Ramsay Hunt syndrome associated with brain stem enhancement.

Postcontrast T1-weighted MR images in a patient with Ramsay Hunt syndrome showed an enhancing lesion in the region of the nucleus of the pontine facial nerve and abnormal enhancement of the intrameatal, labyrinthine, and tympanic facial nerve segments and of the geniculate ganglion, as well as enhancement of the vestibulocochlear nerve and parts of the membranous labyrinth. This enhancement most probably resulted from a primary neuritis of the intrameatal nerve trunks of the seventh and eighth cranial nerves.

[Brain stem involvement in eclampsia and HELLP syndrome].

INTRODUCTION: Nowadays, eclampsia is a rare complication of pregnancy and the puerperium. However, it is still one of the main causes of mat … of a thirty year old woman with eclampsia and associated HELLP syndrome, which briefly affected the brain stem. The patient showed progressive general and neurological improvement. Three months afterwards she still had paresia of the sixth right cranial nerve, due to what was presumed to be a right paramedian infarct of the pons. CONCLUSION: We consider it …

MR screening for brain stem compression in hypertension.

PURPOSE: To determine the clinical usefulness of MR imaging to screen for vascular compression of the lateral medulla, considered by some to … t entry zone. Lateral medullary contact only (grade I), contact and depression (grade II), or lower brain stem displacement or rotation (grade III) of the root entry zone were recorded in both hypertensive and normotensive patients. Among hypertensive patients, additional data were gathered from electrocardiographic, echocardiographic, and urinary protein re …

[FLAIR images of cerebral and brain stem infarction].

FLAIR (fluid-attenuated inversion recovery) images are MR images obtained with an inversion recovery sequence having a long inversion time (TI) and a long echo time (TE). We examined 47 cases (56 graphics) of lacunar infarction (LI), 28 cases (32 graphics) of cortical infarction (CI) and 16 cases (23 graphics) of stem infarction (SI) with a FLAIR sequence having a repetitive time (TR) of 6500 msec, a TI of 1700 msec and a TE of 110 msec, and compared these graphics with T2-weighted images by spin-echo sequence (TR 2500 msec, TE 90 msec). LI and CI were better demonstrated with FLAIR images tha

Cavernous angioma of brain stem mimicking multiple sclerosis.

A 14-year-old boy was admitted to our Department due to peripheral palsy of right VII and bilateral of the VI cranial nerves, spasticity, cerebellar symptoms as well as to dysphagia and dysarthria. In general, he was hospitalized 13 times because of the disease of a relapsing-remitting and next progressive course. He died 31 years after onset of the disease. Multiple sclerosis was diagnosed. Brain autopsy revealed tumor involving almost all brain stem structures and a part of right cerebellar hemisphere. Histologically, cavernous angioma was diagnosed.

Rapid growth of a brain-stem cavernous angioma.

A rare case of rapidly enlarging cavernous angiomas of the cerebral hemispheres and brain stem in a child is reported. Rapid growth of cavernous malformations of the brain is rarely observed; it may be due to different mechanisms, including enlargement of the vascular channels, hemorrhage and cysts formation. The possibility of progressive enlargement of cavernomas makes periodical MR studies necessary in all cases. When the growth is radiologically documented, surgical removal is imperative even in patients with trivial symptoms.

[Brain stem auditory evoked potentials in spinocerebellar degeneration].

A study was made of brainstem auditory evoked potentials (BAEP) in 66 patients from 56 families with different forms of spinocerebellar dege … r and degree of BAEP disorders reflect the spreading and gravity of degenerative alterations in the brain stem in different forms of SCD. The authors discuss the possibility of the use of BAEP for objective estimation of the gravity and spreading of the pathological process as well as of the electrophysiological control over its course in SCD patients.

Clival chordoma presenting with acute brain stem hemorrhage.

The authors present a case of a 29-year-old man who developed rapidly progressive cranial nerve palsies and a right hemiparesis secondary to a pontine hemorrhage. The rare but correct diagnosis of a clival chordoma which had invaded the brain stem and subsequently hemorrhaged was based on computed tomography and magnetic resonance imaging. The diagnosis was confirmed at surgery when the patient underwent a successful operative decompression of tumor and clot from the pons via a sub-occipital craniotomy. This represents the first case of a clival chordoma to hemorrhage into the brain stem, whic…

Diagnosis and treatment of vascular brain-stem malformations.

Vascular malformations are a common cause of spontaneous brain-stem hemorrhage in young normotensive individuals. These lesions are no longer cryptic. Magnetic resonance (MR) imaging has renewed interest in the treatment of this disorder because of the precise accuracy in diagnosis and localization of these lesions that it affords. The MR image demonstrates characteristic findings of multiple hemorrhages of varying ages surrounded by a hypointense peripheral zone of hemosiderin. Five cases of vascular brain-stem malformation diagnosed with MR imaging are described. The vascular malformations c

Brain-stem tuberculoma. An analysis of 11 patients.

