Occupational rhinoconjunctivitis and bronchial asthma due to Phoenix canariensis pollen allergy.

We report a case of occupational bronchial asthma and rhinoconjunctivitis caused by Phoenix canariensis (PC) pollen. The canary palm is a type of palm tree, belonging to the Arecaceae family, which is widely distributed in frost-free regions as an ornamental tree. Our patient was referred because he suffered symptoms of bronchial asthma, rhinoconjunctivitis, and contact urticaria when pruning dried leaves from PC during the pollination months. The skin prick test (SPT) with a PC pollen extract was positive, as was the specific IgE to PC pollen determined by Phadezym RAST, indicating an IgE-med

Assessment and outcomes of firefighter applicants with possible asthma.

BACKGROUND: Firefighter applicants (FFAs) with a history of asthma may be refused entry to the fire service because of potentially putting themselves and others at risk. AIMS: We undertook a service evaluation to identify respiratory and employment outcomes of FFAs with a history of asthma who had undergone additional respiratory assessment at our specialist occupational lung disease clinic during 2005-19. METHODS: We reviewed FFA medical records and categorized them as having either no current asthma or definite/probable asthma at the time of clinic assessment. 'No current asthma' was defin

[Occupational airway sensitization due to subtilisin].

Since the 1960s an increasing number of occupational allergies against natural, and against modified recombinant enzymes were observed. Subtilisin, a frequently used enzyme often has been shown to cause airway sensitization. The present evaluation of clinical literature data confirms the sensitization potential of subtilisin. Several cases of specific airway hyperresponsiveness caused by subtilisin are verified by a number of studies. As symptoms, results of skin prick tests, detection of specific IgE-antibodies and results of specific bronchoprovocation tests are consistent, an immunologic me

Mechanisms of airway hyperresponsiveness.

Airway hyperresponsiveness (AHR) to direct (histamine and methacholine) and indirect (exercise, cold air, hyperventilation, AMP) challenges is a universal and defining feature of asthma. One component of AHR is transient or inducible and occurs after allergen exposure, for example, and improves occasionally rapidly after inhaled corticosteroids or environmental control. This transient airway hyperresponsiveness is more marked to the indirect stimuli. There are convincing data linking this component of AHR to airway inflammation; however, the precise mechanisms linking airway inflammation and h

Influence of infused adenosine on bronchial tone and bronchial reactivity in asthma.

Adenosine has been found to contract human bronchial smooth muscle in vitro and to induce bronchoconstriction in asthmatic patients when administered by inhalation. The aim of the present study was to investigate if elevation of circulating levels of adenosine influence bronchial tone or bronchial reactivity. Seven patients with bronchial asthma in whom bronchial hyperreactivity had been confirmed in a pretrial bronchial histamine challenge (PC20 FEV1 0.064 to 2.45 mg/ml) received intravenous infusions of adenosine in increasing doses (10, 30 and 50 micrograms/kg/min, 6 min on each dose step)

Pathological Roles of Neutrophil-Mediated Inflammation in Asthma and Its Potential for Therapy as a Target.

Asthma is a chronic inflammatory disease that undermines the airways. It is caused by dysfunction of various types of cells, as well as cellular components, and is characterized by recruitment of inflammatory cells, bronchial hyperreactivity, mucus production, and airway remodelling and narrowing. It has commonly been considered that airway inflammation is caused by the Th2 immune response, or eosinophilia, which is a hallmark of bronchial asthma pathogenesis. Some patients display a neutrophil-dominant presentation and are characterized with low (or even absent) Th2 cytokines. In recent years

Mechanisms of action of some air pollutants on the airways.

Common air pollutants (O3, SO2) exert their deleterious effects in several ways, mostly on the respiratory functions. Ozone causes formation of peroxides and aldehydes with subsequent release of inflammatory lipids and cytokines. Changes in the activity of neutral endopeptidase and release of neuropeptides may occur. As a result, bronchial hyperreactivity can be detected. In the mechanisms of the disturbances due to sulfur dioxide, cholinergic mechanisms and release of neuropeptides may be involved.

