An 11-year-old boy with a history of behavioral problems was admitted to the inpatient Child Development Program of St. Louis University Medical Center. A diagnosis of Tourette's syndrome was made, and when family objected to a trial of haloperidol for fear of tardive dyskinesia, clonidine was initiated. A temporary period of worsening of motor and phonic tics ensued, while dose adjustments were made. When a maintenance level was achieved, approximately 2 weeks after initiating treatment, remission of symptoms was observed.

Experimental evidence that supports or rejects an involvement of opioid receptors in the cardiovascular effects of clonidine was sought. In anesthetized rabbits, clonidine decreased blood pressure and heart rate. These effects were related to the clonidine dose. High doses of clonidine produced a biphasic response in blood pressure without resulting in a significant baroreflex. However, it was not difficult to produce hypotensive action of clonidine without the biphasic action using low doses of clonidine. We evaluated the tolerance and effectiveness of the oral clonidine test for GH in 75 children, 84% with hyposomia and 16% with other diseases. The test was well tolerated, since 97% of the examined children had no side effects with the exception of occasional drowsiness, pallor and myosis of short duration. 23 patients presenting with symptoms of pheochromocytoma were subjected to the clonidine suppression test. In 6 patients, in whom a pheochromocytoma was surgically approved later on, norepinephrine was not suppressed by clonidine. 17 patients in whom a secondary form of hypertension could be excluded by other diagnostic measures showed a marked decrease of plasma norepinephrine. Apart from drowsiness, no adverse side effects were observed. In our hands, the clonidine test proved as a safe and specific test in the diagnosis of pheochromocytoma.

Methylxanthines increase central noradrenergic turnover by an unknown mechanism. Isobutyl methylxanthine (IBMX, 0.5-16 mg/kg i.v.) was found in this study to increase firing rats of noradrenergic neurons in the locus coeruleus, supporting increased impulse flow as one possible mechanism for increased noradrenergic turnover. Naloxone (1 mg/kg i.p.) pretreatment potentiated the effects of IBMX, while clonidine (0.01 mg/kg i.v.) reversed increases in activity. Some of the behavioral effects of methylxanthines are consistent with increased noradrenergic activity.

Clonidine, injected i.p. 20 min before cold exposure, is able to counteract the blockade induced by alpha-methyl-p-tyrosine of cold-stimulated release of TSH in rats. Under basal conditions the alpha-adrenoceptor stimulating agent, administered into the lateral cerebral ventricle (10 microgram), produces, at 30 min, a sharp increase in plasma TSH that returns to control levels at 60 min. When the drug is given intraperitoneally (0.4 mg/kg) it causes a significant decrease in plasma TSH 120 min after the treatment.

In cats under chloralose-urethan anaesthesia, tachyphylaxis developed to the hypotensive effect of clonidine. Development of this tachyphylaxis was slow in the case of intravenous low doses (10--20 micrograms/kg) and rapid after high doses (50 micrograms/kg). INTRODUCTION: Neonatal abstinence syndrome (NAS) is a complicated medical condition with treatment regimens that traditionally have included methadone and other opioids, barbiturates, and benzodiazepines. Clonidine is experiencing increasing use in the pediatric population as a sedative and analgesic because of its central alpha2-adrenergic agonism. We report three cases of preoperative use of intranasal clonidine in pediatric patients, all for different indications. One patient was treated for preoperative agitation and hallucinations associated with oral midazolam. One patient was given clonidine as a premedicant. The third patient was treated for preoperative agitation and hypertension. All three patients had subjective resolution of indicated symptoms and none experienced adverse outcomes.
