Abraham Colles (1773-1843) was a pioneering Irish surgeon whose contributions significantly advanced the fields of anatomy and surgery. Best known for describing the "Colles' fracture" of the distal radius, Colles' meticulous clinical observations and innovative treatment methods remain influential in modern orthopaedic practice. Born in Kilkenny, Ireland, Colles pursued medical education at Trinity College Dublin and the University of Edinburgh before training in London under the renowned surgeon Astley Cooper. Upon his return to Dublin, Colles was appointed resident surgeon at Steevens' Hosp…

BACKGROUND: The scaphoid fractures account for 50%-80% of all carpal bone fractures in young individuals. Non-union of the fracture occurs in approximately 5%-10% of undisplaced scaphoid fractures. Current management varies significantly among different places and surgeons.
OBJECTIVES: The purpose of this review is to investigate the evidence of the effectiveness and safety of various treatments of acute scaphoid fractures.
METHODOLOGY: Systematic review and metanalysis of all the randomised and quasi-randomised trials comparing different treatments of acute scaphoid fractures.
RESULTS: Thi

We analyzed 366 consecutive patients treated for low-energy trauma fractures below the elbow who had a history of previous fractures below the elbow. Patients with previous metacarpal or finger fractures were liable to have subsequent hand fractures of the ipsilateral side. Healed Colles' fractures appeared to confer a relative protection against refracture. Only one fifth of subsequent Colles' fractures were ipsilateral to the previous injury.

Seventy-two patients were examined 9 weeks after sustaining a Colles' fracture of the wrist for evidence of algodystrophy. They were examined 18 months later for evidence of Dupuytren's disease to determine the incidence of the association between the two conditions. Forty-one per cent of all patients had evidence of Dupuytren's disease at 18 months following Colles' fracture. Sixty-seven per cent of patients with algodystrophy had evidence of Dupuytren's disease compared with 19% of patients who showed no features of algodystrophy.

There is little data concerning the morbidity, mortality, and epidemiology of vertebral fracture. The aim of this study was to evaluate the … risk factor for a new vertebral fracture [RR=4.7 (1.8-11.9)], hip fracture [RR=6.7 (2.0-22.7)] and Colles' fracture [RR=3.0 (1.1-7.8)]. Prevalent and incident vertebral fractures were associated with a higher risk of having a hip fracture [RR=10.0 (2.0-50.2)] and Colles' fracture [RR=5.5 (1.3-23.4)]. In addition, in women, the vertebral fracture was associated wi …

Fourteen patients with pain and limited forearm rotation were treated with a modified Darrach procedure involving a distally based slip of the extensor carpi ulnaris tendon. Seven patients had Colles' fractures, four had rheumatoid arthritis, and three had primary osteoarthritis of the distal radioulnar joint. Patients were assigned to one of two groups: rheumatoid arthritis or Colles' fracture/osteoarthritis. The average age was 50 years. Follow-up averaged 54 months (range, 12 to 96 months). The average amount of ulnar resection was 10 mm (range, 6 to 20 mm). In the Colles' fracture/osteoart…

Kapandji pinning has been proposed as the treatment of choice for unstable Colles' fractures. The aim of this paper is to evaluate our experience treating unstable Colles' type fractures using this technique. Over a nine month period, 36 patients with Colles' type fractures were treated operatively at St. Vincent's Hospital. 22 of these fractures were deemed unstable and were treated using percutaneous intrafocal Kapandji pinning. 20 of these patients were recalled for review at a mean of 11.3 month post injury. At this stage the wrist was examined clinically and radiologically. Initial satisf…

Colles' fracture is rarely associated with a carpal or metacarpal fracture. We report a case of ipsilateral, simultaneous Bennett's and distal radius fracture after moderate trauma. Closed reduction and external fixation led to uneventful healing and no residual symptoms.

INTRODUCTION: In case of distal radius fractures (DRF) the distal fragment generally displaces either dorsally or volarly. Scientific literature however, seldom differentiates between volarly and dorsally displaced DRFs when reporting results. It is no clear, if the direction of displacement has an influence on the clinical and radiological outcome. This study was intended to evaluate the influence of displacement direction in adult patients with surgically treated Colles or Smith type fractures.
PATIENTS AND METHODS: After a mean follow up (FU) time of above 5 years, 50 patients who underwen

In 249 patients with fractures characteristic of bone fragility, i.e. femoral neck fracture, vertebral crush fracture, fracture of the upper end of the humerus, Colles' fracture, fracture of the lateral condyle of the tibia and various ankle fractures, and forearm bone mineral content was measured by gamma absorptiometry. Vertebral crush fracture, fracture of the upper end of the femur and fracture of the distal end of the forearm are associated with a generally decreased bone mineral mass. In young women with femoral neck fracture the difference between fracture cases and control cases was gr…

Acute fractures of the scaphoid were randomly allocated for conservative treatment in a Colles'-type plaster cast with the wrist immobilised in either 20 degrees flexion or 20 degrees extension. The position of the wrist did not influence the rate of union of the fracture (89%) but when reviewed after six months the wrists which had been immobilised in flexion had a greater restriction of extension. We recommend that acute fractures of the scaphoid should be treated in a Colles'-type cast with the wrist in slight extension.

