Hypertension in haemodialysis is common, prognostically important, and difficult to manage because blood pressure measurement is often imprecise, extracellular volume is dynamic, and low blood pressure may reflect heart failure, frailty, impaired forward flow, autonomic dysfunction, or excessive ultrafiltration rather than therapeutic success. Contemporary evidence supports ambulatory blood pressure monitoring as the reference standard and home blood pressure monitoring as the most practical longitudinal method, while accepted targets remain unsettled. Treatment should begin with standardised

The overall aim of this scoping review was to map the existing literature on residual placental blood volume in cesarean sections as an indirect measure of placental transfusion. Multiple databases including Pubmed, Cochrane, Embase and Web of Science were searched for relevant studies published between 1972 and 2023. All studies on cesarean sections with term pregnancies and information on residual placental blood volume were included. The PRISMA checklist was used for guiding the writing of this report. The literature search resulted in 31 studies including 8337 neonates born by cesarean sec…

AIM: The aim of this study was to determine carotid blood flow volume, a surrogate for cerebral blood flow, using Doppler ultrasound in extremely preterm infants.
METHODS: In infants <29 weeks, right common carotid artery flow volume (RCCAF) was calculated from vessel diameter and intensity-weighted mean velocity measured using Doppler ultrasound on days 1 and 3. In addition, left ventricular output (LVO), ductus arteriosus characteristics and invasive mean arterial blood pressure (MABP) were obtained.
RESULTS: Sixty infants with mean gestation of 25.8 weeks were studied. The median RCCAF in

BACKGROUND: The amount of blood detected on brain computed tomography scan is frequently used in prediction models for delayed cerebral ischemia (DCI) in patients with aneurysmal subarachnoid hemorrhage (aSAH). These models, which include coarse grading scales to assess the amount of blood, have only moderate predictive value. Therefore, we aimed to develop a predictive model for DCI including automatically quantified total blood volume (TBV).
METHODS: We included patients from a prospective aSAH registry. TBV was assessed with an automatic hemorrhage quantification algorithm. The outcome mea…

BACKGROUND: Intradialytic morbid events (IMEs) during haemodialysis (HD), including symptomatic hypotension, are related to ultrafiltration (UF)-induced hypovolaemia. Blood volume monitoring and automatic feedback control of the UF rate were developed to limit the extent of hypovolaemia during dialysis. The present study investigated the effect of blood volume (BV)-controlled UF on the incidence of HD treatments with IMEs.
METHODS: This prospective randomised crossover study included hypotension-prone patients, characterised by occurrence of IMEs in at least 33% of HD treatments during a 6-we…

Circulating blood volume is important in managing fluid balance and cardiac function after surgery under cardiopulmonary bypass. Appropriate management differs among the valve disorders, but perioperative blood volume has not yet been considered. From February 2001 to March 2003, perioperative blood volume, fluid balance, cardiac index, and left ventricular stroke work index were measured in 31 patients: 10 with aortic stenosis, 9 with aortic regurgitation, 3 with mitral stenosis, and 9 with mitral regurgitation. All immediate postoperative blood volume measurements were less than preoperative…

1. The intravenous administration of fluid and blood has to balance the danger of unexpected death in response to a reduction of central blood volume (CBV) against that of developing pulmonary and/or peripheral oedema. 2. The initial cardiovascular response to haemorrhage is similar to that developed in response to standing. In the upright position, adults are subjected to a reduction of CBV of approximately 0.5 L and can therefore tolerate a blood loss of approximately 1 L when supine. 3. However, volume administration directed by cardiovascular variables is seldom precise, even with integrat…

BACKGROUND: Despite the use of highly efficient antihypertensive drugs (AHD), blood pressure (BP) is poorly controlled in the vast majority of hemodialysis (HD) patients. Many of them show no reduction in nocturnal BP, a finding that is associated with left ventricular hypertrophy. The aim of the study was to investigate the effect of the removal of a fluid overload on BP by monitoring the ambulatory BP during 48 hours in 16 hypertensive HD patients treated with AHD. Our aim was to obtain a gradual reduction in post-HD body weight (BW) over a period of 3 to 4 months.
METHODS: During a period

The authors describe a new ultrasonographic method for analysis of global cerebral blood volume (CBV) and its application under controlled hyperventilation. CBV was determined as the product of global cerebral blood flow volume (CBF) and global cerebral circulation time. CBF was measured by duplex sonography and calculated as the sum of flow volumes in both internal carotid arteries and vertebral arteries. Extracranial Doppler assessed cerebral circulation time by determining the time interval of echo-contrast bolus arrival between internal carotid artery and contralateral internal jugular vei…

