OBJECTIVE: We sought to clarify the correlates of severity in women with bulimia nervosa.
METHOD: We studied 114 bulimic women at entry to a randomized clinical trial. We used eight definitions of severity that encompassed the DSM-III-R criteria set for bulimia nervosa (i.e., binging, purging, and body dissatisfaction in the current and worst lifetime time frames) and global functioning and symptomatology. We operationalized 28 possible correlates of severity that covered demographic, Axis I psychiatric, personality, family background, symptoms, and neuropsychological domains. We used regress…

OBJECTIVE: The authors describe the distribution of bulimia nervosa among males and identify characteristics that distinguish male bulimics from their female counterparts.
METHOD: Potential references were identified through an English-language literature search using MEDLINE (1966 to April 1990) and through extensive manual searching of textbooks and reviews. All published works with original data or hypotheses concerning bulimia in males were included.
RESULTS: Bulimia affects approximately 0.2% of adolescent boys and young adult men, and males account for 10%-15% of all bulimic subjects i…

A review of research literature that addresses personality correlates of bulimia revealed a major problem which affects that research and its interpretation: Criteria used to define this disorder have not been consistent across investigators. A related issue is that this disorder has been described by a variety of names, such as dietary chaos syndrome, binge-eating syndrome, bulimia nervosa, etc. The second major problem that contributes to the uncertainty about the personality of bulimics is methodology. Among these have been the inclusion of individuals with a history of anorexia nervosa, th…

Bulimia nervosa is an eating disorder syndrome characterized by frequent binge-eating episodes followed by self-induced vomiting, fasting, excessive exercise, or the use of laxatives or diuretics. In addition to the psychological manifestations of this disorder, the patient may exhibit an array of physical symptoms. The major otolaryngologic finding is the presence of benign, persistent enlargement of the parotid and/or submandibular salivary glands in some patients with more severe bingeing and purging behavior. Recent reports in the otolaryngology literature have advocated superficial paroti…

Standardised measures of mental state were used to compare patients with bulimia nervosa with those with major depressive disorder. The two groups were found to be similar in terms of severity of psychiatric disturbance, as measured by the Montgomery & Asberg Scale and the Present State Examination. Noteworthy symptomatic differences were a greater frequency of obsessional ruminations and anxiety amongst the first group, and a greater frequency of depressed mood, apparent sadness, and suicidal ideation amongst the second. Discriminant function analyses revealed that the two patient groups had …

Two groups of bulimic patients, one group characterized by a pattern of laxative abuse for purposes of weight control and the other group characterized by no history of laxative abuse, were compared on several behavioral variables associated with bulimia. Relative to the nonabusers, the laxative abusers were more likely to abuse diuretics; to use diet pills; to chew and spit out food; and to report a history of suicide attempts, self-injurious behavior, and prior inpatient treatment for depression.

Thirty-nine women successfully treated for bulimia nervosa were followed up six months after the conclusion of treatment and classified as maintaining recovery or partially or completely relapsed. Clinical and psychometric variables including body image disturbance and dissatisfaction were entered into regression equations to identify predictors of relapse. Dissatisfaction with body image at the end of treatment was the most potent predictor of relapse. The finding is interpreted as underscoring the importance of addressing issues of body image in therapy if treatment gains are to be maintaine…

In a sample of 130 consecutive patients with a lifetime diagnosis of anorexia nervosa and/or bulimia, 17 displayed psychotic symptoms. In 16 patients, these symptoms appeared attributable to major affective disorder or schizo-affective disorder, while in one, they appeared to represent factitious psychosis. No cases of schizophrenia or organic psychosis were identified.