The use of bronchial challenge to diagnose asthma in preschool children.

Wheezing and respiratory symptoms are very common in preschool children, and the ability to diagnose asthma correctly is important. This review describes an approach to the diagnosis of asthma in preschool children by means of an inhalation bronchial challenge. The child is exposed to increasing doses of challenge agent until wheezing is detected at a dose below that at which normal subjects respond, thus indicating bronchial hyperreactivity. The technique was originally developed using simple stethoscope auscultation of the lungs together with recording of respiratory rate and oxygen saturati

Bronchial challenge with aspirin lysine in the diagnosis of asthmatics with sensitization to aspirin and its inhibition by aerosolized furosemide.

We performed bronchial challenge with ASA lysine in 9 patients with a history of aspirin-induced asthma, 4 asthmatics with no history of hypersensitization to aspirin and 4 control subjects. The test consisted of successive inhalations of increasing concentrations of ASA lysine (11.25, 22.5, 45, 90, 180 and 360 mg/ml) and was interrupted when FEV1 showed a decrease of at least 20%. In order to determine the degree of bronchial hyperreactivity, we first carried out a bronchial challenge with histamine. All patients in the group sensitive to aspirin had a positive reaction to ASA lysine, while t

Protectin D1 and maresin 1 attenuate airway hyperreactivity induced by IL-13 in human isolated small bronchi.

BACKGROUND AND PURPOSE: Interleukin (IL)-13 is implicated in airway hyperreactivity (AHR), a key feature of asthma. We explored the potential anti-AHR activity of selected specialised pro-resolving mediators (SPMs) in IL-13-induced AHR models, using human bronchial smooth muscle cells (BSMCs) and human isolated bronchi. EXPERIMENTAL APPROACH: Calcium flux responses induced by histamine or LTD4 were assessed in BSMCs preconditioned with IL-13 and SPMs for 24 h. Human bronchi were isolated from lung tissue and preconditioned for 48 h in the presence or absence of IL-13 and SPMs. Concentration-r

Methacholine inhalation challenge in children with idiopathic chest pain.

Bronchial reactivity to inhaled methacholine (MCH) was evaluated in 32 patients with 'idiopathic' chest pain. Each pain was recurrent in nature. The incidence of cases with a provocative concentration causing a 20% fall in the forced expiratory volume in 1 sec (PC20) of 10 mg/ml or less was 62.5% (20 cases), while it was only 11.1% (three cases) in 27 healthy controls. Seventeen patients had no personal history of allergic diseases, elevated serum IgE level or positive house dust mite-specific IgE antibody. Among these 17, eight (47.1%) had a PC20 of 10 mg/ml or less, the incidence of which wa

Defects of airway smooth muscle cell function are important in asthma.

For many years asthma has been regarded as an inflammatory disease of the airway mucosa leading to bronchial hyperreactivity. Recent studies showed marked abnormalities in airway smooth muscle behaviour in patients with asthma. The pathogenesis of asthma seems to consist of airway inflammation combined with airway smooth muscle remodelling. The latter pathology is linked to a lack of the ant-proliferative transcription factor C/EBP-alpha in this specific cell type.

Comparison of airway reactivity induced by cold air and metacholine challenges in asthmatic children.

Bronchial responsiveness to isocapnic hyperventilation with cold air (CAH) and to inhaled methacholine (MCH) was compared in 17 children with bronchial asthma. The response to cold air was expressed as the percent drop in FEV1 from baseline at 4 min. after the challenge (delta % FEV1 CAH), and the response to methacholine as the provocative concentration required to reduce the FEV1 by 20% from baseline (PC20MCH). Both tests were sensitive (94%) for detecting airway hyperreactivity. There was no statistically significant relationship between delta % FEV1 CAH and the log PC20MCH (r = 0.39; P = 0

Interleukin-1 beta: a possible mediator of lung inflammation and airway hyperreactivity.

