Chylothorax is a well-known, albeit infrequent, complication in certain neoplasias including lymphomas. The continuing loss of chyle leads to a profound deterioration in patients' nutritional and immunological status, prevented only by appropriate early nutritional support. There is currently some dispute over its handling, particularly with regard to the most appropriate type of nutritional support and the suitability of conservative treatment versus surgery. The present paper describes the case of a 41-year-old patient diagnosed as having secondary chylothorax following B-cell lymphoma where…

STUDY OBJECTIVE: To establish guidelines for the diagnosis and management of chylothorax in children.
DESIGN: Retrospective study.
PATIENTS: Fifty-one patients with a diagnosis of chylothorax. Twelve patients were excluded because of incomplete data or incorrect diagnosis. The following parameters were analyzed: triglyceride level, total cell number, and lymphocyte percentage; amount of pleural effusion on day of diagnosis, day 5, and day 14; and total time of pleural effusion. Prospectively, the same parameters were analyzed in a control group of 10 patients with pleural drainage.

Chylothorax was associated with constrictive pericarditis in a 6-year-old mixed-breed dog. Clinical signs included hepatomegaly, bilateral jugular pulses, muffled heart sounds, and dyspnea. Pleural effusion was identified on thoracic radiographs. Thoracentesis yielded 3 L of chylous effusion, confirmed by the cholesterol/triglyceride ratio (0.14). Echocardiography revealed a thickened pericardium, with numerous fibrin tags, which was suggestive of constrictive pericarditis. Central venous, right atrial, and right ventricular pressures were high and on right ventricular diastolic pressure traci…

Thirty patients diagnosed as chylothorax and chyloperitoneum (13 of chylothorax, 9 of chyloperitoneum and 8 of both) in PUMC Hospital from 1923 to 1989 were reported. Thus, the incidence rate of these two diseases is rather low. The commonest causes were iatrogenic trauma (9 cases) and malignant tumor (7 cases). Other causes were tuberculosis (5 cases), lymphangioma (2 cases), Behcet's syndrome (1 case) and non-iatrogenic trauma (1 case). The etiology was not known in 5 cases. Acute episode was observed in patients with traumatic chylous effusion; it was usually manifested as dyspnea or abdomi…

Chylothorax is an uncommon occurrence seen most frequently in patients with malignancy. We report a case of chylothorax following fracture-dislocation of the thoracolumbar spine, the ninth reported case in the English literature. Seven cases of chylothorax were identified in a 12-year period at our institution. A total 925 patients sustained fractures of the thoracic or lumbar spine during this period, and this was the only case associated with chylothorax. We have reviewed the literature, and recommend conservative management utilizing closed thoracotomy drainage and total parenteral nutritio…

Chylothorax, the accumulation of lymphatic fluid within the pleural cavity, results from disruption or obstruction of the thoracic duct or its tributaries, or from transdiaphragmatic chylous ascites. When severe, this condition may impair respiratory function and induce metabolic complications because of chronic loss of protein and lipids. Management strategies ranging from dietary modifications, medical therapy, surgical ligation, pleurodesis, and lymphatic embolization, generally follow a tiered approach. Special considerations apply to chylothorax associated with central venous obstruction …

Yellow nail syndrome (YNS), a rare condition characterized by thickened yellow nails, lymphedema, and respiratory manifestations, is rarely caused by bucillamine, an anti-rheumatic agent. Respiratory manifestations of YNS include chylothorax in 18.8-22% of patients with pleural effusion. We herein report the first case of bucillamine-induced YNS with chylothorax. Lymphangiography revealed narrowing of the lymphatic vessels and stagnation of the lymphatic fluid, suggesting impaired lymphatic drainage. The chylothorax persisted despite the discontinuation of bucillamine. The present case illustr…

Dasatinib, a BCR-ABL tyrosine kinase inhibitor, is used in the management of Philadelphia-positive chronic myeloid leukemia (CML). Several adverse complications of this targeted immunotherapy have been reported. This case report focuses on a 79-year-old female who presented with acute dyspnea and generalized chest pressure while undergoing management with this specific tyrosine kinase inhibitor. Bilateral chylothorax was diagnosed with the aid of imaging, laboratory studies, and diagnostic thoracentesis. No other risk factors, including trauma, lung malignancies, or congenital anomalies, were …

