This case report presents a 70-year-old female patient who was diagnosed with an intracardiac cement embolism two months after a percutaneous vertebroplasty of her L4 vertebra. Although percutaneous vertebroplasty is generally considered a safe procedure, this case highlights the potential for serious complications. The patient was on oral vitamin K antagonist for the past 20 years for an unprovoked deep vein thrombosis and on statins for a chronic dyslipidemia. Although being mildly dyspneic after the operation, the patient presented to the emergency department after two months for a worsenin

Bone cement implantation syndrome (BCIS) is a critical and potentially life-threatening condition that manifests during implantation. Characterized by a constellation of symptoms, including hypoxemia, hypotension, cardiac arrhythmias, elevated pulmonary vascular resistance, and occasionally cardiac arrest, BCIS typically ensues shortly after cement introduction, albeit with rare instances of delayed onset. Primarily attributed to the exothermic reaction of bone cement implantation, this syndrome is caused by local tissue damage, histamine and prostaglandin release, and microemboli formation, u

BACKGROUND: There is widespread consensus that adjuvant local use of antimicrobial agents in combination with their systemic administration can better prevent and treat implant-associated musculoskeletal infections. The advantage of local antibiotics lies in their particular pharmacokinetics with initially high antibiotic concentrations at the implant site with only low systemic uptake.
AIM OF TREATMENT: The aim of local application is to protect the foreign bodies directly at the implantation site from bacterial colonization and biofilm formation (prophylaxis) and to support the eradication

Modern cement implantation techniques during hip arthroplasty rely on high intramedullary pressures which can result in cement extrusion towards femoral nutrient vessels, and thus, the occurrence of a particular image in postoperative radiographs (bone cement arterio-venogram). We report a case series of 14 patients in whom a bone cement arterio-venogram was observed after undergoing a cemented hip arthroplasty. No local or systemic complications developed after cementing nor during a mean follow-up of three years. Bone cement arterio-venogram is a radiologic sign that indicates a good cement

AIMS: We present the results, in terms of survival, clinical outcome, and radiological appearance at 20 years, in a cohort of 225 cemented Exeter Universal femoral components (Stryker, Newbury, UK) implanted in 207 patients, at a district general hospital.
METHODS: All patients in this study had a total hip arthroplasty (THA) using an Exeter Universal femoral component with a cemented (n = 215) or cementless (n = 10) acetabular component. Clinical and radiological data were collected prospectively at one year, five years, and every five years thereafter. Patients lost to radiological and clin

BACKGROUND AND PURPOSE: Altered biomechanics or bone fragility or both contribute to spine instrumentation failure. Although revision surgery is frequently required, minimally invasive alternatives may be feasible. We report the largest to-date series of percutaneous fluoroscopically guided vertebral cement augmentation procedures to address feasibility, safety, results and a variety of spinal instrumentation failure conditions.
MATERIALS AND METHODS: A consecutive series of 31 fluoroscopically guided vertebral augmentation procedures in 29 patients were performed to address screw loosening (

This case describes a complication of bone cement use. A 65-year-old male patient with back spine trauma caused by a fall, underwent a percutaneous vertebroplasty. Five years later, he consulted for palpitations, and the electrocardiogram showed supraventricular arrhythmia. A transthoracic two-dimensional echocardiography showed a hyperechogenic linear structure of 7 cm length, running from the lateral wall of the right ventricle to the right atrium through the tricuspid valve. This foreign body, which was suspicious for bone cement embolism, appeared rigid and was attached at the lateral wall

BACKGROUND: Vertebral compression fractures are the most common osteoporotic fractures. Since the introduction of vertebroplasty and screw augmentation, the management of osteoporotic fractures has changed significantly.
AIMS: The biomechanical characteristics of the risk of adjacent fractures and novel treatment modalities for osteoporotic vertebral fractures, including pure cement augmentation by vertebroplasty, and cement augmentation of screws for posterior instrumentation, are explored.
MATERIALS AND METHODS: Eighteen human osteoporotic lumbar spines (L1-5) adjacent to vertebral bodies

