First Twin in Breech Presentation and Neonatal Mortality and Morbidity According to Planned Mode of Delivery.

OBJECTIVE: To compare neonatal mortality and morbidity of first twins according to the planned mode of delivery when the first twin is in breech presentation, in a country where planned vaginal delivery is an option. METHODS: This is a planned secondary analysis of the JUMODA (JUmeaux MODe d'Accouchement) cohort, a national prospective population-based study of twin deliveries conducted in 176 French hospitals. We analyzed pregnancies with first twins in breech presentation and applied the inclusion criteria of the Twin Birth Study (except the criterion for first-twin presentation): both fetu…

Linear association between maternal age and spontaneous breech presentation in singleton pregnancies after 32 weeks gestation.

PURPOSE: To investigate the association between maternal age and spontaneous breech presentation. MATERIAL AND METHODS: Fifteen-year observational study over (2001-2015). All consecutive singleton births delivered at the Centre Hospitalier Universitaire Sud Reunion's maternity. The only single exclusion criterion was uterine malformations (N = 123) women. RESULTS: Of the 60,963 singleton births, there was a linear association (χ2 for linear trend, p< 0.0001) between maternal age and spontaneous breech presentation. Overall rate of breech presentation was 2.7% in deliveries over 32 weeks gest…

Comparison of cesarean versus vaginal delivery of extremely preterm gestations in breech presentation: retrospective cohort study.

Objective: As survival increases at earlier gestational ages, the optimal mode of delivery, especially in cases of breech presentation, is of increasing importance. The objective of this study was to compare outcomes of vaginal delivery (VD) and cesarean section (CS) births for infants in breech presentation at borderline viability. Study design: A retrospective chart review of live breech births between 23 + 0 and 25 + 6 weeks gestation at a tertiary university center from 2003 to 2013 was conducted. Those delivered vaginally were compared with those delivered by CS. Stillbirths and deliverie…

Breech presentation: its predictors and consequences. An analysis of the Hungarian Tauffer Obstetric Database (1996-2011).

INTRODUCTION: Breech presentation is linked to abnormal pregnancy outcomes. However, the causality of this association is unknown. We aimed to investigate predictors of term breech presentation and pregnancy outcomes of breech presentation. MATERIAL AND METHODS: Using a Hungarian registry, all term (≥ 37 weeks), singleton pregnancies with cephalic, and breech presentation in 1996-2011 were analyzed (n = 41 796). Covariates were maternal medical history and data on the present pregnancy. Multivariable logistic regression was used to investigate predictors of breech presentation and of delivery…

Does a probability of breech presentation of more than 50% exist among diseases and medical conditions?

The aim was to study the effect of twin gestations in a uterus with 2 bodies on the probability of breech presentation at delivery. The hypothesis was that the probability of breech presentation was not higher than 50%. A review was undertaken of MEDLINE (1966-2004) and of the article reference list for statistical analysis of presentation at delivery among twins in a normal uterus, singleton gestations in a uterus with 2 bodies, and case studies of twins in a uterus with 2 bodies. There are 10 studies of twin gestations in a normal uterus (Twin A 3036 cases, breech presentation 22.36%; Twin B…

Does intra-uterine environment influence fetal head-position preference? A comparison between breech and cephalic presentation.

BACKGROUND: Cephalic fetuses have increasing lateralised head-position near term. AIM: Is this development affected by breech presentation? SUBJECTS AND METHODS: Fetal head-position was studied longitudinally in 13 healthy fetuses in breech presentation and 10 healthy fetuses in cephalic presentation by means of real-time ultrasound. Recordings were obtained weekly from 33 weeks gestational age until birth. RESULTS: As in previous research, a significant (p = 0.045) decrease in midline head-position was found for the cephalic fetuses with advancing gestational age. The development of a late…

The Coexistence of Spontaneous Femoral Nerve Palsy and Developmental Dysplasia of the Hip in a Breech-Presenting Newborn: A Rare Case Presentation.

CASE: In this report, we present an infant who was diagnosed with femoral nerve palsy (FNP) and developmental dysplasia of the hip (DDH) on the same side after delivery. The patient was treated with hip alternative abduction orthosis, which did not force hip flexion with satisfactory results. CONCLUSION: FNP in newborns with DDH is usually described as a possible complication of the Pavlik harness treatment. However, there is only one report that presents 2 cases. Our objective is to raise awareness of this very rare condition because performing an early intervention is vital to achieving an

Vaginal vs. cesarean delivery for preterm breech presentation of singleton infants in California: a population-based study.

OBJECTIVE: To examine the morbidity and mortality associated with vaginal breech delivery (VBD) of premature, low-birth-weight (LBW) (< 2.5 kg) newborns as compared to delivery by cesarean section. STUDY DESIGN: A retrospective cohort study of singleton, preterm (< 37 weeks), LBW, nonanomalous newborns in California (January 1, 1991-December 31, 1999) was performed. Neonatal morbidity and mortality by route of delivery were compared. RESULTS: Overall, 14,417 LBW, preterm, breech newborns were delivered (14% vaginally and 86% by cesarean). There were 150,570 LBW, preterm, cephalic newborns, o

Planned vaginal delivery versus elective caesarean section in singleton term breech presentation: a study of 1116 cases.

