Phenobarbital compared to benzodiazepines in alcohol withdrawal treatment: A register-based cohort study of subsequent benzodiazepine use, alcohol recidivism and mortality.

BACKGROUND: Long-acting benzodiazepines such as chlordiazepoxide are recommended as first-line treatment for alcohol withdrawal. These drugs are known for their abuse liability and might increase alcohol consumption among problem drinkers. Phenobarbital could be an alternative treatment option, possibly with the drawback of a more pronounced acute toxicity. We evaluated if phenobarbital compared to chlordiazepoxide decreased the risk of subsequent use of benzodiazepines, alcohol recidivism and mortality. METHODS: The study was a register-based cohort study of patients admitted for alcohol wit…

Effect of NMDA antagonists on rapid tolerance to benzodiazepines.

We have reexamined the effect of NMDA antagonists [(+)MK-801 and ketamine] on rapid tolerance to chlordiazepoxide. (+)MK-801 and ketamine blocked the development of rapid tolerance to chlordiazepoxide, but this effect was dependent on the dose ratio of the NMDA antagonist to that of the benzodiazepine used to produce rapid tolerance. Furthermore, NMDA antagonists blocked both learned and unlearned tolerance to chlordiazepoxide. It appears that in addition to impairment of memory and learning, NMDA antagonists may also influence some other mechanism involved in the production of drug-tolerance.

Attenuation of reperfusion hyperalgesia in the rat by systemic administration of benzodiazepines.

1. An investigation into whether reperfusion hyperalgesia is modulated by prior systemic administration of two benzodiazepine agonists (diazepam and chlordiazepoxide), and an antagonist (flumazenil) was conducted. 2. Transient ischaemia was induced in conscious rats by applying an inflatable tourniquet to the base of the tail; when the rats exhibited a co-ordinated escape response, the tourniquet was deflated and reperfusion of the tail was allowed. Reperfusion hyperalgesia manifested as a decrease in tail flick latency, following tail immersion in 49 degrees C water, after the release of the …

Cannabinoids. I. Behavioral effects.

The effects of two new cannabinoids, nabilone and canbisol, have been compared to delta 9-tetrahydrocannabinol (delta 9-THC) and chlordiazepoxide in behavioral tests in mice, rats, dogs and rhesus monkeys. Activity of mice was measured in a photocell device. Oral doses of 5 and 10 mg/kg of delta 9-THC and 200 mg/kg of chlordiazepoxide caused only a decrease in the initial high activity. Doses of 5 and 10 mg/kg of nabilone and 2.5, 5.0 and 10 mg/kg of canbisol decreased the initial high activity but increased the subsequent low activity. In rats delta 9-THC, nabilone and canbisol, but not chlor…

Modulation of gamma-aminobutyric acid responses in the rat optic nerve.

Depolarising GABA(A) receptor-mediated responses recorded from the optic nerve using a grease gap technique were modulated by classical potentiators of GABA(A) receptors. The benzodiazepine, chlordiazepoxide, the barbiturate, pentobarbitone and the widely used anaesthetic, propofol, all potentiated gamma-aminobutyric acid (GABA) responses. They did so with different maximal efficacies, propofol>pentobarbitone>chlordiazepoxide, and potencies on the basis of EC(50) estimates, chlordiazepoxide>propofol>pentobarbitone. The greater than expected GABA potentiating properties of propofol were explain…

The effects of compounds related to gamma-aminobutyrate and benzodiazepine receptors on behavioural responses to anxiogenic stimuli in the rat: extinction and successive discrimination.

In a first set of experiments rats were trained to run in a straight alley for food reward on a continuous reinforcement schedule and the running response was then extinguished. On the last 2 days of training and daily throughout extinction different groups of animals were injected IP with saline, 5 mg/kg chlordiazepoxide, 0.75 mg/kg picrotoxin, chlordiazepoxide + picrotoxin, chlordiazepoxide + 1.5 mg/kg bicuculline, 0.00125 or 0.25 mg/kg muscimol, 1 mg/kg baclofen, chlordiazepoxide + baclofen, or 0.00125 mg/kg muscimol + baclofen. Chlordiazepoxide increased resistance to extinction, a well-kn…

Intracranial self-stimulation distinguishes between two benzodiazepine antagonists.