The clinical and radiologic findings in 11 patients with brain-stem tuberculoma were reviewed. Clinical manifestations included various combinations of focal signs and symptoms of subacute onset, similar to those produced by other space-occupying lesions of the brain stem. Evidence of systemic tuberculosis was found in six cases (55%). Computed tomography (CT) usually showed an isodense or hyperdense brain-stem mass with abnormal contrast enhancement; associated supratentorial granulomas were found in four cases, and hydrocephalus was found in two cases. Magnetic resonance imaging showed irreg…

Brain-stem auditory-evoked potentials in spasmodic torticollis.

The pathophysiology of idiopathic spasmodic torticollis is uncertain. Cerebral, basal ganglia, brain-stem, and cervicomedullary lesions have been implicated. Some investigators have found evoked-potential abnormalities, while others have not. We recorded brain-stem auditory-evoked potentials in six patients with otherwise normal result of examinations and laboratory studies. Brain-stem auditory-evoked potentials were recorded from Cz-A1 and Cz-A2 with rarefaction clicks delivered at 11.1/s and 70 dB above sensory threshold with 40-dB contralateral masking. Analysis time was 10 ms, filter bandp

Ondine's curse in listeria monocytogenes brain stem encephalitis.

A case of necrotizing brain stem encephalitis due to listeria monocytogenes is described in a 48-year-old man who had brain stem encephalitis of complicated course and with selective destruction of the vasomotoric and respiratory centers. He developed that very rare Ondine's curse syndrome, being only able to breathe when awake. The literature on Ondine's curse and brain stem encephalitis due to LM is reviewed. Brain stem encephalitis has a mortality near 100%. The only treatment for Ondine's curse is lifelong artificial ventilation.

[Brain stem auditory evoked potentials and chronic alcoholism].

Brain-stem auditory evoked potentials (BAEPs) were obtained from 25 chronic alcoholic patients. The patients had been drinking heavily for an average of 15 years and a minimum of 5 years. All patients were totally abstinent for a minimum of 3 weeks. The morphology of waves were often altered. All presented prolonged I-III, III-V, inter-peak latencies (IPLs) and a loss of amplitude of wave V. The I-III, III-V and I-VIPLs were increased between 30 and 40 years, and III-V and I-VIPLs between 40 and 50 years. The incidence of abnormal BAEPs in patients with alcoholic epilepsy was not different fro

Auditory brain-stem responses in Marcus Gunn ptosis.

The auditory brain-stem response (ABR) can detect anomalies both in the auditory pathways and in structures adjacent to these pathways. Patients with Duane's retraction syndrome, associated with hypoplasia of the abducens nerve in the brain-stem, and patients with hemifacial spasm, due to compression of the facial nerve in the brain-stem, have been found to have abnormal ABRs. Marcus Gunn ptosis with 'jaw winking' is considered to be due to misconnection of oculomotor, trigeminal and other cranial nerves. Suspecting that perhaps some 'jaw-winking' phenomena may be due to detectable brain-stem

Auditory brain-stem evoked potentials in Bell's palsy.

In an attempt to assess objectively the integrity of the auditory pathways in patients suffering from Bell's palsy, an audiometric evaluation was performed and auditory brain-stem evoked potentials (ABEP) were obtained from 24 patients within the first week from the onset of facial weakness. Despite negative audiometric findings, significant ABEP abnormalities (greater than 2.5 SDs) were observed for 25% of the sample. In each case the ABEP deviation suggested prolonged brain-stem conduction time; for some patients the aberration was only apparent with an increased stimulus repetition rate. Th

Auditory brain-stem response and acoustic reflex test.

Auditory brain-stem response (ABR) and acoustic reflex test (ART) results were reviewed for 30 patients with cerebellopontile angle tumors and for 30 patients without tumor. Patients included in the nontumor group were selected to match the audiometric configurations for individual patients in the tumor group. The ABR waveforms were abnormal for all but one of the patients with tumor and for seven of the patients without tumor. The ART results were abnormal for 25 of the patients with tumor and two of the patients without tumor. Patients without tumor yielding abnormal ABR results usually had

Brain stem auditory evoked responses in clinical practice.

A relatively new noninvasive technique for the diagnosis of abnormalities affecting the central nervous system is presented. It is especially useful in patients with acoustic neuroma, brain stem disorder such as tumors of the posterior fossa with neuraxis impingement, as well as demyelinating disorders such as multiple sclerosis. It is certainly useful in the verification of neurologic normalcy in patients complaining of dizziness and vertigo.