OBJECTIVE: To compare neonatal and maternal morbidity and mortality between planned vaginal delivery and elective cesarean section for singleton term breech presentation. STUDY DESIGN: We studied retrospectively all deliveries of singleton breech presentations at term in the public Hospitals of Lyon between 1 January 1991 and 31 December 1995. To take only the effect of delivery on the fetus into account, we eliminated high risk pregnancies and fetal malformations. Fetal parameters were corrected perinatal mortality, Apgar score<7 at 1 and 5 min, umbilical cord arterial pH<7.15, neurological …

[What effect does leg position in breech presentation have on mode of delivery and early neonatal morbidity?].

The objective of the present study is to establish whether the position of the legs in breech presentation deliveries affects the vaginal or abdominal mode of surgical delivery and the early neonatal morbidity. The patient population investigated (n = 266) comprised 163 primiparae (61.3%) and 103 multiparae (38.7%). Of the 266 term infants (more than 37 complete WOP) with breech presentation, 71.3% (127/178) could be delivered vaginally from a simple pelvic presentation and 55.3% (42/76) from an incomplete or complete footling presentation. The average duration of labor was 460 minutes in a pe…

Favorable neonatal outcome after fetal entrapment and partially successful Zavanelli maneuver in a case of breech presentation.

A 16-year-old primigravida woman presented in advanced labor with a singleton breech presentation. Vaginal delivery was considered to be possible, but entrapment at the level of the fetal thorax occurred. The Zavanelli maneuver was attempted unsuccessfully. A cesarean section was performed immediately but was complicated by lateroflexion of the fetus. The attending pediatrician proceeded to endotracheal intubation and ventilation of the partially born baby. Repositioning of the mother facilitated delivery. The fetus survived without any complications. This case illustrates the lack of scientif…

Is it reasonable to propose vaginal delivery with twin pregnancies, when the first twin is in breech presentation?

BACKGROUND: Breech presentation of the first twin occurs in 20% of twin pregnancies. However, the impact of delivery mode on morbimortality in breech fetuses remains controversial in the literature generally, and has been infrequently studied in twin pregnancies specifically. The aim herein was to evaluate neonatal and maternal outcomes according to delivery mode when the first twin was in breech presentation, and to compare these results with those in the current literature. MATERIAL AND METHODS: This was a single-center, retrospective study in Lille, France, from January 2010 to December 20…

[The structure of neonatal mortality among infants born in the cephalic and breech presentations in 1992-1994].

This retrospective study, including all postnatal died newborns from 1992 to 1994 in our hospital, shows, that the mortality rate among the breech presentations is several times higher then this among the cephalic presentations. Inborn malformations, specially those of CNS, and birth trauma are with significant higher frequency causes of death in the first group. Intrapartal asphyxia, intraventricular haemorrhages and respiratory distress syndrome are equally presented in the neonatal mortality structure of the newborns in breech and cephalic presentation.

[The mode of delivery and neonatal outcome in breech presentations less than 32 weeks' of gestation].

This study was undertaken to determine the effect of the delivery method on neonatal outcome in fetal breech presentation, and the following results were obtained. The rate of cesarean section for breech presentations was 70.7% (104/147). 1) There were 18 neonatal deaths (41.9%) in 43 vaginal breech deliveries compared to 8 (7.7%) in 104 cesarean breech deliveries. Neonatal deaths were significantly higher in vaginal breech deliveries than cesarean breech deliveries (p less than 0.001). 2) In breech presentations, intracranial hemorrhages were more common among vaginally born infants (44.2% vs…

Effectiveness and Safety of External Cephalic Version in Full-Term Breech Presentations: Retrospective Study.

BACKGROUND: External cephalic version (ECV) is a pivotal intervention aimed at converting abnormal foetal presentations to cephalic in full-term pregnancies, thereby reducing cesarean delivery rates. This study evaluated the effectiveness of ECV and examined factors influencing its success. METHODS: We conducted single-centre retrospective observational study at the Fourth Hospital of Shijiazhuang, high-volume tertiary obstetric centre, from January 2018 to December 2023. Three hundred and sixty-five women with singleton-term pregnancies and breech, transverse, or oblique foetal presentation

What Is the Proportion of Late-developing DDH After a Normal Ultrasound in Newborns With Breech Presentation?

BACKGROUND: Ultrasound at 2 to 6 weeks of age is routine clinical practice in neonates at high risk; however, developmental dysplasia of the hip (DDH) may develop after a normal screening examination. There is a current lack of consensus regarding the use and most appropriate timing of subsequent imaging. QUESTIONS/PURPOSES: (1) What proportion of neonates with a breech presentation who had a normal screening ultrasound result for DDH developed DDH at 6-month follow-up? (2) Are patient factors (sex and concomitant congenital muscular torticollis) associated with an increased odds of late-deve…

Fetal size at the 32nd gestational week is associated with the risk of breech presentation at term birth.

OBJECTIVES: The association patterns between breech presentation at birth and fetal biometry at the first, second, and third trimesters, newborn size but also maternal age, body height, prepregnancy weight status as well as gestational weight gain, were analyzed using a dataset of 4501 singleton term birth in Vienna, Austria. METHODS: In this medical record-based study, fetal biometry was reconstructed based on the results of three ultrasound examinations conducted at the 11th/12th, 20th, and 32nd gestational weeks. Head dimensions, abdominal dimensions, and femur length were determined by so…
