Focal or generalized vascular brain damage and vulnerability to depression after stroke: a 1-year prospective follow-up study.

BACKGROUND: Both the lesion location hypothesis and the vascular depression hypothesis have been proposed to explain the high incidence of depression in stroke patients. However, research studying both hypotheses in a single cohort is, at present, scarce. OBJECTIVE: To test the independent effects of lesion location (left hemisphere, anterior region) and of co-occurring generalized vascular damage on the development of depression in the first year after ischemic stroke, while other risk factors for depression are controlled for. METHODS: One hundred and ninety consecutive patients with a fir

Personalized treatment design in the context of functional confounding.

One of the primary goals of individualized treatment rule (ITR) methodology is to identify optimal decision rules using clinical predictors. While functional data has become increasingly available in biomedical research, there has been limited work on incorporating functional data into ITR estimation, particularly in observational studies. In this paper, we propose a novel approach that integrates outcome-weighted learning (OWL) with reproducing kernel Hilbert space to determine optimal treatment regimes involving functional data. Furthermore, to address the issue of data piling, we employ the

Using longitudinal targeted maximum likelihood estimation in complex settings with dynamic interventions.

Longitudinal targeted maximum likelihood estimation (LTMLE) has very rarely been used to estimate dynamic treatment effects in the context of time-dependent confounding affected by prior treatment when faced with long follow-up times, multiple time-varying confounders, and complex associational relationships simultaneously. Reasons for this include the potential computational burden, technical challenges, restricted modeling options for long follow-up times, and limited practical guidance in the literature. However, LTMLE has desirable asymptotic properties, ie, it is doubly robust, and can yi

Antipsychotics and mortality: adjusting for mortality risk scores to address confounding by terminal illness.

OBJECTIVES: To determine whether adjustment for prognostic indices specifically developed for nursing home (NH) populations affect the magnitude of previously observed associations between mortality and conventional and atypical antipsychotics. DESIGN: Cohort study. SETTING: A merged data set of Medicaid, Medicare, Minimum Data Set (MDS), Online Survey Certification and Reporting system, and National Death Index for 2001 to 2005. PARTICIPANTS: Dual-eligible individuals aged 65 and older who initiated antipsychotic treatment in a NH (N=75,445). MEASUREMENTS: Three mortality risk scores (Mor

Ecologic covariates of hookworm infection and reinfection in rural Kwazulu-natal/south Africa: a geographic information system-based study.

Information on hookworm infection and re-infection in a cohort of primary school children and interview data on their socioeconomic background and behavior were combined with environmental data using a geographic information system (GIS). Multivariate models served to explore the covariation of environmental and infection patterns adjusted for possible confounders. Our aim was to identify environmental factors that might serve to predict infection and thus guide control efforts when epidemiologic information is insufficient. Furthermore, we wanted to establish whether soil type has a genuine i

Survey of use of ST-segment monitoring in patients with acute coronary syndromes.

BACKGROUND: Although effective for assessing ongoing myocardial ischemia, ST-segment monitoring may be underused. OBJECTIVES: To determine the proportion of cardiac units in the United States that use ST-segment monitoring, hospital and unit characteristics associated with its use, how units use such monitoring with respect to research recommendations, if units that use ST-segment monitoring find it clinically useful and easy to use, and why some units are not using this type of monitoring. METHODS: A survey on ST-segment monitoring was mailed to a random sample of 500 cardiac nurse managers

Daily deaths are associated with combustion particles rather than SO(2) in Philadelphia.

OBJECTIVES: To assess whether the association between SO(2) and daily deaths in Philadelphia during the years 1974-88 is due to its correlation with airborne particles, and vice versa. METHODS: There is a significant variation in the relation between total suspended particulate (TSP) and SO(2) in Philadelphia by year and season. Firstly, 30 separate regressions were fitted for each pollutant in the warm and cold season of each year. These regressions controlled for weather, long term temporal patterns, and day of the week. Then a meta-regression was performed to find whether the effect of SO(

Dietary fat and obesity: an epidemiologic perspective.

The observation that dietary fat has an effect on weight gain and the development of obesity that is larger than would be expected on the basis of fat's energy value is mainly experimental. Several methodologic problems limit the interpretation of epidemiologic studies of the association between dietary fat intake and obesity. Among the issues relevant in this context are underreporting of energy and fat intakes, dieting behavior, inadequate control for variables such as energy expenditure, and limited between-subject variation in fat intake in developed countries. Different types of epidemiol

Balloon angioplasty versus new device intervention: clinical outcomes. A comparison of the NHLBI PTCA and NACI registries.