OBJECTIVE: DSM-5 has dropped subtyping of bulimia nervosa (BN), opting to continue inclusion of the somewhat contentious diagnosis of BN-nonpurging subtype (BN-NP) within a broad BN category. Some contend however that BN-NP is more like binge eating disorder (BED) than BN-P. This study examines clinical characteristics, eating disorder symptomatology, and Axis I comorbidity in BN-NP, BN-P, and BED groups to establish whether BN-NP more closely resembles BN-P or BED.
METHOD: Women with BN-P (n = 29), BN-NP (n = 29), and BED (n = 54) were assessed at baseline in an outpatient psychotherapy tria…

Increasing empirical evidence supports the validity of binge eating disorder (BED), a research diagnosis in the appendix of DSM-IV, and its inclusion as a distinct and formal diagnosis in the DSM-V. A pressing question regarding the specific criteria for BED diagnosis is whether, like bulimia nervosa (BN), it should be characterized by overvaluation of shape and weight. This study compared features of eating disorders in 436 treatment-seeking women comprising four groups: 195 BED participants who overvalue their shape/weight, 129 BED participants with subclinical levels of overvaluation, 61 BN…

Over the past few years, the incidence of bulimia has markedly increased, perhaps also as an expression of social changes. We here document the case of Ilse, a bulimic patient treated in the framework of a multidimensional concept based on integrative painting and behavioural therapy. The pictures made in the Painting Group reflect a patient's innermost experience, and are the point of departure for our therapeutic regimen. Ilse's pictures show that she is confronted with the underlying conflicts of her disorder, particularly her relationship to her father and her mis-routed corporealisation. …

BACKGROUND: Bulimia nervosa (BN) is associated with functional abnormalities in frontostriatal and frontolimbic circuits. Although structural alterations in the frontal portions of these circuits have been observed, this is the first study of subcortical surface morphometry and the largest study of subcortical volume in BN.
METHODS: Anatomical magnetic resonance scans were acquired from 62 female participants with full and subthreshold BN (mean age ± SD, 18.7 ± 4.0 years) and 65 group-matched healthy control participants (mean age ± SD, 19.3 ± 5.7 years). General linear models were used to co…

Cognitive Behavioral Therapy for Bulimia Nervosa Abstract. Abstract:: Cognitive behavioural therapy for eating disorders is an evidence-based effective method for the treatment of bulimia nervosa. It is based on a multifactorial and behavioal disorder model in which causes, triggering and maintaining conditions are considered and summarized in a behavioral analysis. Symptom-oriented standard procedures are used in the therapy process. Skills such as emotion regulation, interpersonal competence and cognitive techniques are taught. The low self-esteem and body dissatisfaction are of particular i…

OBJECTIVE: There is evidence that people with eating disorders display altered intertemporal choice behavior (the degree of preference for immediate rewards over delayed rewards). Compared to healthy controls (HC), individuals with anorexia nervosa and binge-eating disorder show decreased and increased rates of temporal discounting (TD; the devaluation of delayed rewards), respectively. This is the first study to investigate TD in people with bulimia nervosa (BN).
METHOD: Thirty-nine individuals with BN (2 men) and 53 HC (9 men) completed a hypothetical monetary TD task. Over 80 binary choice…

To examine how different definitions of recovery lead to varying rates of recovery, maintenance of recovery, and relapse in bulimia nervosa (BN), end-of-treatment (EOT) and follow-up data were obtained from 96 adults with BN. Combining behavioral, physical, and psychological criteria led to recovery rates between 15.5% and 34.4% at EOT, though relapse was approximately 50%. Combining these criteria and requiring abstinence from binge eating and purging when defining recovery may lead to lower recovery rates than those found in previous studies; however, a strength of this definition is that in…

Bulimia nervosa (BN) is a distressing condition. Its pathogenesis is not fully understood. Neurocognitive functioning, and particularly inhibitory control, is a potential biomarker that may improve our understanding of BN. A few small-scale studies have used the classical Stroop Colour Word Test (SCWT) in BN with contradictory findings. We examined SCWT performance in a large sample of people with BN (N=72), eating disorder not otherwise specified--bulimic type (N=43) and healthy controls (N=50). The results found no difference between groups on Stroop interference effect. These findings quest…
