HIV-associated photodermatitis in African populations.

Photosensitive dermatoses are seen in 5% of HIV-infected persons. These include drug- and chemical-induced photoallergic and phototoxic reactions, chronic actinic dermatitis of HIV, photo lichenoid drug eruptions, and porphyria. Data on photodermatitis in HIV are limited to case reports and series. The pathogenesis is not completely understood and includes a th2 phenotype in HIV which results in impaired barrier function and resultant allergen sensitisation as well as immune dysregulation. The objective of this manuscript is to review the literature on the clinical phenotype, pathogenesis, rol

Allergic contact dermatitis caused by topical antiacne drugs.

Acne is a chronic inflammatory disease for which a long-lasting therapy, very often with topical drugs, is necessary. Despite the fact that several topical antiacne drugs (in particular, tretinoin, benzoyl peroxide, clindamycin and erythromycin) are used for many years, often on broad skin surfaces and for long periods of time, their potential for contact sensitization is low. Their potential for phototoxic and photoallergic reactions is also low. Much more frequent is irritant contact dermatitis caused by some of these drugs, in particular, retinoids and benzoyl peroxide, for which the short

Occupational allergic contact dermatitis from lichens in present-day Finland.

Lichens are abundant in forests, living on trees, soil, stones and rocks. They contain usnic acid and other lichen acids that are contact allergens. Lichens and liverworts cause woodcutter's dermatitis, eczema that appears in the forest on the bare skin areas, especially in cold and wet weather. Occupational allergic contact dermatitis from lichens occurs in forestry and horticultural workers and in lichen pickers. Lichens can cause immediate allergy, contact urticaria, rhinitis and asthma and probably also photoallergic contact dermatitis. Lichens are used for the manufacture of oak moss abso

Environmental airborne contact dermatoses.

This chapter is complementary to Chapter 4 published in the same series. Airborne contact dermatitis (ABCD) is considered a prototype in the field of environmental dermatology. It is often underestimated in most textbooks of general dermatology, despite its frequent occurrence in daily life. ABCD may be irritant, allergic, phototoxic, or photoallergic. Airborne contact urticaria is another example. A particular clinical aspect is the "head and neck dermatitis", which occurs in atopic adult patients. Occupational ABCD represents a most difficult issue in terms of diagnostic procedures. It is ob

The presence of benzophenone in sunscreens and cosmetics containing the organic UV filter octocrylene: A laboratory study.

BACKGROUND: The reason why patients photosensitized to the drug ketoprofen (KP) may develop severe photoallergic skin reactions to octocrylene (OCT), an organic ultraviolet filter in sunscreens and cosmetics, remains largely unknown. OCT can be synthesized by using unsubstituted benzophenone (BP), a possible human carcinogen. OBJECTIVES: To verify if, and to what extent, BP residues are present in OCT-containing consumer products. METHODS: The raw material of OCT and 39 skincare products, of which 28 contain OCT, were chemically analysed for the presence of BP by means of liquid chromatograp

Feeling the Burn: Phototoxicity and Photoallergy.

An interaction between light's radiation and certain exogenous and endogenous substances can lead to the development of photoallergic and/or phototoxic dermatoses. Clinically, reactions may range from acute and self-limited to chronic and recurrent. Delays in diagnosis are not uncommon due to complex clinical presentations, broad differentials, and limited number of specialists who perform phototesting. Therefore, a critical understanding of these dermatoses is essential for accurate diagnosis and appropriate management. The epidemiology, light sources, mechanisms, clinical presentations, eval

Photosensitivity to exogenous agents.

OBJECTIVE: To better understand cutaneous photosensitivity reactions, a review of its etiologic factors, clinical characteristics, pathogenesis, and treatment modalities was undertaken. METHODS: Articles discussing the above aspects of phototoxic and photoallergic reactions were used to demonstrate what is currently known about photoinduced reactions and how to treat them. RESULTS: Upon interaction of solar UV radiation with the chemical that is present in significant levels on the skin, one of two known reactions may occur in susceptible patients: a phototoxicity and/or photoallergy. Photot

Phototoxicity and photoallergy.

Photosensitivity may be phototoxic or photoallergic. Phototoxicity is much more common. There are 2 types of phototoxicity: photodynamic, which requires oxygen, and nonphotodynamic, which does not. Reactions induced by porphyrin molecules, coal tar derivatives, and many drugs are photodynamic. The reaction induced by psoralens, for the most part, is nonphotodynamic. Acute phototoxic reactions are characterized by erythema and edema followed by hyperpigmentation. Long-term ultraviolet phototoxicity results in chronic sun damage and skin cancer formation. Also, certain chemicals such as psoralen

Drug-induced photosensitivity.

Characteristics and diagnosis of photosensitivity are discussed, and drugs available in the United States that cause photosensitivity are identified. In phototoxic reactions, the drug absorbs energy from ultraviolet A (UVA) light and releases it into the skin, causing cellular damage. In photoallergic reactions, light may cause a structural change in a drug so that it acts as a hapten, possibly by binding to proteins in the skin. Once a hapten-protein complex is formed, Langerhans' cells residing in the epidermis can present the antigen to immunocompetent cells, causing hypersensitivity. Photo

Practical Techniques: Photopatch Testing.

Photopatch testing (PPT) represents the gold standard diagnostic evaluation for photoallergic contact dermatitis (PACD), a type IV hypersensitivity reaction to an antigen that occurs in the presence of ultraviolet A radiation. The most common photoallergens in the United States are organic sunscreen filters, whereas topical NSAIDs predominate in Europe. PACD is not rare among photosensitive patients; however, PPT is underperformed in clinical practice due to several limiting factors. Herein, we provide practical approaches to PPT based on expert opinion, including straightforward recommendatio

[The syndrome of chronic actinic dermatitis. Persistent light reaction--actinic reticuloid].

The clinical, histological and photobiological features of persistent light reaction and actinic reticuloid are discussed with reference to the literature. Both conditions are characterized by extreme light sensitivity, have been reported exclusively in elderly men, and are interpreted as a consequence of a photoallergic reaction. In our view, persistent light reaction and actinic reticuloid do not represent separate entities, but rather the two extremes of a single spectrum of disease; in common with other workers, we prefer to call it chronic actinic dermatitis. This opinion is supported by

Variable Presentations of Photodermatoses - A Case Series.

Photodermatoses represent a broad spectrum of skin diseases caused because of exposure to ultraviolet radiation. It is categorized mainly as idiopathic photosensitive disorders, drug or chemical induced photosensitivity reactions, DNA repair-deficiency photodermatoses and photoaggravated dermatoses. Despite being under the photodermatoses umbrella, the pathophysiology of each type of photodermatoses varies. We reported 4 cases of photodermatoses including azathioprine induced pellagra, adult onset actinic prurigo, and photoallergic contact dermatitis due to NSAIDs and cutting fluids. Photoprot

Photocontact allergy due to suprofen.

A 79-year-old Japanese woman developed edematous erythema on sun-exposed areas 3 months after applying Sulprotin ointment, which contains 1% suprofen (SP), a phenylacetic acid derivative. A patch test with Sulprotin ointment as it was negative, but a photopatch test with Sulprotein ointment as is was positive. A photopatch test with 10-3% SP was positive, as was that obtained with an SP analogue, 3% ketoprofen, but that with the ointment base of Sulprotin was negative. Photopatch tests with other SP analogues such as flurbiprofen, ibuprofen, and pranoprofen were negative. She developed abnorma
