Factors associated with uptake of the Influenza A(H1N1)pdm09 monovalent pandemic vaccine in K-12 Public Schools, Maine 2009-2010.

CONTEXT AND OBJECTIVE: Maine implemented a statewide pre-K through 12-school vaccination program during the 2009-2010 H1N1 influenza pandemic. The main objective of this study was to determine which school, nurse, consent form, and clinic factors were associated with school-level vaccination rates for the first dose of the 2009 H1N1 pandemic vaccine. METHODS: In April 2010, school nurses or contacts were e-mailed electronic surveys. Generalized linear mixed regression was used to predict adjusted vaccination rates using random effects to account for correlations within school districts. Eleme

[Cross-sectional descriptive analysis of the structure and content of specialty medication access forms used in patient support programs].

BACKGROUND: Patient support programs (PSP) aim to facilitate access to specialty medications but rely heavily on often complex forms that create a significant administrative burden and reduce available clinical time. Few data describe their structure and content. OBJECTIVE: To describe the structure and content of specialty medication access forms (SMAFs) available in Québec. METHODS: A cross-sectional descriptive study including all forms available on the platform of the Association québécoise des coordonnateurs d'accès aux médicaments en oncologie (AQCAMO, n=211) and locally available form

Use of a mandatory declination form in a program for influenza vaccination of healthcare workers.

OBJECTIVE: To evaluate the utility and impact of using a declination form in the context of an influenza immunization program for healthcare workers. METHODS: A combined form for documentation of vaccination consent, medical contraindication(s) for vaccination, or vaccination declination was used during the 2006-2007 influenza season in a healthcare system employing approximately 9,200 nonphysician employees in 3 hospitals; a skilled nursing care facility; a large, multisite, faculty-practice plan; and an administrative building. Responses were entered into a database that contained files fro

In California, voluntary mass prenatal screening.

A statewide program in California to detect neural tube and other birth defects may revive enthusiasm for mass prenatal screening. Participation in the program is voluntary, but all expectant mothers are asked to sign a statement of "informed consent/refusal." So far California's program seems to be working well, but questions for the future include the level of participation, the possibility that normal fetuses will be aborted, the kinds of information given to women, and the elusive nature of free choice.

Audit of compliance with NICE guidelines on the use of tension-free vaginal tape slings for stress incontinence.

The aim of this audit was to assess adherence to the National Institute of Clinical Excellence (NICE) guidelines in inserting tension-free vaginal tape (TVT) slings for urodynamic stress incontinence, in gynaecology and urology settings in a district general hospital. It included 95 patients (79 inserted by gynaecologists and 16 by urologists) who had TVT sling insertion from 2002 until 2005, at the Royal Bournemouth Hospital. Conservative measures were tried by 43 patients (45.3%) and documentation of risk factors varied between 70% for failure and intermittent catheterisation and 1% for lapa

High Patient Willingness to Grant Broad Consent for Real-World Data Use in Rheumatology-Implications for Real-World Data Platform Governance: Cross-Sectional Study.

BACKGROUND: Medical real-world data (RWD) are often siloed across organizations, making them inaccessible for research. Unlocking these data could advance clinical research and patient care. The pan-European Data Nexus platform (DNP) links RWD, facilitating its use, for example by artificial intelligence (AI) tools, to support the generation of real-world evidence. In Europe, particularly Germany, the secondary use of health data is governed by stringent regulatory requirements, including informed consent. OBJECTIVE: This study evaluated the informed broad consent form for the RWD (Data Nexus

Urinary catheter use in end of life care.

Patients nearing the end of life can become agitated by a full bladder and may need urgent urinary catheterisation. However, urethral catheterisation needs authorisation by a doctor. Nurses at a London trust developed a urinary catheterisation consent form permitting nurses to perform this procedure when it becomes necessary without having to gain medical authorisation. This article discusses how the consent form can help improve bladder management, ensuring terminally ill patients do not have to attend the emergency department to be catheterised.

Ethical issues in treating gay and lesbian patients.

Since the 1973 decision to remove homosexuality from the list of mental disorders, most mental health practitioners have shifted their clinical focus from "the cure" of homosexuality to treating the concerns of gay and lesbian patients. Some clinicians, however, reject the mental health mainstream's view and continue to conceptualize homosexuality as a mental disorder. Their clinical theories have been incorporated into wider societal debates regarding the status of gay and lesbian people. The sexual conversion or reparative therapies they practice, however, may include routine ethical violati

Non-consensual sterilization of women living with HIV.

Women living with HIV are stigmatized and discriminated against. They often wish to have children, but they are subjected to the practice of forced (involuntary) sterilization in at least 27 countries under the guise of protecting maternal health and preventing the birth of infected infants. Some women are not asked to give consent, or a third party consents on their behalf. Others are given insufficient information or fed misinformation. The circumstances under which such women have been asked to sign consent forms for sterilization include fear, coercion, intimidation, and undue influence. C

Our Life Depends on This Drug: Competence, Inequity, and Voluntary Consent in Clinical Trials on Supervised Injectable Opioid Assisted Treatment.

