[The CO2 laser in pathologic changes in the bronchial system].

The experience of endoscopic laser therapy using a CO2-laser "Skalpel-1" via Friedel bronchoscope is analysed. Treatment was administered to 40 patients with different endobronchial pathology: tumors, bronchonodular fistulas in tuberculosis of intrathoracic lymph nodes, non-specific stump inflammation, mycosis, ligature material, bronchial stenosis, tracheal and bronchial scleroma. High efficiency of endoscopic laser bronchoscopy is proved.

Asbestos bodies in fine needle aspirates of lung masses. Markers of underlying pathology.

Analysis of 52 transthoracic-mass aspirates that contained asbestos bodies (ABs) showed the mass to be due to pathology other than (or superimposed upon) asbestosis in every case. Malignancy accounted for 30 masses, all of which were carcinomas except for one mesothelioma. The remaining 22 lesions were benign, with tuberculosis or lung abscesses accounting for the majority. Fine needle aspiration (FNA) detection of the pathology (benign or malignant) associated with ABs was diminished, probably due to asbestos-induced fibrosis. Other diagnostic methods, including bronchial studies, mediastinos

Surgical management of endobronchial inflammatory myofibroblastic tumors.

BACKGROUND: Endobronchial myofibroblastic tumors are neoplasms composed of clonal populations of smooth muscle cells and a variable lymphocytic inflammatory component. They represent a challenge with respect to diagnosis, classification, and surgical resection due to their infrequent occurrence. METHODS: We retrospectively reviewed our experience with patients who had myofibroblastic tumors in the major airways over a 15-year period, in order to understand the incidence, natural biology, treatment, and long-term outcome of individuals with this type of neoplasm in an endobronchial location.

Bronchial extramedullary hematopoiesis preceding chronic myelogenous leukemia.

Extramedullary hematopoiesis of the bronchus is rare. The case of a 72-year-old man in whom the right lower lobe bronchus was obstructed by extramedullary hematopoiesis is presented. Ten months after the initial presentation, Philadelphia chromosome-negative chronic myelogenous leukemia was diagnosed. Such findings have not been reported previously. The various anatomic locations of extramedullary hematopoiesis are reviewed, with an emphasis on intrathoracic and pulmonary presentations. The clinical and pathologic features and the differential diagnosis in the present case are discussed.

Giant and potentially malignant bullae in a dog.

BACKGROUND: Primary lung neoplasms are, frequently represented by solid, solitary, or multiple formations. However, malignant cavitary lesions may be presented as lung adenocarcinomas. Those malignant lesions differ from benignant bullae by the thickness heterogeneity of its surrounding shape. CASE DESCRIPTION: The present clinical case reports a 14-year-old female dog, of mixed breed, with an increase in the coughs frequency, fatigue, and exercise intolerance. A chest X-ray was taken, a large emphysematous cystic area was found, with thickened and irregular walls located in the left caudal p

[Bronchial carcinoid in a 7-year-old boy].

A case of a 7-year-old boy admitted to the clinic with severe symptoms of 1-month lasting pneumonia not responding to antibiotics is presented. The chest X-ray confirmed inflammatory process in left lung parenchyma. Due to unsuccessful further preservative treatment, bronchoscopy and CT of the thorax were performed. They showed the presence of a tumor narrowing the left main bronchus. Histopathologic examination of the tissue taken during bronchoscopy revealed carcinoid. Through the left-sided thoracotomy, the resection of a 5 cm large oval-shaped tumor, as well as the distal part of the left

Management of malignant tracheobronchial obstruction.

Malignant airway obstruction produces symptoms of dyspnea, cough, and stridor with a significant impact on quality of life. With progressive airway narrowing, a critical stenosis is life threatening, due to impending suffocation. Bronchoscopy provides the mainstay of establishing the diagnosis as well as defining the extent and degree of airway obstruction. Therapeutic bronchoscopy provides the ability to achieve prompt airway stabilization while completing a patient evaluation and workup. Patients may have malignant airway obstruction from primary tracheobronchial tumors, adjacent primary tum

Diagnostic sensitivity: Surgical biopsy versus minimally invasive techniques in bronchopulmonary cancer.