OBJECTIVES: We sought to compare outcomes of patients treated in the National Heart, Lung, and Blood Institute (NHLBI) Percutaneous Transluminal Coronary Angioplasty (PTCA) and New Approaches to Coronary Intervention (NACI) registries. BACKGROUND: Coronary angioplasty has numerous shortcomings. New devices for performing coronary interventions have been introduced in an effort to improve clinical outcomes. METHODS: Under the sponsorship of the NHLBI, a registry of consecutive patients treated with PTCA during 1985 to 1986 was established. In 1990, the NHLBI funded a second registry, the NACI

Delayed entry into prenatal care: effect of physical violence.

OBJECTIVE: To assess whether women who experienced physical violence by their partner during the 12 months before delivery were more likely to delay entry into prenatal care than were women who had not experienced physical violence. METHODS: We analyzed data from the Pregnancy Risk Assessment Monitoring System. The sample included 27,836 women who delivered live infants during 1993-1994 in nine states and were surveyed 2-6 months after delivery. We calculated risk ratios and 95% confidence intervals (CIs) to measure the association between physical violence within the 12 months before deliver

Comparison of effect estimates from a meta-analysis of summary data from published studies and from a meta-analysis using individual patient data for ovarian cancer studies.

To determine the relative merits of two quantitative methods used to estimate the summary effects of observational studies, the authors compared two methods of meta-analysis. Each quantified the relation between oral contraceptive use and the risk for ovarian cancer. One analysis consisted of a meta-analysis using summary data from 11 published studies from the literature (MAL) in which the study was the unit of analysis, and the second consisted of a meta-analysis using individual patient data (MAP) in which the patient was the unit of analysis. The authors found excellent quantitative agreem

Monotonic Association of Increasing Past-Year Substance Use Disorder across a Four-Level Trauma Post-Traumatic Stress Disorder (PTSD) Hierarchy in a Nationwide Sample.

OBJECTIVE: There is limited information on the differences in the association of substance use disorders (SUD) with four clinically relevant hierarchical groups based on trauma exposure and its consequences (1-no trauma; 2-trauma but no PTSD; 3-remitted PTSD; and 4-current PTSD). METHODS: Among adults enrolled in a large nationally representative survey, the National Epidemiologic Survey on Alcohol and Related Conditions Wave III (NESARC III), we compared differences in SUD prevalence between each of the hierarchical trauma group and the prior group adjusting for potentially confounding facto

Clearing the confounding confusion: Benzodiazepines and the risk of dementia?

OBJECTIVE: To examine the relationship between duration of benzodiazepine exposure and the risk of dementia. DESIGN: A retrospective cohort study using administrative health data followed 3 age-based strata (ages 55, 65 and 75) for up to a maximum of 22 years to examine risk of dementia due to benzodiazepine use. Each stratum was analyzed as a whole, then restricted to persons with depression or anxiety, and finally using high dimension propensity scores (HDPS) matched cohort. SETTING: We used administrative data on subjects receiving standard medical care in Manitoba, Canada. MEASUREMENTS:

Serum Microelements in Early Pregnancy and their Risk of Large-for-Gestational Age Birth Weight.

Excessive birth weight has serious perinatal consequences, and it "programs" long-term health. Mother's nutritional status can be an important element in fetal "programming"; microelements such as selenium (Se), zinc (Zn), copper (Cu), and iron (Fe) are involved in many metabolic processes. However, there are no studies assessing the relationship of the microelements in the peri-conceptual period with the risk of excessive birth weight. We performed a nested case control study of serum microelements' levels in the 10-14th week of pregnancy and assessed the risk of large-for-gestational age (LG

Alcohol drinking and gastric cancer risk: an evaluation based on a systematic review of epidemiologic evidence among the Japanese population.

BACKGROUND: We reviewed epidemiologic studies on the association between alcohol drinking and gastric cancer among the Japanese population. This report is one of a series of articles by our research group, which is evaluating the existing evidence concerning the association between health-related lifestyles and cancer. METHODS: Original data were collected by searches of MEDLINE using PubMed, or searches of the Ichushi database, complemented with manual searches. Evaluation of associations was based on the strength of evidence and the magnitude of association, together with biological plausib
