OBJECTIVE: This exploratory study sought demographic and clinical correlates of self-mutilation (self-injury without suicidal intent) in borderline personality disorder.
METHOD: Among 124 consecutively admitted inpatients with borderline personality disorder, there were 62 who did not mutilate themselves, 23 who mutilated themselves infrequently (fewer than five lifetime events), and 39 who mutilated themselves frequently (five or more lifetime events); each received ratings on numerous measures of psychopathology.
RESULTS: Compared to nonmutilators, frequent mutilators were significantly mo…

OBJECTIVE: To compare the efficacy of a neuroleptic (haloperidol) to a monoamine oxidase inhibitor antidepressant (phenelzine sulfate) again … replicate prior reports of efficacy for the neuroleptic.
CONCLUSIONS: Pharmacologic dissection of borderline personality disorder patients into affective and schizotypal subtypes could not be demonstrated.

Eleven "pure" borderlines, ten borderlines with depression, 16 "pure" depressives, and 31 normal subjects were compared on a number of standardized inventories of anxiety. While patient groups experienced more anxiety of all types than did normals, borderlines did not emerge as more anxious than other patient groups. Qualitative differences in the anxiety experienced by borderlines and nonborderlines are discussed.

Although the diagnosis of borderline personality disorder has reasonable interrater and test-retest reliability, its discriminative validity is poor. The authors review studies indicating that the patient group subsumed by the borderline category actually includes those with various affective, organic, and schizotypal disorders. They speculate that borderline personality disorder represents a broad range of affective disorders resulting in part from childhood object loss, which may impair the reinforcement mechanisms necessary for the development of competent object relations.

OBJECTIVE: The schema mode model offers a new conceptualisation of complex dissociative disorders (CDD) as it explains shifts between identities as shifts between schema modes. Furthermore, in this model CDD is conceived as personality pathology, incorporating core features of personality disorders. This study tested the assumptions of this schema mode model of CDD.
METHOD: Questionnaires measuring personality disorder traits, schemas, schema modes and coping styles were filled out by patients with CDD, borderline personality disorder and avoidant personality disorder (N = 210), and their sco…

Borderline personality disorder (BPD) is a serious mental illness that despite evidence and research showing validity in adolescents, is often not diagnosed or misdiagnosed in this population. Reasons for this include misinformation about the age requirement in the diagnostic criteria, myths about treatability and adolescent development, and stigma surrounding the diagnosis. Adolescents with BPD symptomology frequently present to inpatient psychiatric and medical settings due to recurrent suicide attempts and self-harm. A substantial body of research supports the reliable and accurate diagnosi…

Background/Objectives: Borderline personality disorder (BPD) is a severe mental disorder characterised by emotion dysregulation, impulsivity and interpersonal hypersensitivity. Its prevalence ranges from 0.5% to 6.4%. Eye tracking and pupillometry provide objective indices of social attention and inhibitory control, but the BPD literature using these techniques has not been systematically reviewed. The aim of this work was to synthesise the empirical evidence on visuo-attentional and pupillary alterations in BPD. Methods: Following the PRISMA 2020 statement, Web of Science, Scopus and PubMed w…

This article revisits the Bateson group's concept of the double bind, originally proposed as an etiological theory of schizophrenia, and reinterprets it as a central framework for understanding borderline personality disorder (BPD). We propose that paradoxical communication patterns in early development, characterized by contradictory and entrapping messages, become internalized and give rise to the structural and interpersonal paradoxes that define BPD. Drawing upon psychodynamic theory, empirical research, and contemporary object relations perspectives, we conceptualize BPD as a "disorder of…

BACKGROUND: Neurological soft signs (NSS) are subtle sensorimotor abnormalities that have been observed in various mental disorders with neurodevelopmental origin. While NSS have been extensively examined in patients with schizophrenia (SZ), preliminary evidence also suggests that NSS are also present in patients with borderline personality disorder (BPD). However, a transdiagnostic examination of the severity of NSS in BPD compared to SZ is still lacking.
METHODS: Here, NSS were examined with the Heidelberg NSS scale (HNSS) in three groups of female subjects: BPD (n = 45), SZ (n = 30) and he…