INTRODUCTION AND OBJECTIVES: Lung cancer remains the leading cause of cancer-related mortality, with early diagnosis critical to prognosis and survival. Histopathological confirmation is achieved through invasive or minimally invasive methods, the effectiveness of which varies according to clinical context. This study aimed to assess the diagnostic sensitivity of procedures commonly used in the evaluation of suspected bronchopulmonary neoplasms. MATERIALS AND METHODS: We conducted a retrospective study of 210 patients hospitalised in two specialised centres in southeastern Romania between 202

A Rare Case of Posterior Circulation Stroke Caused by Bronchial Artery Embolization.

Bronchial artery embolization (BAE) is an effective treatment for massive hemoptysis. Stroke is a rare complication of BAE, with only a few cases reported in the literature. We report one such case. Posterior circulation strokes after BAE can be caused by connections between the vertebral arteries and the bronchial arteries (shunting between pulmonary and systemic circulations), backflow of embolization material from the bronchial or intercostal arteries to the subclavian artery or through backflow of emblospheres into the aortic lumen leading to subsequent embolization of the cerebral circula

Transbronchial needle aspiration: initial experience in routine diagnostic bronchoscopy.

BACKGROUND: Transbronchial needle aspiration (TBNA) has been shown to be useful not only for the diagnosis and staging of lung cancer, its most widely studied indication, but also for many of other clinical indications. Despite this, it remains largely underutilized, mainly because of concerns with poor yield, safety, lack of experience of the bronchoscopist, and lack of cytopathological support. OBJECTIVE: To study the clinical utility and yield of TBNA as an adjunct to other conventional procedures in diagnostic bronchoscopy at a centre that was relatively inexperienced with this technique,

Paraneoplastic pemphigus associated with bronchiolitis obliterans.

Paraneoplastic pemphigus (PNP) is an autoantibody-mediated mucocutaneous blistering disease associated with underlying neoplasms. Autoantibodies of PNP bind to the plakin family of cytoplasmic proteins and desmogleins of cell-surface target antigens. We describe a 36-year-old female patient with PNP who had non-Hodgkin's lymphoma, and who developed bronchiolitis obliterans and died of respiratory failure. Autopsy findings confirmed luminal narrowing of bronchioles by scarring, which is a histopathologic features of bronchiolitis obliterans. After the onset of respiratory failure, the reaction

Selective venous sampling for ACTH in Cushing's syndrome: differentiation between Cushing disease and the ectopic ACTH syndrome.

We performed selective venous catheterization and sampling for ACTH in six patients with ACTH-secreting pituitary adenomas (Cushing's disease) and four patients with occult ectopic ACTH-secreting neoplasms. In five patients with Cushing's disease in whom the inferior petrosal sinus could be catheterized, ACTH levels were unequivocally higher than simultaneous peripheral values: The ratio was greater than 2.0, with a range of 2.2 to 16.7. In contrast, the inferior petrosal sinus-to-peripheral ACTH ratio in three patients with ectopic ACTH secretion was less than 1.5. In the fourth patient, an a

[Mature teratoma of the anterior mediastinum with a predominance of the pancreatic component and signs of focal nesidioblastosis].

A case of a mature anterior mediastinal teratoma with a predominance of the pancreatic component (80% of the tumor) and signs of nesidioblastosis in a 46-year-old woman is presented. During histological examination, all components of the pancreatic parenchyma were determined - acini with interlobular and intralobular ducts, endocrine cells and islets of Langerhans of various sizes and shapes, as well as islet-duct complexes scattered in the acinar tissue. In addition to pancreatic tissue, cartilage, bronchial respiratory epithelium, small intestine tissue, hair follicles, and sebaceous glands

Intrathoracic amyloid tumors that presented as yellowish multinodular endobronchial protrusions with irregular vascularity and easy bleeding.

Immunoglobulin light-chain (AL) amyloidosis is a monoclonal plasma cell neoplasm that has a tendency to bleed easily. However, the potential risks of transbronchial biopsy in such cases have not been fully proven. Here, we report a case of parotid and intrathoracic AL amyloid tumors that presented as endobronchial protrusions that bled easily. Bronchoscopy under conventional white light and narrow band imaging revealed yellowish multinodular protrusions, in which irregular tortuous or dotted vessels were observed. Unexpectedly, biopsy of the lesion resulted in persistent bleeding. The biopsy s
