If the biliary tract is well seen, but the gallbladder remains unvisualized, one may conclude an obstruction of the gallbladder duct. The author shows by means of 108 cases, confirmed by surgery, that operation is indicated if the duct of the gallbladder is obstructed: Although mechanical obstruction of the cystic duct with a calculus was seen in only 10 cases (9,2%) at the time of operation, calculi were found in 101 patients (93%) in the biliary tract system, presenting with cholecystitis and mucosal oedema. A percutaneous method was used to remove stones from otherwise normal gall bladders, as assessed by cholecystography and ultrasonography. The procedure was performed in a single stage under general anaesthesia, adopting the method and instruments used for one stage percutaneous nephrolithotomy. Benign tumor and pseudotumor of the gallbladder are infrequently reported in our country. In this paper, 5 patients with benign tumor (3 tubular adenoma, 1 leiomyoma and 1 fibroma) and 35 pseudotumor (26 cholesterol polyps and 9 inflammatory polyps) of the gallbladder are presented. Most patients presented a typical clinical manifestation of chronic cholecystitis. Oral cholecystography gave a lower detection rate. A series of 17 cases of gallstones in children observed over a period of 10 years in the area of Marseilles is described and the literature reviewed. The majority of children were girls over 10 years old. The records of 242 patients, operated consecutively for biliary lithiasis, were analyzed to determine the reliability of oral cholecystography (OCG), ultrasonography (US), and HIDA in detecting biliary calculi. Preoperative interpretations were correlated to operative findings. OCG obtained in 138 patients was accurate in 92%. US obtained in 150 was correct in 89%. The accuracy of HIDA was 92% in acute and 78% in chronic cholecystitis. A series of 412 personally performed cholecystectomies is analyzed and discussed. Surgical cases were divided into groups based on whether or not the gallbladder was visualized by cholecystography. A third group required immediate surgery without attempting radiologic investigation. We do not accept the diagnosis of dyskinesia which is a radiological finding as an indication for surgery and no such cases are included in the series. Nine surgically proven congenital absence of gallbladder (CAGB) cases were reviewed. All of them had one or more kinds of biliary symptom. Tests such as abdominal ultrasonography, intravenous or oral cholecystography and even endoscopic retrograde cholangiography not only failed to predict CAGB but misleadingly indicated other similar conditions.
