This report is a 20-year follow-up of 14 patients treated with external beam craniospinal irradiation and intrathecal gold (10-45 mCi) for medulloblastoma. Six of the patients died within 2 years of treatment from persistent disease. No patients are alive without complications. Six of eight surviving patients developed arachnoiditis and cauda equina syndrome within 5 to 10 years of treatment. Seven of eight survivors developed aneurysms and/or cerebrovascular accidents 9 to 20 years after treatment. Four of the cerebrovascular events were fatal. Intrathecal gold pools in the basal cisterns and

Altogether 61 patients with chronic spinal arachnoiditis underwent clinical and laboratory studies for deranged functions of the nervous system. The main neurologic syndromes, the results of examining cerebrospinal fluid and spondylo- and myelography data are given. At the first stage the treatment should be conservative. Surgical intervention is indicated when symptoms of compression increase.

Arachnoid intracranial cysts are benign lesions which until recently have been considered to be quite uncommon. They are benign congenital lesions usually discovered at an early age. We have recently treated two patients with advanced age and arachnoid cysts, unusual due to their location and extent. Both of them presented clinically with an organic dementia syndrome with some similarities to that seen in normal pressure hydrocephalus. Both recovered fully after surgery. We review the literature and try to emphasize the clinical differences between arachnoid and the more rare neuroepithelial c

Generally arachnoid cysts are congenital in origin and found over the cerebral convexity, in the major fissures, and in the folded portions of the brain wherever the arachnoid membrane extends. Including other central nervous system (CNS) cysts, the location of these cysts has implications as to their origin, for examples, intracerebral cysts generally are ependymal cysts, while extracerebral cysts are mostly arachnoid cysts. An 8-month-old boy was admitted to our department because of enlarged head and developmental retardation. Computerized tomographic scan (CT) revealed remarkable hydroceph

The diagnostic and therapeutic courses of 68 patients with the discharge diagnosis of spinal arachnoiditis were reviewed. The combination of oil myelography and spinal surgery was the probable cause of arachnoiditis in almost all cases. The clinical presentation featured leg pain (90%), low-back pain (80%), and sphincter disturbance (25%). Motor, sensory, and reflex changes were present in two thirds of the cases, with a majority having defects attributable to bilateral or multiple root level involvement. Specific analysis of previous operative procedures cerebrospinal fluid (CSF) studies, and

OBJECTIVE: Chronic adhesive spinal arachnoiditis (SA) is a complex disease process that results in spinal cord tethering, CSF flow blockage, intradural adhesions, spinal cord edema, and sometimes syringomyelia. When it is focal or restricted to fewer than 3 spinal segments, the disease responds well to open surgical approaches. More extensive arachnoiditis extending beyond 4 spinal segments has a much worse prognosis because of less adequate removal of adhesions and a higher propensity for postoperative scarring and retethering. Flexible neuroendoscopy can extend the longitudinal range of the

The patient was a 49-year-old woman with a chief complaint of hip and buttock pain that had persisted for 3 years. She visited our hospital for aggravation of the pain. Percussion tenderness of the spinous process was observed and a T1-low, T2-high cystic lesion was detected on the dorsal side of the dural canal at the 12th thoracic vertebral level on MRI performed by a previous physician. Plane CT showed severe scalloping at the same level. During laminectomy for the 11th and 12th thoracic vertebrae, a cystic lesion of about 60 × 25 mm was noted on the dorsal side of the dural canal, with a c

Intracranial arachnoidal cysts (AC) are relevant due to their space-demanding character. The pathophysiological sequelae are dependent on the size and location of the cyst and the patient's age. Direct pressure on surrounding tissue causes headaches (meninges) or rarely seizures (brain tissue). Cerebrospinal fluid (CSF) circulation disturbances resulting from brain mass displacement with occlusion of, for example, the foramen monroi or the aqueduct cause occlusive hydrocephalus, which can lead to an increase in intracranial pressure. Depending on age, the typical primary clinical symptoms or f

The prevalence of arachnoid cysts in children is 1-3%. They are more frequent in boys. They can be located intracranially or in the spine. Intracranial cysts are classified as supratentorial, infratentorial, and supra-infratentorial (tentorial notch). Supratentorial are divided into middle cranial fossa, convexity, inter-hemisferic, sellar region, and intraventricular. Infratentorial are classified into supracerebellar, infracerebellar, hemispheric, clivus, and cerebellopontine angle. Finally spinal arachnoid cysts are classified taking into account whether they are extra- or intradural, and n

BACKGROUND: Arachnoiditis ossificans (AO) is a rare disorder that was differentiated from leptomeningeal calcification by Kaufman and Dunsmore in 1971. It generally presents with progressive lower extremity myelopathy. Though the underlying etiology has yet to be fully described, it has been associated with various predisposing factors including vascular malformations, previous intradural surgery, myelograms, and adhesive arachnoiditis. Associated conditions include syringomyelia and arachnoid cyst. The preferred diagnostic method is noncontrast computed tomography (CT). Surgical intervention

Spinal arachnoiditis describes inflammation of the meninges, subarachnoid space and, in most cases, also involve the pial layer. The vast majority of cases described are secondary and are preceded by a known event, for example,. trauma, infections or irritative substances. Here, we present the case of primary spinal arachnoiditis. A 35-year-old lady was referred to the neurosurgical services in Dublin, Ireland with a 15-month history of progressive, right lower limb weakness. Magnetic resonance imaging revealed cystic distortion of the lumbar spinal canal extending up to the conus. Initially,

Two quadrigeminal arachnoid cysts with different pathogenesis are described in 2 different species. A 10-week-old male Persian kitten with a progressively decreasing level of consciousness died spontaneously. At necropsy, mild internal hydrocephalus, caudal cerebellar coning, and cerebellar herniation through the foramen magnum were associated with a congenital quadrigeminal arachnoid cyst compressing the rostral cerebellum and shifting the entire cerebellum caudally. In contrast, a possibly acquired quadrigeminal cyst was observed in a 2-year-old male neutered Yorkshire Terrier in association

OBJECTIVE: The purpose of our study was to examine the spectrum of abnormalities seen on MR imaging in patients with tubercular spinal arachnoiditis.
MATERIALS AND METHODS: A retrospective analysis of MR findings in 22 cases of tubercular spinal arachnoiditis was carried out. The diagnosis had been established on the basis of clinical features, evidence of associated tubercular meningitis or of tubercular spondylitis, and CSF analysis.
RESULTS: Nineteen (86%) patients had involvement of more than one spinal region, with the dorsal region being most commonly involved. CSF showed increased sig

Clinical signs and the methods of radiological diagnosis in optochiasmatic arachnoiditis are discussed. Difficulties in the interpretation of results of examinations may stem from the physiological fluctuations of the size and the form of the region and the situation of the chiasma and also from an insufficient precision of orientation of the tomographical cross-section. In spite of coordination of the angiography of carotis artery, of pneumocysternoencephalography and computer tomography one may receive faulty negative results. Presentation by means of a magnetic resonance seems to open quite

The authors studied 100 patients with rhinosinusogenic cerebral arachnoiditis who were treated in a neurosurgical clinic. Seventy-eight patients presented optochiasmal arachnoiditis: 12 had trigeminal neuralgia; 1, arachnoiditis of the cerebellopontile angle; 6, arachnoiditis of the convex surface of the brain; and 3, the hypertensive hydrocephalic syndrome due to occlusion of the CSF routes. The authors emphasize the necessity of X-ray examination of the nasal sinuses in all patients when an inflammatory intracranial process is suspected. The treatment of rhinosinusogenic cerebral arachnoidit
