Klippel-Feil syndrome associated with congenital cervical dislocation: report of an autopsy case.

Klippel-Feil syndrome is an uncommon congenital anomaly that is characterized by abnormal fusion of the cervical vertebrae and occasionally accompanied by various anomalies of other bones and internal organs. We report the autopsy case of a 5-year-old girl with this syndrome ssociated with congenital cervical dislocation, with special reference to the pathological findings of the vertebral column and spinal cord. Principal anomalies of the cranio-spinal axis were as follows: partial defect of the clivus, scoliosis, hypoplasia of the whole cervical vertebrae, anterior dislocation of C7 with S-s

Atlas fractures: treatment and long-term results.

Fractures of the first cervical vertebra (C1) represent approximately 7% of all acute cervical spine fractures. Frequently, the atlas will be fractured in combination with the axis (44% of cases). Treatment of isolated C1 fractures should be governed by the rules of Spence. The treatment of combination C1-C2 fractures is dictated by the type and severity of the C2 fracture. Experience with 57 cases of acute traumatic atlas fractures are reviewed. The long-term outcome from an atlas fracture is good (median follow-up, 40 months).

C-2 arteriovenous fistula presenting as a pathologic hangman's fracture: case report.

The authors report on an 81-year-old woman with a pathologic hangman's fracture secondary to a complex arteriovenous fistula (AVF). The patient presented with severe, unremitting neck pain and was found to have fractures bilaterally through the pars interarticularis of C-2 with significant anterior subluxation of C-2 over C-3 along with widening of the left transverse foramen. Due to an abnormally appearing left vertebral artery (VA) on CT angiography, the patient underwent conventional angiography, which revealed a complex AVF stemming from the left VA at the level of C-2 with dilated posteri

Combined injuries in the upper cervical spine: clinical and epidemiological data over a 14-year period.

Concomitant traumatic injuries in the upper cervical spine are often encountered and rarely reported. We examined the data concerning 784 patients with cervical spine injuries following trauma, including 116 patients with upper cervical spine injuries. Twenty-six percent of patients with upper cervical spine injuries (31 cases) were found to have combined injuries involving either the upper or the upper and lower cervical spine. The frequent patterns were combined type I bipedicular fracture of the axis and dens fracture, and combined dens fracture and fracture of the posterior arch of C 1. Ot

A practical classification of acute cervical spine injuries.

This article presents a practical classification of acute cervical spine injuries based on terminology with generally accepted and understood definitions, published results of experimentally produced acute injuries of the cervical spine, and the correlation of the radiographic and pathologic characteristics of the laboratory models with acute clinical injuries.

Patterns of growth in the presacral vertebral column of the leopard gecko (Eublepharis macularius).

Postnatal growth patterns within the vertebral column may be informative about body proportions and regionalization. We measured femur length, lengths of all pre-sacral vertebrae, and lengths of intervertebral spaces, from radiographs of a series of 21 Eublepharis macularius, raised under standard conditions and covering most of the ontogenetic body size range. Vertebrae were grouped into cervical, sternal, and dorsal compartments, and lengths of adjacent pairs of vertebrae were summed before analysis. Femur length was included as an index of body size. Principal component analysis of the vari

Subaxial injuries.

Injuries to the subaxial cervical spine must be suspected in any patient who suffers a head injury or complains of neck pain or neurologic symptoms of the arms or legs following an accident, particularly a motor vehicle or diving accident. Careful neurologic examination and lateral roentgenograms are indicated in all patients with suspected injury. If there is any neurologic deficit, fracture, or dislocation seen on roentgenogram, skull-traction tongs should be applied to provide stability and prevent further damage. If the neurologic examination and roentgenograms are normal, a stretch-test r

Spectrum of the hangman's fracture.

The authors review their experience in managing 26 cases of "hangman's fracture." The basic mechanism of injury was hyperextension of the upper cervical spine. Radiographic studies revealed a spectrum of injury beginning with the classical hangman's fracture and progressing to the simple C-2 laminar-pedicle fracture. Appreciation of the fracture instability will dictate the method and duration of fracture treatment. Management by a closed, nonoperative means resulted in solid union of the bone in all fully treated cases, with a minimum of morbidity.

What is the best treatment option for cervical spinal cord injury by os odontoideum in a patient with athetoid dystonic cerebral palsy?

Context: Atlantoaxial instability in an athetoid dystonic cerebral palsy patient due to os odentoideum represents a rare cause of high-level cervical spinal cord injury. There is no evidence-based treatment protocol for this injury and a number of reports have debated whether nonsurgical or surgical treatment is the best option.Findings: Here, we report the case of a 32-year-old athetoid dystonic cerebral palsy patient with os odontoideum (OO) causing compressive myelopathy on the C1-2 levels. About two weeks after atlantoaxial fusion, the patient started an intensive rehabilitation program an

[Diagnosis of hyper- and hypomotility of the upper cervical spine using functional computerized tomography].

At present, there are few methods that are of any assistance in evaluating the ligaments of the upper cervical spine. It is obvious that a lesion of these ligaments could result in instability of the cranial cervical junction. Nine healthy adults and 45 patients with suspected rotatory instability of the upper cervical spine after trauma were examined using functional CT-scans. After CT scans of the upper cervical spine had been obtained with the patient in a neutral position, the head was passively rotated to the left and right and held in these positions by adhesive tape. CT scans were perfo

[Changes in the cervical spine in juvenile rheumatoid arthritis (author's transl)].

Changes in the cervical spine in juvenile rheumatoid arthritis can be shown by A.P., lateral and functional views with maximal flexion and extension, and particularly by tomography. The latter is essential for proper evaluation of the atlanto-occipital and atlanto-axial joints and of the odontoid. Atlanto-axial subluxation was found in eight out of eighty-four patients. Destruction of the odontoid was found in twelve out of fifty-three patients examined by tomography. The earliest tomographic changes at the atlanto-occipital joint consists of isolated joint narrowing, erosions and fusion and w

Traumatic vertical atlantoaxial instability in a case of atlanto-occipital coalition.

STUDY DESIGN: This is a report of a woman with an atlanto-occipital coalition who experienced a traumatic vertical atlantoaxial facet dislocation and severe associated neurologic injury. OBJECTIVE: To describe the radiographic and pathoanatomic characteristics of the injury process. SUMMARY OF BACKGROUND DATA: Severe injuries of the occipito-atlantoaxial complex are uncommon and only rarely are associated with patient survival. This is the first report of this particular injury. METHODS: Plain radiography and tomography demonstrated the facet dislocation and instability of the injury. Surgi