Low doses of Ro 15-1788 and CGS 8216 were without effect on variable-interval self-stimulation, but completely abolished the enhancement of responding produced by chlordiazepoxide (5 mg/kg). Higher doses of Ro 15-1788, unlike other benzodiazepine receptor antagonists, produced an increase in response rates similar to that found after chlordiazepoxide. This result is consistent with its suggested action as a partial agonist. The combination of a high (benzodiazepine-like) dose of Ro 15-1788 with chlordiazepoxide produced a depression of responding similar to that seen with high doses of benzodi…

The GABA/benzodiazepine receptor complex in the central amygdalar nucleus and stress ulcers in rats.

The effects of bilateral microinjections of chlordiazepoxide and GABA into the central amygdalar nucleus on gastric ulcer formation induced by cold-restraint were examined in chronically implanted Wistar rats. Higher doses of chlordiazepoxide (20 and 30 micrograms/amygdala) significantly reduced stress ulcer development, whereas a lower dose (2.5 micrograms) produced a nonsignificant increase in ulcer severity. A similar dose/response pattern was observed following GABA administration. The benzodiazepine receptor antagonist Ro15-1788, applied to the amygdala, abolished the protective effects o…

Pharmacological properties of tracazolate: a new non-benzodiazepine anxiolytic agent.

Tracazolate (ICI 136,753, 4-butylamino-1-ethyl-6-methyl-1H-pyrazolo[3,4-b]pyridine-5-carboxylic acid ethyl ester) demonstrated dose-related anticonflict activity in rats and mice. The potency of tracazolate appears to be one-quarter to one-half that of chlordiazepoxide. No tolerance to the anticonflict activity of either tracazolate or chlordiazepoxide was evident following 12 consecutive days of treatment. Tracazolate exhibits a much greater separation between sedative and therapeutic doses than does chlordiazepoxide. Furthermore, based on rodent studies, tracazolate should be much less likel…

Multiple effects of drugs acting on benzodiazepine receptors.

The actions on [3H]noradrenaline release of chlordiazepoxide and FG 7142 were investigated in rat hippocampal synaptosomes. The release evoked by GABA, an action mediated by GABAA receptors, was enhanced by chlordiazepoxide and depressed by FG 7142. K+-evoked release, however, was depressed by both chlordiazepoxide and FG 7142 and occurred in the absence of GABA. The actions on both GABA-evoked and K+-evoked release were blocked by Ro 15-1788. The results suggest that the distinction between agonist and inverse agonist applies to the GABA-dependent but not to the GABA-independent action of ben…

Anxiolytic-like action of melatonin on acquisition but not performance of DRL.

The behavioural effects of melatonin have been attributed to a general reduction in motor activity; interference with memory fixation; a decrease in emotionality; or an anxiolytic action. The present experiments compared the effects of melatonin with an anxiolytic benzodiazepine, chlordiazepoxide (Librium), on a schedule of differential reinforcement of low rates of response (DRL) increasing 'burst' responding and premature responding. No doses of melatonin tested (0.03-8.1 mg/kg, IP) affected performance of well-learned DRL. Both low (0.03 mg/kg) and high (1.0 mg/kg) doses of melatonin impair…

A new sensitive method which unlike the Vogel test detects the anxiolytic effect of tofisopam.

A new, more sensitive than previously used anxiolytic test is described. The test consists in measuring of inhibition by punishment of drinking water necessary to swallow dry food by very hungry rats. This test reveals the anxiolytic properties of tofisopam, a clinically effective benzodiazepine anxiolytic, and of very low doses of chlordiazepoxide, both ineffective in the Vogel test, as well as anxiolytic properties of high doses of chlordiazepoxide and other anxiolytics, and confirms the lack of anxiolytic effects of major tranquilizers. The results suggest that chlordiazepoxide (and possibl…

Effects of some benzodiazepine derivatives on Triton WR-1339-Induced hyperlipidaemia in rats.

