Effectiveness of Combined Peripheral Nerve Blocks Versus Isolated Greater Occipital Nerve Block in the Management of Cluster Headache.

INTRODUCTION: Cluster headache (CH) is a primary headache disorder classified under trigeminal autonomic cephalalgias, characterized by severe, disabling pain attacks. In treatment-resistant cases, where pharmacological interventions are insufficient, peripheral nerve blocks have emerged as a promising interventional option. This study aimed to compare the efficacy of isolated greater occipital nerve (GON) block with combined GON, supraorbital nerve (SON), and sphenopalatine ganglion (SPG) blocks in reducing attack frequency, pain intensity, and treatment response in patients with CH. METHODS

Non-neurodestructive ganglion impar blocks for coccydynia and related disorders: a systematic review and meta-analysis.

BACKGROUND/IMPORTANCE: Chronic coccydynia is a challenging condition to manage. Conflicting evidence exists regarding the role of the ganglion impar in coccygeal nociception. When conservative treatments fail, minimally invasive interventions at the ganglion impar may be effective in providing relief. OBJECTIVES: To evaluate the effectiveness and safety of ganglion impar blocks (GIBs) for the management of chronic coccydynia. EVIDENCE REVIEW: A systematic review and meta-analysis was conducted following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Relevant s

[Stellatum ganglion block and post-traumatic stress disorder].

PTSD treatment in Denmark is difficult, with many treatment-resistant patients and a high dropout rate from conventional treatments. This review describes a newer supplementary therapy, stellatum ganglion block (SGB), which may prove to have a good effect by; 1) reducing patients' hyperarousal symptoms such as flashbacks, palpitations and anxiety, and 2) improving patients' ability to process intrusive memories due to increased hippocampal activity, and 3) leading to better compliance with behavioral therapy such as trauma-focused psychotherapy or EMDR.

Regional anesthesia for arrhythmias: a review of current literature.

PURPOSE OF THE REVIEW: The aim is to provide a comprehensive review of regional anesthesia techniques to control ventricular arrhythmias. RECENT FINDINGS: While promising, the use of stellate ganglion block (SGB) for arrhythmia control is still under investigation, and further clinical trials are warranted to fully understand its efficacy, long-term outcomes, suitable patient group, and safety profile. Nevertheless, it remains a potential adjunctive therapy in the management of ventricular arrhythmias in select patients. Continuous stellate ganglion block (C-SGB) offers an alternative to sing

[Percutaneous stellate ganglion block: the technique which is changing the way to deal with electrical storm].

Electrical storm represents a significant challenge in the management of cardiological emergencies, with a high mortality rate. The therapeutic armamentarium is limited, and the efficacy of available drugs is not always consistent. Furthermore, non-pharmacological strategies are restricted in their application and not universally accessible in medical facilities. There is a close link between the autonomic nervous system and ventricular arrhythmias, as the activation of the sympathetic nervous system can modify the electrophysiological characteristics of the substrate and increase triggered ac

Stellate Ganglion Blocks for Post-COVID-19 Headaches: Case Report.

BACKGROUND: Long COVID can impact patients with a wide range of new, returning, or ongoing health symptoms, lasting for months to years despite recovery from the acute infection. One of the symptoms is headaches. Post-COVID-19 headaches can affect the quality of life for many patients, and as a result, the management and treatment of the symptoms have become important issues. CASE REPORT: We present a case series of patients with varying presentations of post-COVID-19 symptoms, including headaches, who responded positively to the use of stellate ganglion blocks (SGBs). CONCLUSIONS: We report

Stellate ganglion block in the treatment of SAPHO syndrome: A case report.

Synovitis, acne, palmoplantar pustulosis, hyperostosis, and osteitis (SAPHO) syndrome is a rare and refractory autoinflammatory disease, and there is no consensus on its treatment. Stellate ganglion block (SGB) blocks sympathetic nerves, ameliorates immune dysfunction, and alleviates stress response, which has been used to treat various chronic pain syndromes, arrhythmias, and post-traumatic stress disorder (PTSD). Also, the SGB has been reported to be successfully used to treat certain skin diseases, autoinflammatory diseases, and menopausal symptoms. In this study, over 3 years of follow-up,

Real-time ultrasound-guided stellate ganglion block for migraine: an observational study.

