Common causes of dyspnoea in athletes: a practical approach for diagnosis and management.

KEY POINTS: "Dyspnoea" during exercise is a common complaint in seemingly otherwise healthy athletes, which may be associated with fatigue and underperformance.Because dyspnoea is an general term and may be caused by numerous factors, ranging from poor aerobic fitness to serious, potentially fatal respiratory and nonrespiratory pathologies, it is important for clinicians to obtain an appropriate case history and ask relevant exercise-specific questions to fully characterise the nature of the complaint so that a targeted diagnostic plan can be developed.Exercise-induced bronchoconstriction and

Drugs affecting the respiratory system with particular reference to asthma.

In 70-80% of asthmatic patients, an attack of asthma will follow 6-10 min of strenuous exercise. The increase in airway resistance is accompanied by hyperinflation and arterial hypoxemia and makes it difficult to continue or resume exercise. The past 10 yr have seen major advances in the prevention and treatment of exercise-induced asthma (EIA). The beta-sympathomimetic drugs, when administered as aerosols, have been shown to be most effective in reversing the airway obstruction, hyperinflation, and exercise-induced hypoxemia in patients with asthma. When administered as aerosols prior to exer

The use of anticholinergics in asthma.

Anticholinergic medications have been accepted as an important treatment modality in chronic bronchitis and chronic asthma, but their use in acute asthma is more controversial. A brief historical context of anticholinergics is given. The innervations of the lung that govern bronchoconstriction and bronchodilatation are reviewed. The pharmacological and neurological properties of anticholinergics make them excellent modalities for treatment of asthma. The benefits of anticholinergics in acute asthma, exercise-induced asthma, nocturnal asthma, and psychogenic asthma are reviewed. The use of anti…

Gastroesophageal Reflux Burden Before and After Exercise Challenge in Children with Poorly Controlled Asthma: A 48-Hour MII-pH Study.

Background: Gastroesophageal reflux and exercise-induced bronchoconstriction (EIB) commonly coexist with asthma, but the effect of exercise on reflux remains unclear. We aimed to assess the prevalence of abnormal impedance-detected acid exposure in children with partially controlled or uncontrolled asthma, its association with EIB, and changes in reflux parameters after an exercise challenge test (ECT). Methods: Thirty patients aged 7-18 years underwent spirometry, treadmill ECT, and 48 h multichannel intraluminal impedance-pH monitoring. EIB was defined as a ≥10% fall in FEV1, and abnormal ac

Exercise-Induced Bronchoconstriction and the Air We Breathe.

An association between airway dysfunction and airborne pollutant inhalation exists. Volatilized airborne fluorocarbons in ski wax rooms, particulate matter, and trichloromines in indoor environments are suspect to high prevalence of exercise-induced bronchoconstriction and new-onset asthma in athletes competing in cross-country skiing, ice rink sports, and swimming. Ozone is implicated in acute decreases in lung function and the development of new-onset asthma from exposure during exercise. Mechanisms and genetic links are proposed for pollution-related new-onset asthma. Oxidative stress from

Air pollutants, bronchial hyperreactivity, and exercise.

The interaction of air pollutants and their effect on bronchial hyperreactivity show that sulfur dioxide and ozone both increase bronchial hyperreactivity. Sulfur dioxide concentrates as low as 0.5 ppm and ozone concentrations as low as 0.2 ppm have demonstrated this effect. Exercise with either sulfur dioxide or ozone exposure can significantly increase bronchial hyperreactivity in susceptible individuals. Other factors including ambient air temperature, time of day, wind velocity and direction, geography, climate, tobacco smoke exposure, and other indoor air pollutants may enhance the impact

Gender differences in the prevalence of airway hyperresponsiveness and asthma in athletes.

BACKGROUND: Although athletes have a high prevalence of airway hyperresponsiveness (AHR) and asthma, little is known about possible gender differences in regard to these features. We looked at the comparative prevalence of AHR, physician-diagnosed asthma and respiratory symptoms during exercise in female (F) and male (M) athletes. METHOD: A retrospective analysis was done on 2 groups of athletes: Group 1 (n=100) taking part in a study on the prevalence of AHR to methacholine (PC(20)<16mg/ml) and Group 2 (n=698), taking part in a provincial survey on the prevalence of physician-diagnosed asthm

Parasites and asthma in Tanzanian children.

It has been claimed that parasitic infestation suppresses the development of asthma. Eight Tanzanian schoolchildren in whom asthma was confirmed by the demonstration of exercise-induced bronchospasm were found in an ascaris-endemic area of rural Tanzania. Faecal parasites were found equally commonly in the children with asthma and the ninety-seven non-asthmatic controls. The prevalence of asthma (3.3%) in the school under study was comparable with that found in developed countries.

Time for proper physiological assessment: the role of cardiopulmonary exercise testing in helping step-up therapy in difficult-to-treat and severe asthma.