The strategy for preoperative volume treatment is not clear, partly because of the difficulty in monitoring the central blood volume (CBV). The cardiovascular responses to hypovolaemic shock are reviewed with emphasis on the often very low heart rate, which develops when the CBV is reduced by about 30%. A proposal has been put forward that normovolaemia is the CBV that does not limit cardiac output. The possibility of monitoring CBV by electrical impedance is also reviewed. It is concluded that the deviations reflect the CBV accurately in both experimental and clinical studies. A strategy that…

BACKGROUND AND PURPOSE: A noninvasive reliable technique that can reveal cerebral blood flow volume could be a valuable tool in screening programs for stroke prevention. In diagnostic ultrasonography, spectral Doppler imaging (SDI) is popular among sonologists and vascular technologists to estimate blood flow volume despite its documented inaccuracy and the availability of the more accurate technique of color velocity imaging (CVI). The aim of the present study was to demonstrate the discrepancy of blood flow volume estimation with CVI and SDI with use of an "internal" standard.
METHODS: The

The total circulating red cell volume (RCV) is a better guide to the oxygen-carrying capacity of the blood in the whole circulation than is the haemoglobin concentration (Hb) or haematocrit in a blood sample. Pre- and post-transfusion RCV (and blood volume (BV)) may be determined by flow cytometry by exploiting antigen differences between transfused donor red cells and the recipient's red cells. This paper describes the use of red cell antigen differences of Duffy, Kidd, MN and RhD between donor and recipient. In 20 infants, transfused on 21 occasions, pretransfusion RCV ranged from 12 to 39 m…

OBJECTIVE: To test a prototype hydraulic, non-invasive, continuous finger blood pressure monitor based on the volume-clamp principle for procedure-related factors likely to influence precision.
DESIGN: The influence of these factors was determined by repeatability of finger blood pressure measurement and the relationship to contralateral arm-cuff blood pressure.
METHODS: Repeated blood pressure measurements from three different fingers were made in 60 subjects following re-initialization of the device and re-insertion of the finger into the cuff. Repeatability was assessed in relation to sim

The distribution of extracellular fluid over the intra- and extravascular spaces was determined in hypokalaemic and normokalaemic patients. In six patients with Bartter's syndrome, four with pseudo-Bartter's syndrome, and twenty with essential hypertension (EH) chronically treated with chlorthalidone, serum potassium (serum K+) and extracellular fluid volume (ECFV) were decreased, while plasma volume (PV) and blood volume (BV) were normal (see Table 1 for means, standard deviations, and levels of significance). Consequently, the ratio of BV to interstitial fluid volume (IFV) was increased. In …

During a 24-month period, 5,625 blood culture specimens were collected at the Seattle Veterans Administration Medical Center in 20-ml volumes and divided into separate 10-ml aliquots. The two aliquots were processed as duplicate sets (set 1, set 2) by the BACTEC system (Johnston Laboratories, Inc., Towson, Md.). Specimens (5 ml) from each set were inoculated into aerobic (6B) and anaerobic (7C/7D) vials. A total of 434 significantly positive blood cultures were found. In 342 of these positive cultures, yielding 379 isolates (112 members of the family Enterobacteriaceae, 104 staphylococci, 87 s

In normal human pregnancy there is considerable expansion of both plasma volume and total extracellular fluid volume, together with marked diminution of vascular reactivity in response to intravenous infusion of vasopressor agents. Pregnancy-associated hypertension (pre-eclampsia) is characterised by increased vascular reactivity, vasoconstriction and a fall in plasma volume associated with a shift of fluid from intravascular to interstitial fluid space. Direct expansion of plasma volume in patients with pregnancy-associated hypertension leads to partial correction of the abnormalities of volu

We used an emission tomographic brain scanner to investigate the relationship between local cerebral blood volume (LCBV) and arterial blood carbon dioxide tension (PaCO2) in normal awake man. Measurements were made separately in three dimensions in various regions of grey and white matter, and the resting LCBV as well as the difference in sensitivity among these regions were compared. Over the range of PaCO2 studied (20-42 torr), the response of both the grey matter and the white matter to carbon dioxide was linear. The LCBV sensitivity of the grey matter to changes in PaCO2 was 0.053 ml/100 g…
