Conservative Management of Cerebrospinal Fluid Leaks.

Cerebrospinal fluid leaks are a complex clinical challenge with variable causes and management. Conservative therapy focuses on lowering intracranial pressure through bed rest, sinus precautions, antiemetics, bowel regimens, and lumbar drainage. Leaks persisting beyond 1 week may require surgical repair due to meningitis risk. Iatrogenic, postoperative, and spontaneous leaks rarely resolve without surgery, while small traumatic leaks tend to have the highest chance of resolution. Prophylactic antibiotics remain controversial. Management should be tailored to etiology, location, and patient-spe…

Cerebrospinal fluid flow modulates brain health.

Cerebrospinal fluid dynamics play an important role in maintaining brain health and clearing metabolic waste from the brain. In this issue of the JCI, Gursky et al. investigate how CSF distribution is affected when its primary efflux pathway - the deep cervical lymph nodes - is disrupted by cauterization. This timely study reveals compensatory fluid drainage routes from the skull, age-dependent adaptations in CSF homeostasis, and the emergence of neuroinflammation when an efflux pathway is occluded. The findings underscore the need to better understand the physiological mechanisms governing CS…

Spinal Cerebrospinal Fluid Leaks/Intracranial Hypotension.

Spinal cerebrospinal fluid (CSF) leaks occur due to CSF volume loss at the level of the spine. They can occur spontaneously, from trauma, or iatrogenic causes. Classically, patients present with orthostatic headache, but atypical presentations also occur. Initial workup involves ruling out other etiologies with careful history, neurologic examination, and MRI of brain/spine. Treatment starts with conservative measures and nontargeted epidural blood patching (EBP). If not successful, precise leak localization is required with myelography. Targeted therapy can be offered, including EBP/fibrin gl…

Morphological changes in cerebrospinal fluid production.

The ventricular system and subarachnoid space are filled with cerebrospinal fluid, which plays a key role in the nervous system. This fluid is produced by the choroid plexus, an organ rich in ion transporters that precisely control the transport of specific ions into the cerebrospinal fluid thanks to tight junctions between the plexus cells; these prevent the passage of substances other than the transporters, thus allowing for precise control of the fluid composition. Cerebrospinal fluid production is based on a network of interrelationships between specific ion flows enabled by the numerous t…

Endoscopic Repair of Cerebrospinal Fluid Leak.

BACKGROUND AND IMPORTANCE: The evolution of endoscopic techniques in spine has allowed novel approaches to various pathologies. We describe the use of endoscopy for repair of refractory cerebrospinal fluid (CSF) leak after lumbar puncture (LP). CLINICAL PRESENTATION: A 31-year-old female presented with refractory postural headaches after undergoing LP. The patient underwent 4 total epidural blood patches (EBPs) with minimal symptomatic relief. The LP opening pressure was found to be 18 cm of water, inconsistent with underlying pseudotumor cerebri. A myelogram demonstrated persistent CSF leak …

Cerebrospinal fluid venous fistula: Illustrative case.

BACKGROUND: Spinal cerebrospinal fluid (CSF) venous fistulas are an under-recognized cause for spontaneous intracranial hypotension (SIH) which may result in postural headaches. CASE DESCRIPTION: A 60-year-old-male presented with 6 years of a persistent headache. The initial brain and spine MRIs and conventional CT myelogram (CTM) showed no CSF venous fistula. However, the lateral decubitus dynamic CTM demonstrated a hyperdense paraspinal vein on the right at the T10-11 level consistent with a CSF venous fistula. It was subsequently successfully treated with surgical ligation. Temporary CSF d…

Posterior Fossa Spontaneous Cerebrospinal Fluid Leaks.

OBJECTIVE: Describe the diagnosis and management of spontaneous lateral skull base cerebrospinal fluid (CSF) leaks that originate from the posterior fossa. STUDY DESIGN: Retrospective case review. SETTING: Tertiary university hospital. PATIENTS: Adult patients from 2005 to 2015 who underwent surgical repair of a spontaneous lateral skull base CSF leak with intraoperative confirmation of a posterior fossa leak source. INTERVENTION: Surgical repair. MAIN OUTCOME MEASURES: CSF leak resolution. RESULTS: Five patients had CSF leaks from the posterior fossa. The mean age at presentation was 54…

Cerebrospinal fluid in Creutzfeldt-Jakob disease.

Cerebrospinal fluid (CSF) contains a dynamic and complex mixture of proteins, which reflects physiologic or pathologic states of the central nervous system. Changes in CSF proteome have been described in various neurodegenerative disorders. Earliest publications came from the field of prion disease. Two major approaches have been followed aiming to detect the pathologic form of prion protein (PrPSc) in various peripheral tissues on one hand, but also looking for surrogate parameters as a consequence of the underlying neurodegenerative process. First observations were made using two-dimensional…

Endoscopic management of cerebrospinal fluid rhinorrhea.

Cerebrospinal fluid (CSF) rhinorrhea occurs due to communication between the intracranial subarachnoid space and the sinonasal mucosa. It could be due to trauma, raised intracranial pressure (ICP), tumors, erosive diseases, and congenital skull defects. Some leaks could be spontaneous without any specific etiology. The potential leak sites include the cribriform plate, ethmoid, sphenoid, and frontal sinus. Glucose estimation, although non-specific, is the most popular and readily available method of diagnosis. Glucose concentration of > 30 mg/dl without any blood contamination strongly suggest…
