We report a 2 1/2 years old female child who presented with Malignant Sacrococcygeal Yolk sac (Endodermal Sinus) tumor as presacral mass with ulceration of overlying skin. Her ultrasonography (USG)abdomen revealed a huge presecral mass with irregular margins extending in to the pelvis, pushing the rectum antero-laterally. CT scanning of abdomen confirmed USG finding with out enlargement of retroperitoneal lymph nodes and no involvement of the liver. Serum AFP was abnormally elevated. The tumor was excised along with Coccyx in-toto through posterior approach dividing the sacrum and 3-Drugs (VAC…

There have been no previous concrete reports of leukemoid reactions associated with squamous cell carcinoma originating in cutaneous tissue. Here we report a case of epidermal squamous cell carcinoma of the sacral region and an associated leukemoid reaction. The tumor invaded deeply and destroyed both the sacrum and coccyx. The white blood cell count was greater than 20,000/mm3. After resection of the tumor, white blood cells transiently decreased, but did not fall under 10,000/mm3. Post-operative infection by methicillin-resistant Staphylococcus aureus and Bacteroides caccae caused sepsis and…

Four Japanese male students, aged 14, 16, 18 and 19, were found to have a nodule in their coccygeal regions. The typical lesion was a rugby- … d to the saddle of the bicycle. X-ray abnormalities included bending or anterior dislocation of the coccyx in all cases and spina bifida in two cases. These nodules may have developed as a result of irritation of this region by the saddle of the bicycle.

Ano-rectal neuralgia consists of pain in the ano-rectal region without any underlying organic proctological disease. Classically, three major clinical groups are defined, although associated, successive and intermediary forms may also exist. Proctalgia fugax is the most distinctive condition, consisting of brief episodes of nocturnal pain recurring at very variable intervals. In coccygodynia, the pain is postural and reproduced by pressure and mobilisation of the coccyx. The other forms of ano-rectal pain are grouped under the heading ano-rectal neuralgia or neurosis, which are characterised b…

We report a 46-year-old man who presented with a 2week history of worsening headaches and acute onset left sided hemiplegia. He had undergone a surgical resection of a sacral chordoma 13years prior, followed by adjuvant radiotherapy and chemotherapy. MRI revealed multiple enhancing lesions in the brain, and the two largest were resected. The histopathology was consistent with chordoma. Sacrococcygeal chordomas are locally invasive notochord-related sarcomas. They rarely metastasize to the brain, and only eight patients have been reported. While currently available adjuvant radiotherapy and sys

Human tails have been attributed to a disturbance in fetal tail regression which normally occurs at the gestation age of about 6 weeks. To date, more than 100 cases of human tails have been reported. However, reports of true human tails, which involve the coccygeal vertebrae, are rare. We recently encountered a patient with a true human tail which involved the coccygeal vertebrae and was accompanied by lumbar spinal lipoma and spina bifida. A four-year-old boy was brought to our clinic with complaints primarily of painful mass. The boy had no neurological abnormalities. Physically, a tail bone

Articular lesions were studied in the extremities and vertebral column in murine erysipelas infection. Pathologic alterations found in those different joint systems were essentially the same in character. In the early stage of infection, phlegmon-like inflammation was observed in the periarticular loose connective tissue. Abundant fine Gram-positive bacilli were free in inflamed edematous tissue. Then, active serofibrinous exudation and emigration of leukocytes occurred in the synovial membrane, articular cartilage, annulus fibrosus, and nucleus pulposus with the development of the disease. Hidradenitis suppurativa is a chronic inflammatory skin disease characterized by recurrent nodules, abscesses, and sinus tracts, most often located in axillary, inguinal, and gluteal regions. Malignant transformation into squamous cell carcinoma is a rare but serious complication. We present the case of a 50-year-old man with ˃10 years of poorly controlled hidradenitis suppurativa who developed painful gluteal discharge and multiple exophytic lesions. Imaging demonstrated large perianal and perineal masses with destruction of the coccyx, but no distant spread. Biopsy confirmed well-differentia…

This review summarises the anatomy and lymphatic systems around the pelvic floor. We investigated the lymphovascular network in the anorectal region, focusing on the hiatal ligament, which comprises smooth muscle fibres derived from the longitudinal muscle and connecting the anal canal and coccyx, and the endopelvic fascia, which seems to comprise collagen and elastic fibres. During rectal surgery, endopelvic fascia is recognized as a sheet of fascia covering the levator ani muscle. Endopelvic fascia is extensively attached to the smooth muscle fibres diverging from the longitudinal muscle of …

Possible causes of sacral and low back pain in the postpartum patient include sacroiliac joint dysfunction, sacroiliitis, lower lumbar diskitis, and irritation of the sciatic nerve. Postpartum stress fracture is a recognized cause of pain that should be considered in the differential diagnosis of the postpartum patient's low back pain. Several case reports of postpartum stress fracture are now in the literature (1, 2, 3, 4, 5, 6, 7, 8). A 30-year-old female presented postpartum with pain in the coccyx region that was most severe nine weeks after the uneventful spontaneous vaginal delivery of h…

We describe here a newborn with a de novo 22.6Mb interstitial deletion of chromosome 5q22.3. The clinical findings included brachycephaly, a high forehead, hypertelorism with prominent eyes, low-set ears, clenched hands, club feet, a prominent coccyx with hair, ambiguous genitalia, inguinal hernia, heart defect and severe failure to thrive. This case had a more severe phenotype, compared with the previous reports of interstitial 5q syndrome. High resolution multicolor banding and array comparative genomic hybridization (array CGH) analysis delineated the breakpoints at 5q22.3 and 5q31.2. There…

Tailgut cyst (TGC) is an uncommon congenital lesion, located in the retrorectal/presacral space. We report a 12-year-old girl with lumbar pain and a retrorectal mass. She had mental retardation, hypothyroidism, didelphus uterus, sacrum vertebrae, and coccyx agenesis, without chromosomic anomalies. Three surgeries were performed for the complete excision of the tumor. Microscopically, the mass consisted of cystic spaces lined by a wide variety of epithelia and a stroma composed of fibrous tissue containing bundles of smooth muscle fibers. According to these findings, the diagnosis was TGC. This…

This is a case report of a 2-year-old male who presented with walking disability and sphincter incontinence since birth. His mother has diabetes mellitus. The patient had a normal mental function, and his lower extremities were hypoplastic and akinetic. Spinal neuroimaging investigations revealed agenesis of the spinal column below T9 vertebral level with abrupt termination of the spinal cord at the T6 vertebral level. With this severe caudal regression syndrome (CauReS), cardiac, anorectal, urologic, and orthopedic consultations were made looking for other potential associated malformations.

BACKGROUND: Coccygectomy may be indicated for the treatment of debilitating coccygodynia unresponsive to non-operative treatment. Perineal contamination and postoperative wound infection following coccygectomy remains a major concern. We present a rare post-coccygectomy complication of rectal-cutaneous fistula. To our knowledge no such case has been previously described.
CASE PRESENTATION: A 24-year-old woman presented with recurrent wound infections 1 year after coccygectomy at another institution, which persisted despite two surgical debridements and antibiotic treatment. Wound cultures sho