Interleukin-1 (IL-1), a peptide released from monocytes/macrophages, plays an important role in the inflammatory and immune responses. Airway hyperreactivity and the underlying airway inflammation are common features in asthma pathology. We investigated and characterized the inflammatory alterations induced within the guinea-pig respiratory system by IL-1 beta. Injection of IL-1 beta (1-6 micrograms/animal) into the pleural space resulted in a dose-dependent inflammatory response, as shown by the formation of pleural exudate and leucocyte recruitment. A threshold dose of IL-1 beta (1 microgram

[Thoracic ectopic kidney. Report of a case].

We describe a 23 months old boy who presented a clinical picture of bronchial hyperreactivity and a tumoral intrathoracic mass in a plain thorax X-ray, receiving for these reasons, antibiotic therapy during 30 days. Intravenous pyelography and ultrasonography revealed the presence of an ectopic intrathoracic kidney. Since this renal anomaly is usually asymptomatic and do not requires treatment, it is necessary to suspect this abnormality whenever an intrathoracic mass is found.

[The silent correlate of rhinitis on lower airway].

The respiratory tract, from the nose to the lung, behaves as an anatomical and pathophysiological unit under a holistic model. Lower airway abnormalities, such as bronchial hyperresponsiveness, reduced lung function and inflammation of the bronchial mucosa without clinical expression, have been observed in patients with rhinitis without asthma. These would be the consequence of a common systemic inflammatory phenomenon with simultaneous impact on the nose and lung. For unknown reasons, these patients do not exhibit a full clinical expression, which could mean an increased risk of developing as

Advances and Challenges of Antibody Therapeutics for Severe Bronchial Asthma.

Asthma is a disease that consists of three main components: airway inflammation, airway hyperresponsiveness, and airway remodeling. Persistent airway inflammation leads to the destruction and degeneration of normal airway tissues, resulting in thickening of the airway wall, decreased reversibility, and increased airway hyperresponsiveness. The progression of irreversible airway narrowing and the associated increase in airway hyperresponsiveness are major factors in severe asthma. This has led to the identification of effective pharmacological targets and the recognition of several biomarkers t

[The cough variant asthma].

Cough variant asthma (CVA) was first described by W. Corrao. CVA was described as the isolated chronic cough as the only presenting symptom responsive to bronchodilator therapy. This phenotype of asthma is present with airway hyperresponsiveness, eosinophilic inflammation airways and bronchodilator responsive coughing without typical manifestation of asthma such as wheezing or dyspnea. CVA shares common features with classic asthma such as eosinophilic inflammation and airway remodeling. Because of that, CVA is clinically considered as a variant type of asthma.

[The new insight into the pathogenic unity of the upper and lower airways].

Between the upper and the lower respiratory tracts exists a link. Numerous epidemiological, immunological studies and clinical observations suggest the pathogenic unity of the upper and lower airways. The most important observations regarding the nose-lung interaction is rhinitis and asthma. The inflammatory process in the nose is the same as in the bronchi, clinically defined as rhinosinusitis, nasal polyps, asthma, bronchial hyperreactivity, allergy, viral infections. The strict link between the rhinosinusitis and asthma implies new possibility of influencing one of the two complaints by tre

[Evaluation of the sequelae of occupational asthma].

Current knowledge of the evolution of occupational asthma is based on few clinical studies in groups of patients sensitized to red cedar, colophony, crab boiling water and organic isocyanates. Whatever the sensitizing agent, respiratory sequelae appear to be very frequent (50% or more of subjects removed from exposure) and characterized by the persistence of an asthmatic condition with a variable degree of severity. For the greater part of these cases certain features at the time of diagnosis assessment appear to be of prognostic value for evolution: age, duration of occupational exposure, dur
