Platelet-associated Ig (PAIg) is raised in patients with idiopathic thrombocytopenic purpura. However, it is also raised in conditions not traditionally considered to be immune mediated. The latter finding suggests that raised PAIg may be an epiphenomenon of thrombocytopenia. The assays currently available do not distinguish between PAIg present due to a specific Fab binding or present due to non-specific binding of Ig via Fc or complement receptors. In this paper we show that by prior incubation with normal rabbit serum, an assay for PAIg can be made specific for Fab binding.

Thirty-five cases of Babesia gibsoni infection and 11 cases of Babesia canis infection were diagnosed and treated in dogs at Kadena Air Base in Okinawa, Japan, between April 1979 and February 1980. Diagnosis was made by demonstrating the organisms in blood smears and by serologic examination, using an indirect fluorescent antibody test. A 60-year-old man with pneumoconiosis complained of general fatigue, exertional dyspnea, and anorexia. The patient had severe anemia, and laboratory examination revealed autoimmune hemolytic anemia with positive direct and indirect Coombs tests. After corticosteroid therapy, the anemia resolved markedly, and the antinuclear antibody test became negative. However, the silicotic shadow on chest X-ray film showed no remarkable change. The autoimmune hemolytic anemia in this case was probably due to an immunological disturbance caused by silicosis.

Paroxysmal cold hemoglobinuria (PCH) is a rare disease in adults, and its concurrent presentation with warm-type autoimmune hemolytic anemia (AIHA) has not yet been reported. We encountered a 19-year-old woman with AIHA and a positive Donath-Landsteiner test result identified by a hemolytic attack during blood transfusion. She also showed positive results for the direct Coombs and Donath-Landsteiner antibody tests. A 12 year old boy was found to be deficient in immunoglobulins (Ig) A, G2 and G4, and common variable immunodeficiency was diagnosed. He also had cyclic thrombocytopenia at intervals of approximately 28-30 days. His bone marrow revealed normocellular with slightly decreased megakaryocytes. In vitro colony assays showed markedly imparied megakaryocytopoiesis, erythropoiesis and granulopoiesis. Platelet-associated IgG was elevated at his thrombocytopenic phase. Direct Coombs' test was repeatedly positive. Although not defined at present, we suggest the autoimmune nature of the disease.

We have described the case of a 58-year-old woman in whom acute hemolytic anemia, renal failure, and granulocytopenia developed after intake of mefenamic acid and diclofenac. Results of direct and indirect Coombs' tests were negative, but the lymphocyte transformation test was positive for these two nonsteroidal antiinflammatory drugs (NSAIDs). We suggest that the immune complex mechanism could have induced these adverse reactions. Although these drugs are very popular, simultaneous administration should be avoided because serious adverse effects can potentially occur.

Three children have been presented who have had long-standing, typical systemic juvenile rheumatoid arthritis. They also had a transient illness with thrombocytopenia, antiplatelet antibodies, and the simultaneous appearance of anti-DNA antibodies and hypocomplementemia. Other single abnormalities included the development of a positive ANA, Coombs, and lupus bands test. All responded to high-dose steroid therapy. Their similar clinical and serologic findings are noteworthy and represent one of the numerous complications seen in patients with systemic juvenile rheumatoid arthritis.

We studied 91 offspring of ABO incompatible preganacies and 30 controls resulting from O--O pregnancies to test whether cord bilirubin levels could be used to predict the severity of hyperbilirubinaemia in ABO incompatibility.
