Population screening for colorectal cancer: advantages and drawbacks.

Screening for colorectal cancer (CRC) has been shown to reduce mortality from the disease as well as incidence. Several methods for screening are available, but none are ideal. The present short review discusses the advantages and drawbacks using fecal occult blood tests (FOBTs), fecal DNA tests, flexible sigmoidoscopy (FS), colonoscopy, double contrast barium enema, and computed tomographic colonoscopy. The use of FOBTs with following colonoscopy in persons with a positive test has been shown to reduce mortality as well as the incidence of CRC in several randomized trials. It is likely that o

Q&A on diagnosis, screening and follow-up of colorectal neoplasia.

The impressive and brisk evolution of medical science prevents many physicians from a thorough update on all the research fields. Colorectal cancer diagnosis, screening and follow-up is well known to require a multi-disciplinary approach, as it is faced by several specialties such as primary care physicians, gastroenterologists, non-gastroenterologist internists, radiologists and surgeons. To address this issue in a mutual perspective, we focused on the main points of the epidemiology, diagnosis, screening and follow-up of colorectal neoplasia by using a simple "Question & Answers" structure.

A simultaneous onset of organizing pneumonia and rheumatoid arthritis, along with a review of the literature.

Organizing pneumonia (OP) is a specific type of interstitial pneumonia that has been noted as one of the pulmonary manifestations during the course of rheumatoid arthritis (RA). In this study, we report a case with a simultaneous development of OP and RA. The patient presented with concurrent flu-like symptoms and arthralgia of multiple joints, and antibiotic therapy was not effective. The rheumatoid factor (RF) and anti-cyclic citrullinated antibodies were both high. Multiple air-space opacities on chest radiographs and bilateral peripheral consolidations on high-resolution computed tomograph

Strategy for colorectal cancer screening.

Colorectal cancer is a leading cause of cancer death in Israel. Our current understanding of the colorectal adenoma-carcinoma sequence has led to the use of screening for timely detection of polyps and cancer. Digital examination of the rectum is a test that can be performed by all doctors. Fecal occult blood testing, flexible sigmoidoscopy and colonoscopy are the standard screening techniques for patients. Computerized tomography colonography is now entering this field. This review discusses the merits and uncertainties of these strategies as related to the risk of colorectal cancer in select

Extracolonic and incidental findings on CT colonography (virtual colonoscopy).

OBJECTIVE: The objective of our study was to prospectively determine the frequency and clinical importance of extracolonic findings on CT colonography in symptomatic patients. SUBJECTS AND METHODS. One hundred eleven symptomatic patients referred for colonoscopy underwent CT colonography before colonoscopy. Helical CT from the diaphragm to the symphysis was performed with the patient in the supine and prone positions after rectal air insufflation. Image interpretation was done on a digital workstation. Extracolonic findings were classified as minor, moderate, or major according to potential cl

[CT-colonography: current status and future directions in consideration of the technical evolution].

Computertomography and with it CT-colonography evolves technically and also possibilities of data processing proceed rapidly. The current status of CTC can be summarized as follows:The method represents a true alternative to barium enema if technical conditions are given.CTC can be performed with excellent results immediately following an incomplete colonoscopy. In pretherapeutic diagnosis of colorectal carcinoma, CTC can be combined with a staging CT of the abdomen. The recent technical advances with new 16 row technology in image quality and data acquisition - mainly concerning the spatial a

CT- and MR colonography.

BACKGROUND: Colorectal cancer is the second most frequent cancer and adenomas are widely accepted as precursors to colorectal cancer. Diagnosis and removal of adenomas are recommended to reduce cancer incidence and mortality. The current diagnostic methods include sigmoidoscopy and colonoscopy. Lately, CT- and MR colonography have emerged as non-invasive methods for colon imaging. METHODS: At present, CTC and MRC require bowel preparation. However, preliminary studies have been carried out without colon preparation. After the colon has been filled with air or contrast, the patient is scanned

Evaluation of submucosal lesions of the large intestine: part 1. Neoplasms.

At luminal evaluation of the large intestine, any masslike protrusion that is covered by normal mucosa, whether the underlying process is intramural or extramural in origin, may be reported as a submucosal lesion. The full characterization of submucosal lesions may be difficult with optical colonoscopy alone, and endoscopic biopsy is often nondiagnostic. Cross-sectional radiologic imaging studies allow evaluation of the entire thickness of the bowel wall and surrounding tissues and often provide additional information with regard to lesion origin, internal composition, and extent of disease. Left-sided polyps detected at screening CT colonography: do we need complete optical colonoscopy for further evaluation?