OBJECTIVE: To observe whether ultrasound-guided stellate ganglion block (SGB) can effectively relieve migraine pain and improve the quality of migraine patients' life. METHODS: 81 patients with migraines were enrolled in this study. The patients received SGB with 6 ml of 0.15% ropivacaine once every week for four times. Migraine was assessed with the Migraine Disability Assessment Scale (MIDAS) at baseline and three-months follow-up (Tm). The numerical rating scale (NRS) score at baseline, one day after treatment (Td) and Tm, the frequency of analgesic use in 3 months and the side effects wer

Sympathetic Blocks for Visceral Pain.

For patients with chronic pain or cancer-related pain, the most common indication for sympathetic block is to control visceral pain arising from malignancies or other alterations of the abdominal and pelvic viscera. When it is recalcitrant to conservative care, or if the patient is intolerant to pharmacotherapy, consideration of sympathetic blocks or neurolytic procedures is considered. Potential advantages of a neurolytic procedure, compared with spinal and epidural anesthetic infusions, include cost savings and avoidance of hardware. Interventional therapies that target afferent visceral inn

Stellate Ganglion Blockade for Refractory Ventricular Arrhythmias: Implications of Ultrasound-Guided Technique and Review of the Evidence.

Refractory ventricular arrhythmias (VAs) carry high mortality rates despite electrical and pharmacologic therapy utilization. These patients often require aggressive hemodynamic support, including mechanical circulatory devices such as extracorporeal membrane oxygenation because of progressive hemodynamic and metabolic deterioration. Sympathetic nervous system stimulation and neuronal remodeling after myocardial insults have been implicated as drivers of refractory VAs. This understanding has led to interest in and a growing body of experience with percutaneous blockade of the stellate ganglio

The New Possibility of Lymphoscintigraphy to Guide a Clinical Treatment for Lymphedema in Patient With Breast Cancer.

OBJECTIVES: Lymphedema is a chronic disease that occurs frequently after breast cancer treatment. For the treatment of lymphedema, surgical approach such as vascularized lymph node transfer and super-microscopic lymphovenous anastomosis surgery is well established. Complex decongestive therapy (CDT) and sympathetic ganglion block (SGB) are two non-surgical therapeutic options. This study aimed to evaluate the effectiveness of lymphoscintigraphy in guiding the selection of the optimal non-surgical treatment in lymphedema. METHODS: A total of 81 lymphedema patients who underwent lymphoscintigra

Prevention of Post-herpetic Neuralgia from Dream to Reality: A Ten-step Model.

Herpes zoster (HZ) is a painful, blistering skin eruption in a dermatomal distribution caused by reactivation of a latent varicella zoster virus in the dorsal root ganglia (DRG). Post-herpetic neuralgia (PHN) is the most common complication of acute herpes zoster (AHZ).Severe prodrome, greater acute pain and dermatomal injury, and the density of the eruption are the risk factors and predictors for developing PHN. PHN has a substantial effect on the quality of life; many patients develop severe physical, occupational, social, and psychosocial disabilities as a result of the unceasing pain. The

[Pain in chronic pancreatitis and pancreatic cancer - treatment options].

Treatment of pain is one of the main pillars of treatment of pancreatic diseases. Abdominal pain is a common and often debilitating symptom in patients with chronic pancreatitis and pancreatic cancer. Treatment involves abstinence from tobacco, alcohol and analgetics and adjunctive agents. Surgical and endoscopic treatment requires careful patient selection based on a detailed analysis of ductal anatomy. The limited possibilities of this therapy are patients without dilatation of the main pancreatic duct. Results of randomized trials suggest that the effect of surgical treatment is sustained o

Recurrent migratory sympathetically maintained pain syndrome in a child: a case report.

A 10-year-old boy presented with pain and swelling of short duration in his right hand. A traumatic cause was ruled out. He had clinical features suggestive of sympathetic overactivity in the form of hyperesthesia, edema and dry skin. The results of a few baseline investigations performed at this stage were all within normal limits. A provisional diagnosis of sympathetically maintained pain syndrome (SMPS) was made. He responded dramatically to a stellate ganglion block. His symptoms recurred within 3 weeks, though now in the lower limb. Again the clinical features were consistent with SMPS. H
