Surgical management of cervical ossification of the posterior longitudinal ligament: natural history and the role of surgical decompression and stabilization.

OBJECT: Ossification of the posterior longitudinal ligament (OPLL) is a complex multifactorial disease process combining both metabolic and biomechanical factors. The role for surgical intervention and choice of anterior or posterior approach is controversial. The object of this study was to review the literature and present a single-institution experience with surgical intervention for OPLL. METHODS: The authors performed a retrospective review of their institutional experience with surgical intervention for cervical OPLL. They also reviewed the English-language literature regarding the epid

Does microsurgical vascular decompression for trigeminal neuralgia work through a neo-compressive mechanism? Anatomical-surgical evidence for a decompressive effect.

The positive effect of Microsurgical Vascular Decompression (MVD) on idiopathic trigeminal neuralgia still remains controversial between a decompressive mechanism and a "neo-compressive" one. This paper is a summary of a comparative study of the results on pain obtained with two technical modifications of the MVD procedure. The first consisted of interposition of a foreign material between the nerve and the transposed artery after dissection of the trigeminal nerve, whilst in the second the offending vessel(s) was dislodged without using any material touching the nerve. The two series of 60 pa

IPD without bony decompression versus conventional surgical decompression for lumbar spinal stenosis: 2-year results of a double-blind randomized controlled trial.

PURPOSE: Interspinous process devices (IPDs) are implanted to treat patients with intermittent neurogenic claudication (INC) based on lumbar spinal stenosis. It is hypothesized that patients with lumbar spinal stenosis treated with IPD have a faster short-term recovery, an equal outcome after 2 years and less back pain compared with bony decompression. METHODS: A randomized design with variable block sizes was used, with allocations stratified according to center. Allocations were stored in prepared opaque, coded and sealed envelopes, and patients and research nurses were blind throughout the

Surgical Site Infection After Posterior Cervical Decompression: The Role of Adiposity-Related Factors.

BACKGROUND: Local subcutaneous fat thickness has been identified as a significant predictor of surgical site infection (SSI) risk in lumbar spine procedures. This study aims to further investigate this association by comparing the impact of body mass index (BMI) and localized fat thickness at the C5 level on SSI risk in patients undergoing posterior cervical decompression without fusion. METHODS: A retrospective analysis of patients treated with posterior cervical decompression without fusion or stabilization for cervical spondylotic myelopathy. A combination of univariate and multivariate an

Optimizing Surgical Cord Decompression in Traumatic Spinal Cord Injury and Degenerative Cervical Myelopathy.

Preoperative assessment of intramedullary lesion length has become part of a planning algorithm for decompression surgery. In degenerative cervical myelopathy, intramedullary signal abnormalities on MRI often reflect ongoing biological activity rather than fixed tissue loss. As compression persists, reduced perfusion and altered cerebrospinal fluid dynamics may create a compartment-like intradural environment in which the spinal cord has limited reserve capacity. The goal of surgery remains restoration of a permissive intradural environment for the spinal cord. Intraoperative adjuncts such as

Recurrent nontraumatic common peroneal neuropathy after surgical decompression: Case report and literature review.

RATIONALE: Common peroneal neuropathy (CPN) is a common peripheral neuropathy usually caused by trauma or prolonged compression. Nontraumatic and recurrent cases, however, are rare, especially in older adults. This report presents a case of recurrent nontraumatic CPN, emphasizing the diagnostic and therapeutic challenges. PATIENT CONCERNS: A 69-year-old male presented with sudden right-sided foot drop, numbness, and paresthesia without recent trauma. DIAGNOSES: Magnetic resonance imaging and electrodiagnostic studies confirmed nontraumatic CPN with compression near the fibular head. INTERVE

Surgical Technique: Core Decompression of Femoral Head Osteonecrosis with Bone Marrow Aspirate Concentrate.

Osteonecrosis of the femoral head is a progressive and potentially debilitating disorder that is responsive to core decompression, especially in early-stage disease. This is typically accomplished through use of an 8 to 10mm trephine or multiple, small-diameter percutaneous drilling. Use of the large diameter trephine is associated with risk of fracture and may not allow healing across large gaps. Here, we present a technique for core decompression using percutaneous drilling that allows bone marrow aspiration concentrate to be introduced. We used the aspirate needle to decompress the femoral

Machine Learning Prediction of Visual Outcome after Surgical Decompression of Sellar Region Tumors.

INTRODUCTION: This study aims to develop a machine learning-based model integrating clinical and ophthalmic features to predict visual outcomes after transsphenoidal resection of sellar region tumors. METHODS: Adult patients with optic chiasm compression by a sellar region tumor were examined to develop a model, and an independent retrospective cohort and a prospective cohort were used to validate our model. Predictors included demographic information, and ophthalmic and laboratory test results. We defined "recovery" as more than 5% for a p-value in mean deviation compared with the general po

Surgical technique modification of circumferential decompression for thoracic spinal stenosis and clinical outcome.

Progressive thoracic myelopathy caused by ossification of posterior longitudinal ligament (OPLL) responds poorly to conservative therapy. The most direct decompression is extirpation of ossified posterior longitudinal ligament (PLL). Surgical outcomes of posterior approaches to remove ossified PLL are not always satisfactory because of the risk of neurological deterioration. In this study, we modified the conventional anterior decompression technique via a posterior approach for thoracic OPLL. From an anterior approach, the posterior cortex of vertebral body was exposed and the ossified PLL wa

Comparison of Surgical Encounter Direct Costs for Three Methods of Cubital Tunnel Decompression.

BACKGROUND: In situ decompression, subcutaneous transposition, and submuscular transposition for cubital tunnel syndrome have historically yielded similar outcomes. The authors' null hypothesis is that no differences exist in surgical encounter total direct costs for in situ decompression, subcutaneous transposition, and submuscular transposition. METHODS: Adult patients treated surgically for cubital tunnel syndrome by four fellowship-trained hand surgeons between August of 2011 and December of 2016 were identified by CPT code (64718) at their tertiary academic institution. Intraoperative venous balloon angioplasty during surgical thoracic outlet decompression in Paget-Schrötter syndrome.

The management of primary subclavian-axillary vein thrombosis is controversial. Indications and time of operative or endovascular intervention after successful thrombolysis remain unresolved. To improve the long-term functional outcomes in patients with primary subclavian-axillary vein thrombosis, early reestablishment of venous patency and prevention of recurrent thrombosis are required. We present a case in which, after catheter-directed thrombolysis, positional venography showed costoclavicular compression of the subclavian vein. At the time of surgical thoracic outlet decompression, transl