INTRODUCTION: Persistent exertional dyspnea remains a major unmet need in patients with difficult-to-treat and severe asthma, even in the era of optimized inhaled therapy and biologics. Resting lung function and inflammatory biomarkers often fail to explain exercise intolerance, leading to empirical step-up therapy without clear physiological justification. AREAS COVERED: This review discusses the role of cardiopulmonary exercise testing (CPET) in the evaluation of exertional dyspnea in difficult-to-treat and severe asthma, within the treatable traits framework. The review focuses on the main

Risk factors for respiratory symptoms and asthma in the residential environment of 5th grade schoolchildren in Taipei, Taiwan.

This study examined the association of residential environmental factors with respiratory symptoms and asthma in 2,290 fifth graders in Taipei, Taiwan. A self-report survey questionnaire elicited experiences of respiratory symptoms, disease history, and characteristics of residential environmental factors from schoolchildren. The proportion of schoolchildren having physician-diagnosed asthma was 9.8% and suspected asthma was 16.1%. The proportions of having respiratory symptoms in the past 12 months ranged from 9.8% for wheezing without a cold to 40.5% for exercise-induced cough. Higher propor

[Exercise-induced bronchoconstriction].

Terms exercise-induced asthma (EIA) or exercise-induced bronchoconstriction (EIB) are used to describe transient bronchoconstriction occurring during or immediately after vigorous exercise in some subjects. For the diagnosis of EIB it is necessary to show at least 10% decrease in FEV1 from baseline following physical exercise. The prevalence of EIB has been reported to be 12-15% in general population, 10-20% in summer olympic athletes, affecting up to 50-70% of winter athletes (particularly ski runners and skaters). There are two key theories explaining EIB: thermal and osmotic. Differential d

Management of Exercise-Induced Bronchoconstriction in Athletes.

Exercise-induced bronchoconstriction (EIB) is a phenomenon observed in asthma but is also seen in healthy individuals and frequently in athletes. High prevalence rates are observed in athletes engaged in endurance sports, winter sports, and swimming. The pathophysiology of EIB is thought to be related to hyperventilation, cold air, and epithelial damage caused by chlorine and fine particles in inspired air. Several diagnostic procedures can be used; however, the diagnosis of EIB based on self-reported symptoms is not reliable and requires an objective examination. The hyperosmolar inhalation t

Prevalence of respiratory symptoms in an athlete population.

This study aimed to look at the prevalence and type of respiratory symptoms experienced by athletes and to assess the possible influence on the perception of symptoms of training duration and environment. A group of 698 athletes (107 with diagnosed or self-reported asthma) filled out a questionnaire on their respiratory condition. They exercised either in cold air (n = 176), dry air (n = 384), humid air (n = 95) or mixed dry and humid air (n = 43). Past exercise-related symptoms reported by athletes were breathlessness (48.7%), phlegm production (22.8%), wheezing (15.6%), cough (15.2%), and ch

Exercise-Induced Lung Disease: Too Much of a Good Thing?

Exercise in children has important health benefits. However, in elite endurance athletes, there is an increased prevalence of exercise-induced bronchoconstriction and airway inflammation. Particularly at risk are those who practice in cold weather, ice rinks, swimming pools, and air pollution. The inflammation is caused by repetitive episodes of hyperventilation of cold, dry air, allergens, or toxins such as chlorine or air pollution. Children may be particularly at risk for lung injury under these conditions because of the immaturity and ongoing development of their lung. However, studies in

Special problems of the asthmatic patient.

Environmental factors and physical activity present special problems for the asthmatic patient. Bronchoconstriction due to workplace exposure is one of the most common forms of occupational lung disease and has similar pathogenic mechanisms to nonoccupational asthma. Diagnosing occupational asthma may be difficult because the association between symptoms during or after work and a causative agent may be obscure. Nevertheless, it is important to recognize occupational asthma because even trace amounts of the causative agent may trigger bronchoconstriction, and the affected employee must almost

Exercise-induced laryngochalasia: an imitator of exercise-induced bronchospasm.

BACKGROUND: Patients with exercise-induced laryngochalasia present with dyspnea and stridor during exercise. Symptoms are due to a subtotal occlusion of the larynx resulting from mucosal edema from the aryepiglottic folds being drawn into the endolarynx. METHODS: We report on three patients with exercise-induced bronchospasm, refractory to standard therapy. RESULTS: Spirometry with flow-volume loops revealed truncation of the inspiratory limb. Abnormal movement of the arytenoid region was visualized on laryngoscopy. A diagnosis of exercise-induced laryngochalasia was made. CONCLUSIONS: Eval

[Inducible laryngeal obstruction vs. bronchial asthma].

UNLABELLED: Inducible Laryngeal Obstruction vs. Bronchial -Asthma Background: Inducible laryngeal obstructions (ILO) represent paroxysmal and sometimes severe dyspnea caused by different factors. Symptomatically ILO resembles bronchial asthma and is therefore often misdiagnosed. In the following 3 cases regarding a special type of ILO, the exercise induced laryngeal obstruction (EILO) will be presented. It will also be demonstrated, how EILO can be diagnosed and differentiated from bronchial asthma. METHOD: Laryngeal symptoms were provoked by spiroergometry (treadmill or bicycle) and inspecte
