BACKGROUND: Reoperative sternotomy often necessitates peripheral arterial cannulation to initiate systemic or cerebral perfusion. In patient … involved anastomosis of a Dacron side-arm graft at the prior site or immediately adjacent along the axillary artery.
RESULTS: In 55 patients (mean age, 58.9 years; 72.7% male), redo axillary cannulation was performed a median of 4.6 years after the original operation. The most common indications for reoperation were aneurysm (60.0%) and structural valve degenera …

BACKGROUND: The aim was to analyze the anatomic feasibility of the percutaneous axillary access (PAXA) using cadaverous models and then to a … s were performed on cadavers. The axillary vein was injured twice (18.2%); the puncture site on the axillary artery was found on the arterial proximal part, behind the clavicle. Fifty-three patients underwent a F/BEVAR using a PAXA. The mean (SD) age of patients was 74.5 (9.7) years. Most indications for endovascular repair were para-renal aneurysms (66%). Two Pr …

BACKGROUND: This study seeks to assess the outcomes of direct axillary artery (AX) cannulation for thoracic aortic surgery.
METHODS: From October 2009 to November 2021 direct AX cannulation was planned in 515 patients for thoracic aortic pathology. An important aspect of our technique is that the cannula is not inserted deeper than 3 cm. AX cannulation-related events included shift of cannulation site from the initial site, vascular injury, and iatrogenic dissection.
RESULTS: Half of the patients had acute type A dissection (ATAD). An angled cannula was used in 442 patients and a straight ca…

To investigate postoperative pectoral atrophy in 141 patients undergoing aortic arch surgery involving bilateral axillary artery cannulations with side grafts. The depth from the skin to the axillary artery surrounding the thoracoacromial artery (zone 1), and the thicknesses of pectoralis major (zone 2) and pectoralis minor (zone 3) were measured by computed tomography before surgery, at 1 and 6 months after surgery, and at the most recent follow-up assessment (PostT2) (mean = 41 months, range 11-75 months). Based on the median value (47.4 mm) of zone 1, the preoperative pectoral thickness was…

Pseudoaneurysm formation caused by iatrogenic arterial injury during a regional anesthetic block is a rare complication. We report a case of a 56-year-old male patient who developed an axillary artery pseudoaneurysm caused by brachial plexus block performed for an upper extremity dialysis access operation. Successful repair of this pseudoaneurysm was achieved with endovascular stent graft exclusion. The repaired axillary artery with the stent graft remained patent after 10 years of follow-up. The successful long-term patency of this treatment and a strategy to potentially avoid this complicati…

Arterial cannulation into the right axillary artery is a commonly adopted perfusion strategy in the treatment of acute aortic dissection. Here we describe our experience of accidentally cannulating the axillary artery in a case of acute aortic dissection with an aberrant right subclavian artery, which was missed preoperatively because its proximal segment was malperfused by the dissection and thereby not enhanced. The rapid hemodynamics collapse at the start of the bypass was reversed by prompt switching to femoral perfusion. Postoperative follow-up computed tomographic angiography revealed a …

INTRODUCTION: Retrograde arterial dissection is a recognized complication of endovascular intervention but has not been well reported especially in the context of dialysis arteriovenous access procedures. The management of this complication is also not well defined.
CASE PRESENTATION: We report the case of an 80-year-old female with end-stage renal disease (ESRD) on dialysis who developed an asymptomatic retrograde dissection of the left axillary artery during an arteriogram performed via a retrograde approach as part of access dysfunction evaluation. The condition was managed conservatively …

OBJECTIVES: Descriptions of the variant arterial pattern of upper limb are not exceptional and are therefore frequently reported in anatomy archives.
BACKGROUND: A noteworthy deviation from the usual branching pattern was observed unilaterally in a single cadaver. This unique division of axillary artery (AA) was present on the right side in an adult human cadaver of Indian origin.
RESULTS: The first part of axillary artery gave off the superior thoracic and thoraco-acromial arteries. Just proximal to the upper border of pectoralis minor the AA was observed to divide into two trunks a medial …

The efficacy of covered stents for treatment of upper extremity pseudoaneurysms (PSA) has not been well documented. We report a case of an 82-year-old woman who presented with a hematoma in the left infraclavicular region as a result of previous permanent pacemaker placement. Subsequent ultrasound and chest computed tomography confirmed an axillary artery PSA. An endovascular approach enabled successful coil embolization and Viabahn stent (W. L. Gore & Associates, Flagstaff, AZ) placement, which excluded the axillary artery PSA. To our knowledge, there is only one other case of this nature rep…

Evolution of endoluminal technologies, enhanced stent designs, and smaller delivery systems have increased the potential for using covered s … a competitive option to open surgery. We report a patient with an unusual mechanism for a traumatic axillary artery pseudoaneurysm that was repaired using a polytetrafluoroethylene-covered stent-graft.

PURPOSE: The gradient between temperatures measured at different body sites is not constant; one factor which will change this gradient is rapid changes in body temperature. Measurement of this gradient was done in patients undergoing rapid changes in body temperature to establish the best site to measure temperature and to compare two brands of commercial tympanic thermometers.
METHOD: A total of 228 sets of temperatures were measured from probes in the oesophagus, rectum, and axilla and from two brands of tympanic thermometer and compared with pulmonary artery (PA) temperature in 18 adults

Review of a case involving simultaneous management of injuries to the brachial, axillary, and subclavian arteries stresses the need for arteriography in suspected subclavian injuries and the need for knowledge of the operative exposures available to the operating surgeon.

The frozen elephant trunk technique is increasingly used in total arch replacement (TAR) for acute Type A aortic dissection, with left axillary artery bypass commonly employed for left subclavian artery revascularization. We report a case in which an intrapleural left axillary artery Dacron bypass graft led to dense perigraft adhesions that severely compromised subsequent left internal thoracic artery (LITA) harvesting during redo coronary artery bypass grafting. A 45-year-old man underwent valve-sparing aortic root reimplantation, TAR, and left axillary artery bypass. Intraoperative findings …