Oral administration of the benzodiazepines (diazepam, lorazepam, chlordiazepoxide, bipotassium chlorazepate) in Triton WR-1339-induced (200 mg/kg, blood collection 18 h later) hyperlipidaemia in rats elicited marked decrease of serum total lipids, total cholesterol and triglyceride levels, and alterations in free fatty acid and free glycerol content. The optimal doses for diazepam, lorazepam, chlordiazepoxide and bipotassium chlorazepate were estimated to be 5 mg/kg. Other benzodiazepines, namely oxazepam, medazepam and nitrazepam, elicited only minor changes in serum lipids levels, while with…

Indulging Curiosity: Preliminary Evidence of an Anxiolytic-like Effect of Castor Oil and Ricinoleic Acid.

In the process of validating the elevated zero maze, a common test of anxiety-like behavior, in our laboratory, we demonstrated an anxiolytic-like effect of castor oil and its primary component, ricinoleic acid. We tested the effects of vehicle and chlordiazepoxide in male mice in the elevated zero maze following a 30-min pretreatment time. Chlordiazepoxide is a United States Food and Drug Administration-approved drug that was previously shown to exert anxiolytic-like effects in both the elevated zero maze and elevated plus maze. Chlordiazepoxide was administered at doses of 5 or 10 mg/kg. We …

The effects of the beta-carboline FG 7142, on intracranial self-stimulation in the rat.

The effects of FG 7142 were examined, alone and in combination with chlordiazepoxide, on self-stimulation of the mid-lateral hypothalamus. Rewarding stimuli were delivered according to a 10-sec variable-interval schedule of reinforcement. FG 7142 (1-20 mg/kg) produced a dose-related depression in responding, and chlordiazepoxide (5 mg/kg) enhanced it. When these two drugs were given together, response rates did not differ significantly from control rates.

Clinical pharmacology and therapeutics of benzodiazepines.

Benzodiazepines are among the most commonly prescribed drugs in the world. In contrast to their extensive use, the therapeutic indications and potential of benzodiazepines are limited. All benzodiazepine derivatives available in Canada are similar structurally and in their pharmacologic actions. Few have specific advantages over any others. For example, no benzodiazepine has been shown to be superior to chlordiazepoxide in the treatment of acute anxiety, chronic anxiety neurosis or insomnia. Barbiturates should not be prescribed for these problems since benzodiazepines are just as effective an…

A comparison of the porphyrin-inducing activity of barbiturates and benzodiazepines in chick embryo liver cells.

Five benzodiazepines, flurazepam, nitrazepam, diazepam, oxazepam, and chlordiazepoxide, have been tested for porphyrin-inducing activity in chick embryo liver cell culture and for delta-aminolevulinic acid (ALA) synthetase inducing activity in the 17-day-old chick embryo. Flurazepam and nitrazepam were found to have considerably lower potency than secobarbital whereas diazepam, oxazepam, and chlordiazepoxide were less potent than phenobarbital in these test systems. The following conclusions were arrived at. (1) A hypnotic dose of flurazepam or nitrazepam would be less likely than a comparable…

Anxiolytic drugs and the acquisition of conditioned fear in mice.

Previous studies have shown that, in rodents, chlordiazepoxide and other benzodiazepines can interfere with learning in passive avoidance or conditioned suppression procedures. The most consistent effects are observed when the drugs are administered before the acquisition trial and subjects are re-tested in the non-drugged state. It is not clear, however, whether this effect on learning is associated with the behavioural depressant actions of these drugs. In the present study mice were injected with chloridiazepoxide, diazepam, zopiclone, or CGS 9896 and locomotor activity measured in a two-co…

Benzodiazepine tolerance, physical dependence and withdrawal: electrophysiological study of spinal reflex function.

The in situ segmental spinal reflex system of the rat was used to determine changes in excitatory and inhibitory synaptic function associated with benzodiazepine tolerance, physical dependence and withdrawal. Rats were made physically dependent on chlordiazepoxide using a chronically equivalent dosing method. After spinalization, dorsal and ventral lumbar roots (L5 or L6) were isolated for extracellular stimulation and recording. Testing of spinal function was performed during peak withdrawal (8 days) and at peak effect (4 hr) after first ("acute") and last ("chronic") dose of chronically equi…
