Child abuse and neglect: diagnosis and management.

BACKGROUND: The findings of studies on the frequency of violence against children imply that many cases go undetected. METHODS: Selective literature review based on a search of different databases for publications on all types of violence against children, except sexual abuse. RESULTS: The physical abuse of children can involve blunt trauma, thermal injury, and the so-called shaking trauma syndrome (STS). Physical and psychological child neglect have very serious long-term effects. It can be difficult to draw a clear distinction of child abuse and neglect on the one hand, and acceptable beha…

The eye examination in the evaluation of child abuse.

Retinal hemorrhage is an important indicator of possible abusive head trauma, but it is also found in a number of other conditions. Distinguishing the type, number, and pattern of retinal hemorrhages may be helpful in establishing a differential diagnosis. Identification of ocular abnormalities requires a full retinal examination by an ophthalmologist using indirect ophthalmoscopy through a pupil that has been pharmacologically dilated. At autopsy, removal of the eyes and orbital tissues may also reveal abnormalities not discovered before death. In previously well young children who experience

A cognitive approach to child abuse prevention.

This investigation tested the incremental utility of cognitive retraining as a component within a program designed to prevent child maltreatment. High-risk families (N = 96) were randomly assigned to a control condition, home visitation modeled after the Healthy Start program (unenhanced home visitation), or home visitation that included a cognitive component (enhanced home visitation). Mothers were identified late during pregnancy or soon after birth, and their participation continued for 1 year. Lower levels of harsh parenting were found among mothers in the enhanced home visitation conditio

Pitfalls in the diagnosis of child sexual abuse.

Case 1: nurses in charge of a 6-year-old girl in a holiday camp noticed some blood spots on the girl's underwear. The possibility of sexual abuse was considered and the girl alleged that her father was responsible. The father was arrested. A surgeon was asked to examine the girl and planned to do it under general anesthaesia. Meanwhile, the girl was brought to the Forensic Medicine Unit. We found a normal hymen and no detectable anal lesions. The girl complained of dysuria and pollakiuria. Urinalysis revealed the presence of blood, leukocytes, and nitrite. Antibiotic treatment for lower urinar

The pediatrician's role in child abuse interviewing.

The history of sexual abuse that a child gives to the pediatrician may be the single most important factor in determining if a child has been abused. Most children have completely normal anogenital examinations; very rarely is definitive forensic evidence obtained or a sexually transmitted disease discovered. Physicians, in general, are given unique authority and privilege in reporting such hearsay evidence. The pediatrician often is the first professional with whom a child has contact when an allegation of abuse is made. Child protective issues then become paramount. It is important, therefor

Accumulating experience in a child abuse clinic.

OBJECTIVE: To examine the patient profile encountered in the first year of operation of the Child Abuse and Neglect Clinic of the Transvaal Memorial Institute. DESIGN: Record review of all cases presenting to the Clinic from May 1988 to April 1989. RESULTS: Females comprised just over 80% of the 227 patients. Sexual abuse was the presenting complaint in 89.8%. Most were young, 7% under 3 and 55% under 10 years of age. Almost one-third of the boys and 5.0% of the girls had chronic signs of anal abuse. Of the girls 56% had signs of chronic and 10% signs of acute vaginal abuse. Where the certai…

Accidents and child abuse in bathtub submersions.

Non-accidental bath drowning is an infrequently reported form of child abuse. Details of 44 children who suffered from drowning or near drowning in the bath were analysed from a two year (1988-9) UK study to investigate factors that might point to abuse. Cases of near drowning were notified through the British Paediatric Surveillance Unit inquiry system and drowning cases from the Office of Population Censuses and Surveys, the Scottish Government Record Office, and the Northern Ireland Office. In 28 cases the story was of accidental submersion with a baby of modal age 9 months being left unsup…

The worst combinations of child abuse and neglect.

We have studied the impact of various kinds of abuse and neglect on the child's perception of himself and his future. We found, when considering physical abuse, physical neglect, verbal abuse, emotional neglect, and sexual abuse, that less than 5% of these mistreatments occurred in isolation. Since the vast majority of mistreated children are subject to more than one kind of abuse or neglect, it was important to delineate which combinations have the greatest effect. We found that a combination of physical neglect, physical abuse, and verbal abuse had the greatest impact on children, affecting

[Oral injuries in cases of child abuse].

Child abuse is more common than previously supposed, and non-accidental injuries may also present in the oral cavity. Results of physical abuse are most commonly diagnosed as such, but injuries from sexual abuse and results of neglect may be observed in the mouth. Bruises are the most common types of non-accidental injuries, the lips and frena being specially exposed, through burns may be cause by over-hot food or caustic liquids. The teeth may fracture, be avulsed, displaced or discoloured as a result of non-accidental injury. Sexually transmitted diseases in children's mouths are strong indi…

Esophageal perforation by a tooth in child abuse.

A unique case of child abuse in a six-year-old Hispanic girl is reported. In this case, a tooth avulsed and swallowed during an episode of physical abuse and/or sexual abuse, caused an esophageal perforation resulting in a retropharyngeal abscess and mediastinal abscess containing the tooth. Although traumatic esophageal injuries in child abuse have occurred, the presence of a tooth within the mediastinal abscess is an unique manifestation of child abuse.

The role of the child in physical abuse: a reappraisal.

Friedrich and Boriskin's (1976) seminal review presented compelling evidence pointing toward the contribution of child factors in heightening risk for physical abuse. Indeed, many authors currently accept that certain child characteristics (e.g., prematurity, low birthweight) can directly lead to abuse. Much of the data in this area, however, is based on methodologically weak designs, and recent findings do not support the premise that children have a major role in the etiology of abuse. There is some suggestion that children with relatively circumscribed features may add to risk in families t

Patterns of reported child abuse and neglect.

Confirmed reports of abuse and neglect logged in a large state registry file from 1977 to 1984 are analyzed. Boys tend to sustain more frequent and more serious injuries. Victims of male perpetrators tend to sustain more serious injuries. A modest same-sex perpetrator/victim pattern is revealed for physical abuse; males are more likely to physically abuse boys while females are more likely to physically abuse girls. Among younger victims (aged 12 and younger) boys outnumber girls in all reporting categories except sexual abuse. Among adolescent victims, female victims greatly outnumber male vi

Developmental effects of child abuse: a review.

UNLABELLED: Most research investigating the effects of physical child abuse on children's development has been based on uncontrolled or poorly controlled studies having small samples, no long-term follow-up, and weak instrumentation. These studies, as well as some which have been more carefully designed, have generally suggested seriously detrimental developmental consequences in physical, intellectual, behavioral, and emotional areas. Some recent controlled follow-up studies challenge such a negative picture of abuse. In particular, a study by Elmer, which is probably the most carefully condu…