Data concerning the safety of sacroplasty in terms of cement leakage is scarce. Frequency, distribution patterns and clinical consequences of cement leakage were assessed in 33 patients (28 female, mean age: 74 +/- 10 yrs; bilateral SIF: n = 30, 63 sacroplasties) treated with sacroplasty between 06/2003 and 11/2010 in a retrospective study using patients' records, operative notes and postoperative radiographs. Cement leakage was noted within the fracture gap (27%), into veins (6%), neuroforamina (3%) or in the intervertebral disc space L5/S1 (2%). In one patient, cement leakage into the fractu

The purpose of this randomized clinical trial was to compare the >20-year outcomes of cemented (n=124) versus cementless (n=126) total hip replacements (THRs) in patients with end-stage, unilateral hip osteoarthritis. At 20 years, 168 patients (67%) were available for follow-up, 78 (31%) had died, and 4 (2%) were lost. A power analysis was performed to determine the number of patients needed in each study cohort. Patients were assessed pre- and postoperatively by validated disease-specific Western Ontario McMaster Osteoarthritis Score, patient-specific McMaster Arthritis Score, global health (

OBJECTIVE: To evaluate the efficacy hydroxyapatite cement as an alternative to autologous fat in the obliteration of the frontal sinus after trauma or chronic suppuration.
METHODS: Single institution, nonrandomized, prospective analysis of patients treated in an academic, tertiary care center. Patients were followed with physical examinations and were monitored for recurrent infections, which would have necessitated removal of the implant. Follow-up radiographs were obtained at 12 months. Standard surgical techniques were used to obliterate the frontal sinus with the addition of the placement

Hip arthroplasty is a common surgical intervention in our hospital practice, involving high perioperative risk related to patients age and multiple concomitant diseases. Hemodynamic complications described vary from slight hypotension during surgery to heart failure and sudden death, particularly if the operation involves a cemented femoral component. Because of the type of patients undergoing such operations (elderly patients, with osteoporosis and scarce cardiopulmonary reserve), the unclear origin of complications and the lack of consensus on what constitutes adequate monitoring during surg

In 1989-1992 the authors used at the Orthopaedic Clinic, First Medical Faculty, Charles University Prague 34 ceramic conical non-cemented sockets. They used 12 original sockets type AUTOPHOR Osteo Co. and 22 local sockets manufactured by Dias Co. On evaluation of 25 patients, i. e. 26 hip joints, when using Harris score, the mean increment was 56 points, the mean follow up period two years. For evaluation the authors used AP projections as well as tangential pictures focused on the threads which make a more exact evaluation of the integration of the socket possible. The authors found that the

Once a root surface lesion has extended into tooth structure beyond the point at which it can be successfully remineralized it is necessary to carry out traditional restorative procedures. Often the extent of the lesion is difficult to define and development of a pulp exposure is an ever present risk. Access may be difficult and placement of a restorative material, such as amalgam, which requires positive condensation for correct placement, poses a problem. The principal requirement of a restoration in such a lesion is that it should completely seal the restoration/tooth interface and prevent

Great caution is required when choosing materials coming into direct contact with pulpal tissue. The aim of this study was to assess the connective tissue tolerance to Cap-cap and Cinacal, clinically used for pulp capping, in experimental animals. On the basis of tissue reactions around implanted material specimens at intervals of 2, 14 and 28 days after application, the two materials were found to belong to a group of biocompatible materials. Marked infiltration with inflammatory cells around Cap-cap specimens during the first few days after injection pointed to irritative feature of the mate

The total cobalt and nickel concentration of 11 brands of Asian cement ranged from 8.1 to 14.2 micrograms/g and 14.9 to 28.5 micrograms/g, respectively. These metals exist mainly as insoluble salts; the water-soluble concentration of cobalt and nickel in the cements ranged from 0.39 to 0.65 micrograms/g and from 0-1.2 micrograms/g, respectively. 1.5% (4/272) of construction workers in a prefabrication construction factory had cobalt sensitivity. All had allergic contact dermatitis from chromate in cement. No worker had isolated cobalt sensitivity and cement dermatitis. It appeared that sensiti