Everolimus is an orally administered mechanistic target of rapamycin inhibitor in solid-organ transplant patients. In addition to the common adverse side effects of this treatment, such as hyperlipidemia, rash, stomatitis, anorexia, diarrhea, anemia, thrombocytopenia, and leukopenia, pulmonary toxicity is also an important adverse side effect. Although pulmonary toxicity due to everolimus has been reported mostly as pneumonitis, cases of pleural effusion due to everolimus have also been reported rarely. Chylothorax is defined as the accumulation of lymphatic fluid in the pleural space. It may …

Dasatinib is a tyrosine kinase inhibitor drug used for chronic myeloid leukaemia (CML) treatment. Chylothorax has been rarely reported as a secondary effect of dasatinib occurring especially in long-term treated patients, although its pathophysiology is not yet fully understood. Laboratory analysis of the pleural effusion is crucial for chylothorax diagnosis. We report a case of a 53-year-old male patient presenting a chylothorax after 14 years of dasatinib therapy where the clinical laboratory was key in the diagnosis.

Dasatinib is a potent second-generation tyrosine kinase inhibitor (TKI) used in the first- and second-line treatment of chronic myeloid leukemia (CML). Chylothorax is a rare presentation that results in chyle leakage from the lymphatic system into the pleural space as a consequence of thoracic duct damage. Pleural effusion has been reported frequently in patients treated with Dasatinib however chylothorax has been rarely reported. Here we report an 18year old female presenting with chylothorax after 63 months of Dasatinib intake along with a review of the relevant literature. Currently there a…

Chylothorax, an uncommon cause of pleural effusion, results from the accumulation of lymph in the pleural space due to damage or obstruction of the thoracic duct. Chylothorax can be due to several aetiologies, many of which are rare, and it is often a diagnostic challenge to identify the cause. This case report refers to a patient with rapid recurrent episodes of bilateral pleural chylothorax due to damage of the thoracic duct after external electrical cardioversion treatment. The diagnosis took place by the method of exclusion, when all known causes of chylothorax, both non-traumatic and trau…

Chylothorax secondary to thorax drainage is a rare entity, with only five previously reported cases. A patient that sustained injuries after an accident involving a power tool, originally seen at another service, presented with a metal fragment embedded in the 4th intercostal space of the right hemithorax and ipsilateral pneumothorax. The patient underwent thoracic drainage and was referred to our hospital. After one day, the drained fluid became milky and laboratory tests confirmed chylothorax. Videothoracoscopy disclosed compression of the thoracic duct topography by the drainage tube.

Chylothorax is a potentially deadly complication that can occur after thoracoabdominal aortic aneurysm (TAAA) repair. We describe our contemporary experience (2005-2014) with this complication, our efforts to identify perioperative variables associated with it, and our attempts to assess treatment outcomes. We reviewed the records of 1092 consecutive patients who underwent TAAA repair between 2005 and 2014. Standard bivariate analysis was used to test for between-group differences. Eleven patients (0.9%) developed postoperative chylothorax. Nonoperative management was used in 8 of these patien…

Nontraumatic chylothorax is a relatively rare condition in which the intestinal lymph (chyle) leaks into the pleural cavity. Nontraumatic chylothorax is more difficult to treat than the more common traumatic chylothorax because the site of chylous leak may occur in less predictable locations. In the past, patients with nontraumatic chylothoraces were offered traditional fluoroscopically guided lymphangiography and thoracic duct embolization similar to traumatic chylothorax. However, the observation that thoracic duct embolization outcomes for nontraumatic chylothorax differed based on the imag…

Chylothorax is a rare disease caused by both traumatic and nontraumatic events. Chylothorax can cause cardiopulmonary abnormalities and significant nutritional, metabolic, and immunologic consequences. We present an exceptional case of chylothorax due to penetrating chest trauma. The diagnosis was made by thoracentesis. Conservative management with nothing by mouth and total parenteral nutrition failed; therefore the patient needed surgical closure of the duct leak.
