GHB can be produced either as a pre- or postmortem artifact. The authors describe two cases in which GHB was detected and discuss the problem of determining the role of GHB in each case. In both cases, NaF-preserved blood and urine were analyzed using gas chromatography. The first decedent, a known methamphetamine abuser, had GHB concentrations similar to those observed with subanesthetic doses (femoral blood, 159 microg/ml; urine, 1100 microg/ml). Myocardial fibrosis, in the pattern associated with stimulant abuse, was also evident. The second decedent had a normal heart but higher concentrat

HISTORY AND FINDINGS: By error a pharmacist had dispensed to a 41-year-old woman Tegretal (carbamazepine; 400 mg retard tablets) instead of Trental (pentoxifylline; 400 mg retard dragées). Similarly, a 76-year-old woman had been prescribed carbamazepine-neuroxpharm (600 mg retard tablets) instead of piracetam-neuraxpharm (800 mg tablets) by her family doctor. Thirdly, a 79-year-old man was, as an in-patient, by error given by a nurse Tegretal, 400 mg retard tablets three times daily, presumably instead of 400 mg Trental retard. All 3 patients developed mild cerebellar symptoms. The two elder p

Confusion in the elderly patient is usually a symptom of delirium or dementia, but it may also occur in major depression and psychoses. Until another cause is identified, the confused patient should be assumed to have delirium, which is often reversible with treatment of the underlying disorder. Causes of delirium include metabolic disorders, infections and medications. Thyroid dysfunction, vitamin deficiencies and normal-pressure hydrocephalus are some potentially reversible causes of dementia. Major irreversible causes include Alzheimer's disease, central nervous system damage and human immu…

The purpose of this study was to describe cases of oral cancer in which the initial presenting symptoms were considered to be due to third molars and in which the underlying carcinoma therefore escaped early diagnosis. Between 1986 and 1990 a total of five cases with squamous cell carcinoma associated with a third molar (impacted, partially or totally erupted, or recently extracted) were treated at our unit. Because of acute or chronic symptoms related to disturbed eruption of third molars, the detection of a simultaneous malignant change and referral were delayed. In conclusion, all third mol

Equine parasite control has historically been characterized by confusing and conflicting information, posing significant challenges for veterinarians and horse owners to make evidence-based decisions. Since 2012, equine parasite control guidelines have been developed and published for different parts of the world to address this situation and provide trusted sources of current guidance. At the 2024 International Equine Infectious Disease Conference in Deauville, Normandy, France, lead authors of equine parasite control guideline documents published in the USA, UK, Sweden, Denmark, the Netherla

At present, women with ductal carcinoma in situ are counseled that they have a pre-malignant condition which carries the possibility of progression to a fully malignant breast cancer. However, in most cases, the treatment of DCIS resembles that of a small invasive breast cancer and this is a source of confusion to many. In order to properly evaluate the benefit of radiotherapy, mastectomy and contralateral mastectomy, it is necessary to consider the risks of ipsilateral invasive cancer and of contralateral breast cancer in women with DCIS and with small invasive breast cancer. Several registry…
