Immunological abnormalities in children with biliary atresia.

Since orthotopic liver transplantation is the treatment of choice for bilary atresia, the role of nutritional support preceding this procedure is significant. The aim of this study was to assess the selected parameters of both humoral and cellular immunity before and after nutritional support. Eight children aged 1.08-7 years. with biliary atresia, qualified to LTx, received high-calorie standard diet supplemented with MCT oil. The distribution of functionally different lymphocyte subpopulations in the peripheral blood was evaluated using double color flow cytometry (EPICS-MCL, Coulter). The c…

Second operation for repair of biliary atresia.

In our experience with biliary atresia, there are few cases amenable to reoperation for recurrent jaundice. All authors would agree that specific conditions such as complete bile flow recovery from the first operation followed by early recurrence should be an unquestionable case for revision of the anastomosis, inasmuch as no biologic signs of ongoing cholangitis can be traced. The same decision would apply to the problem of bile leakage after hepatoportocholecystostomy. In other cases, however, one should be aware that these reoperations expose the child to ascitis, poor healing of the abdomi…

Hepaticportoenterostomy as surgical therapy for biliary atresia.

The Kasai I and Suruga I procedures are the most commonly reported surgical procedures used for the correction of biliary atresia. Clinical results are similar for these two procedures, but the Suruga I procedure is associated with a lower incidence of death due to ascending cholangitis. Postoperative care is an important consideration in choosing between these two procedures. The hepaticportocholecystostomy is associated with a decreased incidence of ascending cholangitis, however, it is only an option for a select group of patients. For all procedures mentioned, the level of transection of t…

A radiopharmaceutical in the diagnosis of congenital biliary atresia.

Diethyl-HIDA, a new hepatobiliary radiopharmaceutical labeled with 99mTc, was used in the preoperative diagnosis of biliary atresia and in the postoperative evaluation of hepatic-porto-enterostomy function. The good results obtained in the four first cases suggest extensive use of the method, with the aim of a more precocious and more attendable diagnosis.

Prognosis of uncorrected biliary atresia: an update.

In order to provide a baseline for comparison with the results of the Kasai hepatic portoenterostomy, follow-up information was obtained for 89 infants who underwent surgical exploration alone for biliary atresia from 1956 through 1975. Of these patients, one is apparently cured, one is alive with liver failure, three are lost to follow-up and 84 are dead. The rate of apparent cure among patients with follow-up was 1.1%. The average age at death was 12 mo, the mean 10 mo. During this same period, nine patients were operated upon for neonatal biliary obstructions other than biliary atresia, and…

Role of CD56-expressing immature biliary epithelial cells in biliary atresia.

AIM: To analyze the clinical and pathological parameters and expression of the neural cell adhesion molecule (CD56) in patients with biliary atresia (BA). METHODS: Established clinical laboratory markers of hepatic function, including enzyme activity, protein synthesis, and bilirubin metabolism, were evaluated in patients with BA and compared with those in patients with choledochal cysts and neonatal hepatitis. Pathological changes in tissue morphology and fibrosis were examined by histological and tissue collagen staining. Immunohistochemical staining for the biliary epithelial cell markers …

Biliary atresia with extrahepatic biliary cysts--cholangiographic patterns influencing the prognosis.

PURPOSE: Biliary atresia (BA) with extrahepatic biliary cysts (EHBC) has been recognized generally as "correctable" BA, which indicates a good prognosis. The variants of BA with EHBC according to cholangiographic findings and their outcomes were reviewed. METHODS: An EHBC was observed in 8 (20%) of 40 patients with BA who underwent operation at our institute. Intraoperative cholangiographic patterns included visualization of the intrahepatic bile ducts (type I BA with EHBC) in 6 patients and no visualization (type III BA with EHBC) in 2. Intrahepatic biliary cysts (IHBC) and EHBC were observe…

A prospective multicentre study evaluating the performance of the modified simple biliary atresia scoring system in predicting biliary atresia.

PURPOSE: Early diagnosis of biliary atresia (BA) is critical for best outcomes, but is challenged by overlapping clinical manifestations with other causes of obstructive jaundice in neonates. We evaluate the performance of the modified Simple BA Scoring System (SBASS) in diagnosing BA. METHODS: We performed a prospective, cross-sectional study on infants with cholestatic jaundice (June 2021-December 2022). Modified SBASS scoring was applied and compared to the eventual diagnosis (as per intraoperative cholangiogram (IOC) and liver histopathology). The score (0-6), consists of gall bladder len…

Impact of Steroid Therapy on Early Growth in Infants with Biliary Atresia: The Multicenter Steroids in Biliary Atresia Randomized Trial.