Supervised injectable opioid assisted treament (siOAT) prescribes injectable opioids to individuals for whom other forms of addiction treatment have been ineffective. In this article, we examine arguments that opioid-dependent people should be assumed incompetent to voluntarily consent to clinical research on siOAT unless proven otherwise. We agree that concerns about competence and voluntary consent deserve careful attention in this context. But we oppose framing the issue solely as a matter of the competence of opioid-dependent people and emphasize that it should be considered in the context

Enhancing Documentation Standards of Complications in Informed Consent for Neck of Femur Fracture Surgery.

Background Neck of femur (NOF) fractures are a major cause of morbidity in the United Kingdom. Operative management is the standard of care, and informed consent is essential to ensure patients understand potential benefits and complications arising during the surgical management of NOF fractures. This study aimed to assess and enhance the quality of Consent Form 1 documentation of complications of NOF fracture surgery as per the British Orthopaedic Association (BOA) and to assess improvements following focused interventions. Methodology A closed-loop audit was conducted in the Trauma and Orth

Informed consent for surgery: risk discussion and documentation.

Informed consent for surgery has become a critical component of surgical practice. There are specific legal requirements for what has to be disclosed to patients and for the accompanying documentation. The hospital consent for surgery form with the patient's signature is a small part of the process. A quality assurance review of the documentation of informed consent by our surgical group indicated major deficiencies and prompted this article (and its Appendix containing further information). Our intent was to inform surgeons about modern standards and to discuss particular challenges. Informed

Parental Knowledge and Decisional Satisfaction Across Genomic Sequencing Consent Processes in Pediatric Cancer.

PURPOSE: To determine whether transitioning from a two-visit consent process led by an expert consenter to a decentralized model of consenting using an educational video and a clinical care provider translated into sustained decisional satisfaction and comparable levels of understanding about genomic sequencing for cancer predisposition. MATERIALS AND METHODS: A total of 150 parents of children with cancer agreed to participate in this study approximately 4 weeks (±2 weeks) after a consent conversation for genomic sequencing using a decentralized model to assess their genetic knowledge and de

Assessing chronic stroke survivors with aphasia sheds light on prevalence of spatial neglect.

BACKGROUND: Stroke is a chronic disease. Standardized assessment is essential in order to determine areas for treatment. Individuals with aphasia are often excluded from research, because it is believed that their language impairments may impact their ability to provide informed consent. Thus, right spatial neglect could be under-diagnosed. OBJECTIVE: This study was developed to (1) determine the frequency of spatial neglect in chronic left-brain stroke survivors with aphasia, (2) determine the clinical utility of an aphasia-friendly consent form, and (3) determine any differences between neg

Informed consent procedure in a double blind randomized anthelminthic trial on Pemba Island, Tanzania: do pamphlet and information session increase caregivers knowledge?

BACKGROUND: In clinical research, obtaining informed consent from participants is an ethical and legal requirement. Conveying the information concerning the study can be done using multiple methods yet this step commonly relies exclusively on the informed consent form alone. While this is legal, it does not ensure the participant's true comprehension. New effective methods of conveying consent information should be tested. In this study we compared the effect of different methods on the knowledge of caregivers of participants of a clinical trial on Pemba Island, Tanzania. METHODS: A total of

Children's views on their involvement in clinical research.

OBJECTIVE: To examine the level of children's understanding of informed consent in clinical trials and factors that may influence these processes. DESIGN: Twenty nine children who were included in clinical trials for treatment of cancer or HIV, were offered the possibility to complete a semidirective interview, with parental permission. METHODS: Children's understanding was measured by a score of 0-9 including items required to obtain a valid consent according to French and European legislations. RESULTS: Children were 8.5-18 years old (13.6 +/- 2.8 years). The higher percentage of understa

Autonomy with responsibility ¿Is informed consent just a signature on a paper? Evaluation in patients who underwent spine's surgery.

PURPOSE: To evaluate the information that patients undergoing spine surgery truly receive and assimilate when they sign their informed consent documents. METHODS: This was a retrospective study on patients who underwent spine arthrodesis or spine discectomy. Patients were given a full explanation of the surgical technique to be employed and its potential risks before they were included on the surgical waiting list. Before surgery, they were asked to sign an informed consent form. The studied variables included whether patients read the informed consent form, whether they recalled the surgical

Informed consent for medical photographs. Dysmorphology Subcommittee of the Clinical Practice Committee, American College of Medical Genetics.

Patient images are used for many purposes in medical practice. They are placed in the medical record as an adjunct to clinical care, displayed to colleagues, students and other audiences in educational settings, and published in medical journals or other media as part of medical research. In each case it is not only prudent, but necessary for the patients’ protection and interest that appropriate consent be obtained. While medical journals invariably require written consent for photographs that may identify the patient, the format of the photograph consent form is usually not specified, nor is

Stakeholder perspectives regarding alternate approaches to informed consent for comparative effectiveness research.

INTRODUCTION: Traditional informed consent approaches, involving separate discussions and lengthy consent forms, may be an imperfect fit for comparative effectiveness research (CER) that is integrated into usual care and compares non-investigational treatments. However, systematic efforts to collect broad stakeholder perspectives about alternative streamlined approaches to disclosure and consent in this context have been limited. METHODS: We used a deliberative engagement method to solicit the views of a multi-stakeholder group regarding 3 alternative models of disclosure, consent, and author