PURPOSE: To estimate the relative yield of therapeutic flexible sigmoidoscopy compared with complete optical colonoscopy for isolated left-sided polyps (≥6 mm in diameter) identified at screening computed tomographic (CT) colonography. MATERIALS AND METHODS: This retrospective institutional review board-approved and HIPAA-compliant study included a review of CT colonographic screening results in 6570 consecutive asymptomatic adults (3551 women, 3019 men; mean age, 56.8 years ± 7.3 [standard deviation]). Of 887 (13.5%) patients with cases positive for nondiminutive polyps (≥6 mm), a subset of

G-SET-DCL: a guided sequential episodic training with dual contrastive learning approach for colon segmentation.

PURPOSE: This article introduces a novel deep learning approach to substantially improve the accuracy of colon segmentation even with limited data annotation, which enhances the overall effectiveness of the CT colonography pipeline in clinical settings. METHODS: The proposed approach integrates 3D contextual information via guided sequential episodic training in which a query CT slice is segmented by exploiting its previous labeled CT slice (i.e., support). Segmentation starts by detecting the rectum using a Markov Random Field-based algorithm. Then, supervised sequential episodic training is

Exploration of analysis methods for diagnostic imaging tests: problems with ROC AUC and confidence scores in CT colonography.

BACKGROUND: Different methods of evaluating diagnostic performance when comparing diagnostic tests may lead to different results. We compared two such approaches, sensitivity and specificity with area under the Receiver Operating Characteristic Curve (ROC AUC) for the evaluation of CT colonography for the detection of polyps, either with or without computer assisted detection. METHODS: In a multireader multicase study of 10 readers and 107 cases we compared sensitivity and specificity, using radiological reporting of the presence or absence of polyps, to ROC AUC calculated from confidence sco

Value-of-information analysis to guide future research in colorectal cancer screening.

PURPOSE: To identify the most useful areas for research in colorectal cancer (CRC) screening by using a value-of-information analysis. MATERIALS AND METHODS: Cost-effectiveness of screening strategies, including colonoscopy, computed tomographic (CT) colonography, flexible sigmoidoscopy, and barium enema examination, were compared by using a Markov model. Monetary net benefit (NB), a measure of cost-effectiveness, was calculated by multiplying effect (life-years gained) by willingness to pay (100,000 dollars per life-year gained) and subtracting cost. A value-of-information analysis was used

Characterization of Computed Tomography Colonography Findings of Ulcerative Colitis-Associated Neoplasia.

BACKGROUND: Computed tomography colonography (CTC) is increasingly utilized for the evaluation of colorectal neoplasms. However, in patients with ulcerative colitis (UC), current European Crohn's and Colitis Organisation guidelines recommend CTC only for limited indications, such as the presence of strictures. METHODS: This single-center, retrospective observational study included consecutive patients with UC who underwent preoperative CTC and were scheduled for pancolectomy for UCAN between January 2014 and June 2024. Lesion detectability on CTC was assessed in comparison with endoscopic fin

Effect of Reducing Abdominal Compression during Prone CT Colonography on Ascending Colonic Rotation during Supine-to-Prone Positional Change.

OBJECTIVE: To determine the effect of reduced abdominal compression in prone position on ascending colonic movement during supine-to-prone positional change during CT colonography (CTC). MATERIALS AND METHODS: Eighteen consecutive patients who had undergone prone CTC scanning with cushion blocks placed under the chest and hip/thigh to reduce abdominal compression and had confirmed sessile polyps ≥ 6 mm in the well-distended, straight, mid-ascending colon, were included. Radial location along the ascending colonic luminal circumference (°) was measured for 24 polyps and 54 colonic teniae on su

Colorectal cancer screening: the role of CT colonography.

Computed tomography colonography (CTC) in colorectal cancer (CRC) screening has two roles: one present and the other potential. The present role is, without any further discussion, the integration into established screening programs as a replacement for barium enema in the case of incomplete colonoscopy. The potential role is the use of CTC as a first-line screening method together with Fecal Occult Blood Test, sigmoidoscopy and colonoscopy. However, despite the fact that CTC has been officially endorsed for CRC screening of average-risk individuals by different scientific societies including
