As arctic warfare becomes a center focus within Special Operations, cold weather injury looms as both a medical and operational threat. While cold weather injury can range from pernio to hemodynamically unstable systemic hypothermia, the more minor injuries are far more common. However, these present a challenge in austere medical care and can drastically impact mission capability. We present a case of a Special Operations crew chief with cold weather digital injury while at the Arctic Isolation Course in Alaska and his subsequent clinical course. Prevention remains the key for mitigating thes

BACKGROUND: The applicants' self-declaration of medical history is crucial for safety. Some evidence indicates that under-reporting of medical conditions exists. However, the magnitude in a population of aviation personnel has not been reported earlier.METHODS: A total of 9941 applicants for medical certificate/attestation for aviation-related safety functions during the last 5 yr up to December 2019 were registered at the Civil Aviation Authority Norway. E-mail addresses were known for 9027 of these applicants, who were invited to participate in a web-based survey.RESULTS: Among the 1616 resp…

Acute hypobaric hypoxia (HH) is a major physiological threat during high-altitude flight and operations. In military aviation, although hypoxia-related fatalities are rare, incidences are common and are likely underreported. Hypoxia is a reduction in oxygen availability, which can impair brain function and performance of operational and safety-critical tasks. HH occurs at high altitude, due to the reduction in atmospheric oxygen pressure. This physiological state is also partially simulated in normobaric environments for training and research, by reducing the fraction of inspired oxygen to ach…

Aviation operations in Alaska often occur in remote locations and during inclement weather. Limited infrastructure and staff in some locations often requires aviation workers to perform tasks outside of their specific job descriptions. Researchers identified workers' compensation claims as a valuable data source to characterise nonfatal injuries among Alaskan aviation workers. Keyword searches of injury claim narrative fields and industry codes were used to identify potentially aviation-related workers' compensation claims during 2014-2015. These claims were manually reviewed to verify whether…

Sertraline (Zoloft) is a selective serotonin reuptake inhibitor that is a commonly prescribed drug for the treatment of depression, obsessive-compulsive disorder, panic disorder, social anxiety disorder, premenstrual dysphoric disorder and post-traumatic stress disorder. Although the use of sertraline is relatively safe, certain side effects may negatively affect a pilot's performance and become a factor in an aviation accident. The authors' laboratory investigated the distribution of sertraline and its primary metabolite, desmethylsertraline, in various postmortem tissues and fluids obtained …

This collective review has the following purposes. First we will describe each of the components of the Maryland State Police Aviation Division. We will then provide detailed information about the Atlas and Database of Air Medical Services (ADAMS) compiled by the center for transportation and injury research (Buffalo, New York) in alliance with the Association of Air Medical Services (Alexandria, Virginia) that provides a unique opportunity for each state to evaluate the comprehensive nature of their air medical services. The concept "the golden hour" has been molded into the world's premier p…

The statutory authority and responsibility of the National Transportation Safety Board in the investigation and determination of the probable cause of aviation accidents is described. The Independent Safety Board Act of 1974 acknowledges the importance of obtaining data regarding trauma sustained in an accident by survivors, as well as those persons injured fatally. Information regarding occupant injuries, coupled with extensive examination of the aircraft wreckage, is necessary in order to identify hazards which inflicted the injuries. The human factors investigator is responsible for reporti…

Civil aviation medicine and cardiothoracic surgery intersect at a point where individual clinical outcomes and public safety are tightly coupled. Professional aircrew and other safety-critical aviation personnel must meet legally defined medical standards that are designed around an engineering approach to risk, including a low annual tolerance for sudden incapacitation, whereas passengers and cabin crew are exposed to the physiological constraints of the flight environment (hypobaric hypoxia, gas expansion, limited access to medical care, immobility, and circadian rhythm disruption). Yet, the…

This paper aims to develop an on-board shock absorber detection method for general aviation aircraft. The effects of common gas and oleo leakage are analyzed in this paper. Based on the principle of landing gear dynamics, it is found that gas leakage and oleo leakage would mainly affect air spring force of shock absorbers in various ways. A rigid-flexible coupled landing gear multi-body system (MBS) model is developed by considering strut flexibility, aiming to offer more accurate simulated responses. A database is developed that considers common leakage faults and typical landing conditions u…

