Listeria monocytogenes cholecystitis.

Listeriosis usually manifests at the extremes of age, during pregnancy or among immunocompromised individuals as an acute meningoencephalitis with or without associated septicemia. Localized infections are rare. We investigated the incidence of Listeria monocytogenes cholecystitis using the data of the Listeria reference laboratory of the University of Würzburg/FRG. Out of 467 culture proven L.m-infections in the years 1986 and 1987, two cases were localized infections of the gall bladder. Although studies of the microbiology of gallbladder infections have not demonstrated the recovery of List…

Acute acalculous cholecystitis.

Sixty-three patients, 49 men and 14 women, developed acute cholecystitis without gallbladder stones. Only eight patients had a history suggestive of gallbladder disease. In 17 patients cholecystitis developed in the postoperative period, and cholecystitis occurred in 7 patients who had extensive trauma. The signs and symptoms did not differ markedly from those found when acute cholecystitis is associated with cholelithiasis. Pain and tenderness in the right upper abdominal quadrant, vomiting, abdominal distention, decreased bowel sounds, jaundice and fever were common. Thirty (47.6 percent) ga…

Brucella-induced cholecystitis.

Acute cholecystitis is a rare complication of systemic brucellosis. This report details the occurrence of acute noncalculous cholecystitis in which Brucella suis was cultured from both the blood and the gallbladder. A discussion of the various diagnostic tests and the recommended therapy for brucellosis is included.

[Acute emphysematous cholecystitis].

Emphysematous cholecystitis is a rare form of acute cholecystitis, characterized radiographically by the presence of gas within the gallbladder. We report of a patient, who was admitted to the hospital with the diagnosis of acute abdomen. This patient had an emphysematous cholecystitis caused by Clostridium perfringens. We found the wall of the gallbladder emphysematous and gangrenous, the gallbladder was distended and contained purulent material, but no stones. However, in addition, the films of abdomen showed gas in the ducts. Diagnosis, pathogenesis and the aetiological and therapeutical as…

Haemophilus aphrophilus cholecystitis.

Cultures from the gallbladder and blood of a 60-year-old man with acute cholecystitis grew Haemophilus aphrophilus. This organism, an unusual isolate in clinical specimens, is most frequently seen in patients with either endocarditis or brain abscesses. Haemophilus aphrophilus may be distinguished from Eikenella corrodens and Actinobacillus actinomycetemcomitans on the basis of colonial morphology and the biochemical tests for oxidase and catalase production and fermentation of lactose, sucrose, glucose, mannitol, xylose, and trehalose.

[Lamblia-induced cholecystitis].

Case reports of 32 patients with lambliasis induced cholecystitis, treated in the surgical clinic for a 28-year period, are analysed. The clinical data allow establishing the differential diagnosis between cholecystitis attack induced by the presence of Lamblia in bile ducts and an attack induced by the presence of concretions in the gallbladder. It is the authors' opinion that the complex exploration of patients should be carried out. A carefully selected anamnesis, data of multiple duodenal sounding and coprologic studies serve as a valuable adjunct in establishing the correct diagnosis and …

Acute emphysematous cholecystitis.

Acute emphysematous cholecystitis is an uncommon condition caused by gas-forming organisms and characterized by the presence of gas in the wall and lumen of the gallbladder. Its incidence is higher among male diabetics. AEC in an elderly North American diabetic man with Indian ancestry is reported with a brief review of the world literature. The diagnosis was made preoperatively with the aid of plain radiographic films of the abdomen. A gangrenous distended gallbladder was removed at operation. Clostridium perfringens was cultured from the gallbladder contents and wall. If AEC is suspected, in…