Lymphedema of the hand following a fracture of the distal radius is a disabling, but rare complication. Although the pathogenesis of this mechanism is poorly understood, extensive review of the literature suggests that an underlying cause of this lymphedema is psychogenic in nature. After numerous therapeutic modalities including intense physiotherapy, sympathetic nerve blocks and hospitalization, our patient still continued to persist with this debilitating disease. This article presents a case of Colles fracture complicated by nonpitting edema in a 62-year-old woman in whom psychogenic cause

A significant risk factor for osteoporotic fracture is a previous atraumatic fracture. The objective of this study was to investigate whether patients with Colles fracture from minimal trauma were subsequently identified, assessed and treated for their elevated risk of fracture. Medical records at Sir Charles Gairdner Hospital in Perth, Western Australia, from August 1999 to July 2000 were audited and 111 patients who had sustained a Colles fracture from minimal trauma were identified. Questionnaires were subsequently posted to participants to determine whether any assessment or treatment was

AIM: The purpose of the study was to verify test criteria of the upper-limb DASH questionnaire in patients with Colles' fractures.
METHODS: 107 of 139 patients with Colles' fractures treated operatively were examined and asked to complete the questionnaire. To establish reliability, the item answers of all questionnaires were analysed by using Cronbach's Alpha correlation coefficient and corrected item total correlation. Construct validity was evaluated by comparing the DASH points with clinical measures according to Gartland/Werley and Castaing (Spearman correlation coefficient). Discriminan…

INTRODUCTION: The classical colles fracture (extra-articular, dorsally angulated distal radius fracture) in patients with osteoporotic bone is becoming increasingly more frequent. There still appears to be no clear consensus on the most appropriate surgical management of these injuries. The purpose of this study is to appraise the use of percutaneous extra-focal pinning, in the management of the classical colles fracture.
METHODS: We retrospectively analysed 72 consecutive cases of colles fractures treated with inter-fragmentary k-wire fixation, in female patients over 60 years of age, in two

The pathogenesis and treatment of posttraumatic compression of the median nerve in the carpal canal are analysed in 36 patients. Nine patients had mal-union of a Colles' fracture. In these patients the symptoms of median nerve compression were completely relieved by correction osteotomy of the radial metaphysis, which also improved radiocarpal joint function. At follow-up examination after four years the results were satisfactory and the median nerve appeared normal in clinical and electro-physiological tests. In the other twenty-seven patients there had been a Colles' fracture in seventeen, h…

Fractures of the distal forearm are widely regarded as the result of "fragility". We have examined the extent to which patients with Colles' fractures have osteopenia. We measured the bone mineral density (BMD) in the contralateral radius of 235 women presenting with Colles' fractures over a period of two years. While women of all ages had low values for ultra-distal BMD, the values, in age-matched terms, were particularly low among premenopausal women aged less than 45 years. This result was not due to the presence of women with an early menopause. This large survey confirms and extends the f…

We investigated the discriminative ability of total body bone-mineral expressed as the total body bone-density (TBBD) measured by dual photon absorptiometry (DPA) in 79 healthy premenopausal women, 27 healthy postmenopausal women, and 120 female osteoporotic fracture patients presenting with either Colles' fracture, vertebral fracture or femoral neck-fracture. TBBD was compared to the bone-mineral density of the lumbar spine (BMDspine) also measured by DPA, and to the bone-mineral content of the forearms (BMCforearm) measured by single photon absorptiometry (SPA). TBBD, BMDspine and BMCforearm…

Distal radius fractures, specifically Colles-type injuries, are highly prevalent in older females and typically result from falls on an outstretched hand. Clinically, these fractures appear to originate on the volar side of the distal radius due to a substantial dorsiflexion bending moment, yet most biomechanical and finite element (FE) studies rely on simplified axial loading assumptions that fail to reproduce the tensile strain gradients responsible for these fractures. This study quantified the full six-degree-of-freedom (6DOF) force and moment components acting on the distal radius during