Loneliness has been linked with suicidal ideation (SI) in people with a borderline personality disorder (BPD) diagnosis. However, the temporal dynamics of this association remain unclear: we do not know whether loneliness amplifies SI within clinically relevant short-term timeframes (e.g., day to day). To fill this gap, we used data from a 21-day ambulatory assessment study of individuals diagnosed with BPD (N = 152, 103 with a history of attempted suicide) and preregistered the hypotheses and code of our analyses. We tested the hypotheses that daily loneliness would be associated with same- a…

Psychoeducation, delivering up-to-date information about mental illness is crucial for supporting patients' recovery. While recognized for various disorders, its role in borderline personality disorder (BPD) lacks review. This study synthesizes the current evidence about psychoeducational interventions for BPD. Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, this scoping review consulted databases (PubMed, PsychINFO, Web of Science, Cumulative Index to Nursing and Allied Health Literature, and Información Científica y Técnica en Salud de América Latina …

Individuals with borderline personality disorder (BPD) often report chronic, severe self-hatred. It is frequently experienced as immutable, seen as a barrier to recovery, and is associated with risk for self-injury and suicide attempts. Yet self-hatred remains a poorly understood, underdiagnosed, and undertreated presentation of BPD. In this concept article and review, we describe the nature of self-hatred in BPD and related disorders, propose a theory as to the development of self-hatred in BPD, review the assessments of and interventions for self-hatred, and consider next steps in the resear…

Borderline personality disorder (BPD) is characterized by increased mood reactivity and affective instability. Since core structures involved in emotion processing, such as the amygdala, demonstrate strong lateralization, BPD is an interesting target for laterality research. So far, a systematic integration of findings on lateralization in BPD is missing. Therefore, we systematically reviewed studies published until February 2024 in PubMed, Web of Science, and PsycInfo databases that measured hemispheric asymmetries and behavioral lateralization in patients with BPD. Inclusion criteria were (a…

Borderline personality disorder (BPD) is characterized by instability in interpersonal, affective, cognitive, self-identity, and behavioral domains. For a BPD diagnosis, individuals must present at least five of nine symptoms, resulting in 256 possible symptom combinations; thus, individuals diagnosed with BPD can differ substantially. Specific symptoms of BPD tend to co-occur, suggesting BPD subgroups. To explore this potential, we analyzed data from 504 participants diagnosed with BPD enrolled in one of three randomized controlled trials conducted at center for Addiction and Mental Health in…

Comorbidity between borderline personality disorder (BPD) and other mental disorders is common. Although no specific pharmacological treatments have been approved for the treatment of BPD, many drugs, including antidepressants such as selective serotonin reuptake inhibitors (SSRIs), mood stabilizers, second-generation antipsychotics, and even benzodiazepines, are routinely prescribed off label. Nonetheless, recommendations for off-label drugs in these patients are highly varied, with a notable lack of agreement among clinical guidelines. The most common reason for pharmacological treatment and…

OBJECTIVE: A hypermentalizing impairment, or tendency to overattribute mental states to others, has been identified among individuals with borderline personality disorder. However, associations between hypermentalizing and other disorders call into question the specificity of this impairment to borderline personality disorder. This study aimed to evaluate the relative strength of the association between hypermentalizing and borderline personality disorder compared with other disorders and to assess the impact of moderators on the relationship between hypermentalizing and psychopathology.

The progressively improving understanding of the borderline personality disorder (BPD) has led to an increased interest in the better clarification of the integrated role of biological and psychosocial factors in the underlying pathophysiology of this condition. The influence of early childhood interactions and stress exposure in shaping our personalities during adulthood cannot be emphasized enough. In this review, we discuss the critical role of parenting-related factors including maladaptive parenting, parenting styles, and parenting psychopathology as early childhood influences in the deve…

Borderline personality disorder (BPD) is characterized by deficits in emotion regulation and affective liability, specifically rumination. Despite this, inconsistencies have existed in the literature regarding which rumination type is most prominent in BPD. Taking this into consideration, a meta-analysis was performed to look at how BPD symptoms correlate with rumination, while also considering clinical moderator variables (i.e., BPD symptom domain, comorbidities, GAF score) and demographic moderator variables (i.e., age, gender, sample type, and education level). Analysis of rumination domain…
