused by significant negative intrathoracic pressure generated by large inspiratory effort against acute upper airway obstruction. Postoperative NPPE is rare but potentially life-threatening if not recognized and treated promptly. This article describes a patient who developed postoperative NPPE following a laparoscopic appendectomy.

Suprapubic single-incision versus conventional laparoscopic appendectomy.

BACKGROUND: Suprapubic single-incision laparoscopic appendectomy (SSILA), a promising new approach with potential benefits such as improved cosmetic results, has been preliminarily shown to be safe and feasible in previous single-arm studies. This study used a propensity-matched analysis to compare SSILA and conventional laparoscopic appendectomy (CLA). METHODS: Patients undergoing SSILA between March 2012 and November 2013 were matched with patients undergoing CLA during the same period at a single institution. These patient groups were compared using a propensity score analysis. The model c…

Minilaparoscopic appendectomy using a new spiral needle.

BACKGROUND: Acute appendicitis is the most frequent abdominal disease and requires urgent surgery. At the present time, laparoscopic appendectomy is a well-accepted emergency procedure at most centers. In this study, we used a new spiral needle to facilitate the procedure, making it easy, minimally invasive, and cost effective. METHODS: The study included 70 patients of both sexes with acute appendicitis that was treated by laparoscopic appendectomy using a new spiral needle. These cases were treated at the General Surgery Department, Zagazig University, Egypt, from May 2012 to August 2013. I…

Single-incision laparoscopic combined cholecystectomy and appendectomy.

BACKGROUND AND OBJECTIVES: Single-incision laparoscopic surgery is becoming more widely used, but few combined procedures have been reported. Herein we share our experience with single-incision laparoscopic combined cholecystectomy and appendectomy. METHODS: We reviewed data from 26 patients who underwent single-incision laparoscopic combined cholecystectomy and appendectomy between May 1, 2009 and June 1, 2013 at Shengjing Hospital. All the procedures were performed with conventional laparoscopic instruments placed through a single operating portal of entry created within the umbilicus. RES…

Pure NOTES transvaginal appendectomy with gasless laparoscopy.

BACKGROUND: The vagina is the most widely used approach to natural orifice transluminal endoscopic surgery. However, a gas leak can significantly affect transvaginal operations during pneumoperitoneum laparoscopy. We tried to establish the proper technique for transvaginal appendectomy under gasless laparoscopy. MATERIALS AND METHODS: Five patients with chronic appendicitis were selected to receive gasless laparoscopic transvaginal appendectomy with concurrent vaginal hysterectomy. An abdominal wall-lifting device was applied after removal of the uterus, and the appendix was removed transvagi…

Perforated appendicitis: is early laparoscopic appendectomy appropriate?

BACKGROUND: Laparoscopic appendectomy for nonperforated appendicitis is associated with improved outcomes; however, laparoscopy has been challenged for perforated appendicitis owing to higher morbidity compared with open or staged procedures. The purpose of this study was to determine whether the laparoscopic approach for perforated appendicitis results in improved outcomes compared with open appendectomy. METHODS: Postoperative patient records for confirmed perforated appendicitis from 2005 to 2008 were reviewed retrospectively. Demographics, surgical approach, conversion rate, and outcomes …

Laparoscopic appendectomy: a junior trainee's learning curve.

BACKGROUND: Appendicectomy has traditionally been a training operation for junior surgical trainees. With the increased incidence of laparoscopic appendicectomy, concern has increased about the safety of this far more technically demanding procedure in the hands of junior surgical trainees. The learning curve of a junior surgeon is presented. METHODS: Consecutive patients having laparoscopic appendicectomy were studied. A 3-port Hasson technique was used. Patient demographics, conversion rate and reason for conversion, operation times, number of complicated cases (retrocecal position, dense a

Diagnosis underlying appendectomy and coeliac disease risk.

BACKGROUND: Earlier studies suggest that appendectomy is associated with a substantially reduced risk of certain types of bowel inflammation such as ulcerative colitis, particularly where the underlying diagnosis is acute appendicitis. Previous research on appendectomy and coeliac disease is inconsistent, based on small numbers with retrospective data collection, and has not differentiated between different diagnoses underlying appendectomy. OBJECTIVE: To investigate the association of diagnosis underlying appendectomy with coeliac disease. METHODS: We used Cox regression to study the risk o…

Helicobacter pylori infection in patients undergoing appendectomy.

AIMS: Helicobacter pylori has been found in the upper gastrointestinal tract; it is incriminated as aetiological factor in various pathological conditions. This prospective study assesses the presence of this microorganism in the appendix flora and the possible role of its infection in the pathogenesis of acute appendicitis. METHODS: H. pylori was investigated in 46 consecutive patients undergoing emergent appendectomy for presumed acute appendicitis. Blood sample for serological test of H. pylori infection was drawn before operation. The removed appendix specimen was stained for H. pylori; c…