OBJECTIVE: To investigate the impact of corticosteroid therapy on the growth of participants in the Steroids in Biliary Atresia Randomized Trial (START) conducted through the Childhood Liver Disease Research Network. The primary analysis in START indicated that steroids did not have a beneficial effect on drainage in a cohort of infants with biliary atresia. We hypothesized that steroids would have a detrimental effect on growth in these infants. STUDY DESIGN: A total of 140 infants were enrolled in START, with 70 randomized to each treatment arm: steroid and placebo. Length, weight, and head…

Analysis of gut microecological characteristics and differences between children with biliary atresia and non-biliary atresia in infantile cholestasis.

INTRODUCTION: In infants with cholestasis, variations in the enterohepatic circulation of bile acids and the gut microbiota (GM) characteristics differ between those with biliary atresia (BA) and non-BA, prompting a differential analysis of their respective GM profiles. METHODS: Using 16S rDNA gene sequencing to analyse the variance in GM composition among three groups: infants with BA (BA group, n=26), non-BA cholestasis (IC group, n=37), and healthy infants (control group, n=50). Additionally, correlation analysis was conducted between GM and liver function-related indicators. RESULTS: Pri…

Osteopontin upregulation in rotavirus-induced murine biliary atresia requires replicating virus but is not necessary for development of biliary atresia.

Biliary atresia (BA) is a progressive fibro-inflammatory pediatric liver disease in which osteopontin (OPN), a glycoprotein with inflammatory and fibrogenic activity, may play a pathogenic role. The current studies were conducted in a mouse model of rotavirus-induced BA to test the hypotheses that live but not inactivated rotavirus causes antigenemia, upregulation of hepatic OPN expression, and induction of BA and fibrosis; and that OPN is necessary for development of BA. Prolonged or transient antigenemia developed in mice inoculated with live or inactivated virus, respectively, but only live…

Pediatric hepatobiliary scintigraphy: biliary atresia and beyond.

Imaging is essential in evaluating pediatric patients with suspected hepatobiliary abnormalities. While anatomic imaging is commonly requested initially, in a variety of clinical scenarios, scintigraphy further refines the differential diagnosis and provides additional necessary functional information. The essential pathologies that benefit from scintigraphy in children encompass congenital disorders, such as biliary atresia and choledochal cysts, biliary stone diseases, functional gallbladder disorders, and suspected bile leaks. This manuscript reviews the proper technique of performing hepat…

Biliary Atresia Splenic Malformation: A Case Study.

Biliary atresia (BA), a rare cholangitis, is considered a progressive inflammatory disease of the biliary tree that if left untreated can lead to cholestatic jaundice, cirrhosis, and liver failure. It remains the most common cause of chronic liver disease and the most common indication for liver transplant in children. Biliary atresia can develop concurrently with laterality defects, namely polysplenia and asplenia. This presentation is termed biliary atresia splenic malformation (BASM) and accounts for merely 10% of all cases of BA. The purpose of this article is to present a case report of a…

Robotic-assisted Kasai portoenterostomy for biliary atresia.

BACKGROUND: Robotic-assisted Kasai portoenterostomy (RAKPE) has been utilized to treat biliary atresia (BA). However, RAKPE is not widely performed and its efficacy remains unknown. We summarized the experience of RAKPE for BA and determined its efficacy. MATERIALS AND METHODS: We retrospectively analyzed 25 consecutive infants with non-syndromic type III BA who received RAKPE in our center from January 2020 to July 2021. RAKPE is a three-arm setup and four-trocar operation. Bipolar coagulation was used to dissect the small blood vessels at the hepatic portal. The fibrous cone was shallowly t…

Biliary Atresia - emerging diagnostic and therapy opportunities.

Biliary Atresia is a devastating pediatric cholangiopathy affecting the bile ducts of the liver. In this review, we describe recent progress in the understanding of liver development with a focus on cholangiocyte differentiation and how use of technical platforms, including rodent, zebrafish and organoid models, advances our understanding of Biliary Atresia. This is followed by a description of potential pathomechanisms, such as autoimmune responses, inflammation, disturbed apical-basal cell polarity, primary cilia dysfunction as well as beta-amyloid accumulation. Finally, we describe current …