Sustainable aviation fuel (SAF) could reduce aviation's CO2 and contrail climate forcing. This study quantifies the contrail mitigation potential and fuel supply chain costs of a uniform SAF distribution scenario, assuming all departing flights use a 10% SAF blend by mass. Building on this, we propose three SAF allocation strategies that optimize the same SAF supply to maximize contrail mitigation, while considering real-world supply chain constraints and additional costs. A seasonal strategy - providing SAF to all flights from October to February at higher blend ratios (28%) - achieves the hi…

Aviation has been shown to cause high particle number concentrations (PNC) in areas surrounding major airports. Particle size distribution and composition differ from motorized traffic. The objective was to study short-term effects of aviation-related UFP on respiratory health in children. In 2017-2018 a study was conducted in a school panel of 7-11 year old children (n = 161) living North and South of Schiphol Airport. Weekly supervised spirometry and exhaled nitric oxide (eNO) measurements were executed. The school panel, and an additional group of asthmatic children (n = 19), performed dail…

INTRODUCTION: The Federal Aviation Administration (FAA) Office of Aerospace Medicine sets medical standards needed to protect the public and pilots from death or injury due to incapacitation of the pilot. As a part of this process, toxicology testing is performed by the FAA on almost every pilot who is fatally injured in an aviation accident to determine the medical condition of the pilot, medications used by the pilot at the time of the accident, and the extent of impairment, if any.
METHOD: The data were extracted from the FAA toxicology database for all pilots who died from 2004 to 2008 in…

INTRODUCTION: The number of aircraft accidents remains on a constant level since the late 1990s. Routine analysis in detail of the causative factors is not carried out in Germany. The analysis of flight mishaps has been demonstrated to be an important basis for flight safety. The Human Factors Analysis and Classification System (HFACS) model is best suitable for aircraft accident analysis. The aim of this study was to classify aircraft accidents in the General Aviation (GA) of Germany according to the HFACS model and to figure out the underlying causes.

INTRODUCTION: Selective serotonin reuptake inhibitors (SSRIs) are popularly prescribed for treating depression. With a few exceptions, these psychotropic medications are not approved by aeromedical regulatory authorities for use by aviators. Since SSRIs have the potential for impairing performance and causing drug-drug interactions, the prevalence of SSRIs in pilot fatalities of civil aviation accidents was evaluated.
METHODS: Postmortem samples from pilots involved in fatal civil aircraft accidents are submitted to the Civil Aerospace Medical Institute (CAMI) for toxicological evaluation. Fi…

HYPOTHESIS: The unknown prevalence of alcohol use and misuse among aviation pilots, crewmembers and associated support personnel call for continuous improvement of methods for detecting recent alcohol use. Early detection is essential to proper treatment and prevention of potentially catastrophic mishaps. Urinary 5-hydroxytryptophol (5HTOL), a serotonin (5HT) metabolite, has shown promise in the clinical setting as a noninvasive marker of recent alcohol consumption.
METHODS: The urinary 5HTOL concentrations of 11 male and female subjects were followed for approximately 24 h following dosing w…

Military aviation exposes aircrew to interacting physiological and operational stressors that are often studied in isolation. These stressors are experienced by the whole body: pulmonary, cardiovascular, musculoskeletal, endocrine, and immune responses may influence cerebral perfusion, blood-brain barrier function, fatigue, and recovery. Evidence from aviation cohorts, altitude-chamber studies, and adjacent fields suggests that some exposures may converge on neurovascular, blood-brain barrier, perivascular, and inflammatory pathways before overt neurological disease is apparent. This narrative…

INTRODUCTION: Military aviators have long undergone enhanced medical screening to minimize accidents and deaths. U.S. Army aviators undergo a rigorous initial screening process followed by annual medical evaluations governed by published standards of medical fitness which are updated periodically. An aeromedical summary is submitted for disqualifying conditions, resulting in either a waiver of the standard or suspension of flight status. This study aimed to identify the most common disqualifying medical conditions in U.S. Army aviators in recent years and analyze trends over time.
