The Jefferson or bursting fracture of the atlas is frequently overlooked. This is because neurological deficit is usually absent and physical findings are nonspecific. In addition, diagnosis of this fracture cannot be made from the lateral radiograph of the cervical spine. Increase in the retropharyngeal soft-tissue space is valuable both as an indication of upper cervical spine trauma and in distinguishing a Jefferson fracture from the more common fracture of the posterior arch of the atlas. A case history illustrating these features and the radiological diagnosis is presented and discussed.

The anuran atlas represents one of the most structurally simplified cervical systems among tetrapods, consisting of a single vertebra that must simultaneously support the head, permit mobility, and withstand mechanical loads generated during locomotion and feeding. This unique configuration provides a compelling framework for investigating how structural constraint, allometry, and ecology interact to shape morphological evolution. Here, we examine atlas shape diversification across Hyloidea using geometric morphometrics and phylogenetic comparative methods on 421 specimens representing 133 spe

A 14-year-old Japanese boy presented with myelopathy due to atlas hypoplasia with complete posterior arch. Decompressive laminectomy of the atlas produced good neurological recovery, and follow-up T2-weighted magnetic resonance imaging showed disappearance of spinal cord edema. Congenital atlas stenosis may be symptomatic even in children, with no accompanying cervical spondylotic change. Such cases have previously occurred only in Asian adults. A radiological study of the patient's brother showed median cleft formation of the posterior arch of atlas, indicative of a wide spectrum of atlas ano

Pseudonotch of the atlas represents a normal variation of the atlas related to a notched protuberance of the medial surface of its lateral masses. A case is presented in which cervical spine roentgenography revealed a prominent pseudonotch that mimicked a fracture of the lateral masses of the atlas. Minimal rotation of C-1 on C-2 was present without apophyseal joint subluxation. Computed tomography imaging provided an accurate diagnosis.

PURPOSE: Head rotation can cause occlusion of the vertebral artery most commonly at the atlas loop, and repetitive compression from head turning induces vertebral artery dissection (VAD). Although ultrasound examinations are useful in diagnosis, dissected lesions unaccompanied by hemodynamic changes can be overlooked. Because the narrowed, dissected vessel in the atlas loop may cause rotational occlusion, we confirmed whether adding submaximal head rotation to a cervical ultrasound examination would facilitate the detection of VAD in the atlas loop.
METHODS: We investigated 7 patients who dev

STUDY DESIGN: This report describes a case of successful surgical treatment of eosinophilic granuloma of the atlas in a 3.5-year-old boy who presented with torticollis.
OBJECTIVE: The purpose of this report was to illustrate the rare clinical presentation of eosinophilic granuloma in the atlas.
SUMMARY OF BACKGROUND DATA: Eosinophilic granuloma, a benign solitary lesion that commonly affects children, has a variable clinical course. Although eosinophilic granuloma has been reported to occur in the cervical spine, there have been only five reported cases of eosinophilic granuloma affecting th

This review describes variation in modern and fossil occiput-atlas-axis complex anatomy of total group Tetrapoda with the aim of documenting the range of structural variation throughout their evolutionary history to establish grounds for comparison of the complex between tetrapod clades. This review reveals that every modern tetrapod has an atlas with morphology unique to its vertebral column that articulates to the skull, composed, typically, of paired neural arch halves and an intercentrum. Maximally complex tetrapod atlantes articulate to paired proatlas halves and are composed of paired ne

A review of the literature, with a short discussion of the possible aetiology, diagnosis and treatment of "la maladie de Grisel" is presented with a case report of atlanto-axial subluxation occurring in a child after pharyngoplasty. The occurrence of a "spontaneous" subluxation after pharyngoplasty has not been previously reported. The possible scale of neurological complications which may range from relatively minor nerve root compression to a potentially lethal major cord compression emphasises the importance of this condition especially as a minor trauma may precipitate a major disaster. We

The authors report the first known case of atlanto-axial subluxation in Reiter arthritis. The type of analysis, including radiographic and laboratory examinations, required for a diagnosis of atypical Reiter disease is an important feature of the case.

A case of atlantoaxial subluxation accompanying pharyngitis is reported in a 4-year-old child. Diagnosis was made by examination and roentgenograms of the lateral cervical spine. Treatment consisted of halter traction and bracing. The patient responded well to treatment and is normal in all respects one year later. Atlantoaxial subluxation should be considered when treating pharyngitis, especially if the patient presents with torticollis.

A 38-year-old man presented with progressive cervical myelopathy due to atlas hypoplasia associated with non-traumatic retro-odontoid mass. The neuroimaging findings suggested hypertrophy of the transverse ligament of the atlas. No histological confirmation of the retro-odontoid mass was obtained. Clinical manifestations improved after posterior decompression. Decompressive laminectomy of the atlas with or without fusion can achieve a good outcome in such cases.

A case of oropharyngeal injury associated with an unsuspected direct atlas anterior arch fracture is presented. Instability at the occipito-atlanto-axial joints developed because of the coexistence of a rather common congenital defect of the posterior atlas arch. Awareness of the frequency of congenital atlas anomalies and the possibility of direct cervical spine injury with pharyngeal trauma will aid early recognition of this problem and allow early effective treatment.
