Operative Fixation of a Displaced Midshaft Clavicle Fracture.

Midshaft clavicle fractures are frequently encountered orthopaedic injuries. Significant displacement portends a higher likelihood of nonunion and symptomatic malunion after nonoperative treatment. Surgical fixation has been shown to reduce the rate of these adverse outcomes. However, the benefit of surgery is narrowly defined and the decision to offer operative treatment involves a careful analysis of patient clinical factors, expectations, and concomitant injuries. In North America, fixation is most commonly performed with a precontoured plate and screw construct. Frequently, fracture patter

Clavicle fixation screw impingement causing subclavian artery pseudoaneurysm.

Vascular injuries are an extremely rare complication of clavicle osteosynthesis, though several cases are reported in the literature. Cadaver studies have established recommended safe lengths for clavicle screws based on precise measurements of bone thickness and distance from subjacent neurovascular structures. We present the case of a 27-year-old female who underwent orthopedic plate and screw fixation of a clavicle fracture and subsequently sustained iatrogenic vascular injury from one of the screws. This necessitated endovascular repair and orthopedic revision. This case underscores the im

Technique Tip: Platysma-flap Approach to the Clavicle.

The surgical treatment of a displaced fracture of the clavicular shaft generally consists in an open reduction and plate fixation. The most common complications of the classic direct "transplatysma" approach are hardware related. These consist of simple hardware irritations requiring secondary plate removal but also feared wound healing problems ranging from dehiscence to superficial and deep wound infections. In the present article, we describe the so-called "platysma-flap" approach in which the platysma muscle is entirely preserved and distally raised as a flap. This modified exposure allows

Surgery for the fractured clavicle: factors predicting nonunion.

BACKGROUND: This study identifies the reasons for failure after plate osteosynthesis of midshaft clavicle fractures, complication rates, and time to radiographic union. METHODS: A retrospective review of 84 consecutive patients who had undergone surgical fixation for a midshaft clavicle fracture was performed. RESULTS: There were 82 patients who were included for analysis and operated on by 11 different surgeons using a mixture of locking (63%) and nonlocking (37%) plates. The rate of osteosynthesis failure was 12.2%. A logistical regression analysis found that failure of osteosynthesis had

Computerized tomography based 3D modeling of the clavicle.

UNLABELLED: Previous studies have suggested clavicular morphology is highly variable, particularly in the lateral retrocurved section. Current clavicle fracture plating systems require three dimensional intra-operative contouring to achieve adequate fit and necessitate variable soft tissue dissection placing fracture perfusion and muscular attachments at risk. The aim of this study was to search for a surgically relevant superficial shape pattern. This is a retrospective CT-based analysis of 174 non-pathological clavicles in 95 adults (45 females, 50 males). Using the principle of cylindrical

[Middle third clavicle fractures--diagnosis, complications and treatment].

Clavicle fractures compose 2.5% to 5% of all fractures, and usually occur due to a direct blow to the shoulder. The incidence of these fractures persistently increases. Most of the fractures are in the middle third--a result of the unique structure of the clavicle and its ligamentous and muscular attachments. The fracture diagnosis includes focused physical examination, radiographic evaluation, and comprehensive evaluation of the patient, considering the fact that the fracture can be the result of significant energy absorption and can hide other injuries. Usually treatment is non-operative, an

[Congenital pseudarthrosis of the clavicle: 25 childhood cases].

PURPOSE OF THE STUDY: Congenital pseudarthrosis of the clavicle is rare, only 200 cases having been reported. Based on 25 personal cases and an overview of the literature, we try to explain the etiology of this condition and the different kinds of treatment. MATERIAL AND METHODS: A retrospective analysis was performed on twenty-five children (16 females, 9 males, mean age at the end of the follow up - 11.5 yrs) from three different centers. We assessed the outcome of surgical and nonsurgical procedures, based on pain, functional ability, cosmetic results, and x-ray examination. RESULTS: A fa

Atraumatic osteolysis of the distal clavicle: MR findings.

OBJECTIVE: The purpose of this study was to describe the MRI appearance in atraumatic osteolysis of the distal clavicle (AODC). MATERIALS AND METHODS: We retrospectively evaluated MRI, medical records, ancillary diagnostic imaging studies and clinical course in five men and two women (mean age, 39 years) in whom the final clinical diagnosis of AODC was established. None of the patients had significant shoulder injury, but all participated in activities involving repetitive strain of the acromioclavicular (AC) joint. In three of these patients, we performed follow-up MRI (ranging from 5 1/2 to

Reflex sympathetic dystrophy after clavicle fracture: case report.

Reflex sympathetic dystrophy (RSD) of the anterior chest wall is a rare entity. A MEDLINE search in all languages of the world's literature from 1966 to the present yielded no report of the association of clavicle fracture with this pain syndrome. Two cases are presented and the anatomic variations of the supraclavicular nerves in relation to the clavicle are discussed.

Pediatric and Adolescent Clavicle Fractures: To Fix or Not to Fix?

Middiaphyseal clavicle fractures are prevalent in children and adolescents, and they are typically treated nonsurgically because of their strong bone-healing capabilities. However, increasing trends toward surgical intervention in adult patients have sparked debate over whether younger populations could experience similar benefits from the surgical approach. Recent studies have suggested that surgical fixation may offer quicker pain relief and faster recovery, particularly in adolescents and specific fracture cases involving severe displacement or complications. However, despite these potentia

[Treatment concepts for the medial clavicle and the sternoclavicular joint].

Medial clavicle fractures and injuries to the sternoclavicular joint are rare injuries but can have life-threatening consequences. There are no standardized treatment algorithms or guidelines for the diagnostics and treatment. This article provides an overview of the individual topographies as well as the conservative and surgical treatment strategies.Conservative treatment is preferred for medial clavicle fractures. The indications for surgical treatment are variable but this is frequently carried out if there is a fracture displacement > 1 cm or 1 shaft width and high functional demands. In

The aim of this study was to conduct a systematic review of relevant case series about the argument to find an up-to-date base of evidence for treatment choice. (2) Methods: an electronic literature research of Ovid, MEDLINE and the Cochrane Library databases was conducted, and articles were selected based on inclusion criteria. Demographic data, clinical features, treatment options, outcomes and complications were analyzed. (3) Resu

The incidence of congenital pseudarthrosis of the clavicle and review of the literature.

The aim of this study was to identify the hitherto unknown incidence of congenital pseudarthrosis of the clavicle (CPC), based on a cohort of continuous livebirths born in our hospital, to review the literature and investigate if there is evidence supporting the published association between left-sided CPC and dextrocardia. From our electronic medical record and radiology databases, we identified all live births and patients with the diagnosis of CPC born from 2000 to 2016. We reviewed the imaging which included one or both clavicles to search for unrecorded CPC cases and reviewed all retrieva

Plunging Dangerously: A Quantitative Assessment of Drilling the Clavicle.

Bicortical drilling of the clavicle is associated with risk of iatrogenic damage from plunging given the close proximity of neurovascular structures. This study determined plunge depth during superior-to-inferior clavicle drilling using a standard drill vs drill-sensing technology. Two orthopedic surgeons drilled 10 holes in a fresh cadaveric clavicle with drill-sensing technology in freehand mode (functions as standard orthopedic drill) and another 10 holes with drill-sensing technology in bicortical mode (drill motor stops when the second cortex is breached and depth is measured in real time
