This column reviews 3 new systemic antifungal agents (posaconazole, micafungin, and anidulafungin) from the standpoint of dermatology. Posaconazole, approved to treat invasive Aspergillus and Candida infections, is available in an oral suspension and resembles fluconazole, but seems to have a broader spectrum of activity. Posaconazole is effective against yeasts and molds and could be effective in treating rare fungal infections involving Zygomycetes, Mucor necrotizing fasciitis, rhinocerebral mucormycosis, some Fusarium species, Penicillium, Histoplasma, Blastomyces, Coccidioides, Paracoccidi

BACKGROUND: Eosinophilic Fasciitis (EF) is a rare immune-mediated fibrosing disorder of the fascia. Immune Checkpoint Inhibitors (ICIs) have transformed oncologic therapy but may precipitate immune-related Adverse Events (irAEs), including dermatologic and rheumatologic manifestations. ICI-associated EF has been increasingly reported, yet remains poorly characterized.
OBJECTIVE: To systematically summarize the clinical features, diagnostic approaches, management strategies, and outcomes of ICI-associated EF.
METHODS: A systematic review was conducted in accordance with PRISMA 2020. PubMed/ME

Petrolatum, also known as petroleum jelly, is a widely used topical agent, with a variety of uses in dermatology. Despite its popularity, many myths surround this ubiquitous dermatologic staple. This review details the history of petrolatum and how it is manufactured as well as how its biologic properties make it a great moisturizer. Additionally, data on its potential for flammability, allergenicity, and comedogenicity are detailed, dispelling misconceptions about petrolatum use around oxygen and as a cause of acne. The uses and benefits of petrolatum in dermatology are wide-ranging-a patch t

The landscape of available therapeutic options for treatment of genitourinary (GU) cancers is expanding dramatically. Many of these treatments have distinct, sometimes severe, skin toxicities including morbilliform, bullous, pustular, lichenoid, eczematous, psoriasiform, and palmoplantar eruptions. Pruritus and skin pigmentation changes have also been noted. This review aims to synthesize dermatologic events observed with antibody drug conjugates, poly (ADP-ribose) polymerase (PARP) inhibitors, androgen receptor pathway inhibitors, tyrosine kinase inhibitors, immune checkpoint inhibitors, and

Hepatitis C virus infection has been associated with many dermatologic conditions such as lichen planus, porphyria cutanea tarda, and cryoglobulinemia. Recently, an association of HCV with systemic sclerosis has been reported. However, there are few reports of the association of localized scleroderma or morphea with Hepatitis C Virus infection. We describe the case of a 36 years old female patient suffering from prolonged morphea with difficult management, who was recently diagnosed of Hepatitis C Virus and received direct-acting antiviral agents treatment with Hepatitis C Virus clearance. Molluscum contagiosum (MC) is a common dermatologic infection that usually affects school-aged children, sexually active young adults, and immunocompromised individuals. It is a benign and self-limiting disease, with most cases undergoing spontaneous resolution within 6 to 9 months. However, a more severe and prolonged course is associated with immunosuppression or atopic dermatitis. Treatment is recommended because of the high rate of associated symptoms, risk for transmissibility, and cosmetic or social concerns. Therapeutic modalities are subdivided into 3 types: destructive, immunomodulato

Superficial chemical peels and microdermabrasion are used for many dermatologic conditions. A common condition treated with these modalities is acne vulgaris. In this review, we discuss the theory behind the technique of these procedures and describe the application and complications of each of these procedures in the office setting. The evaluation of patients before proceeding with the procedure and discuss pre- and postpeel regimens used for patients is discussed. We also analyze studies on both of these in-office procedures and comparative studies between the 2 most commonly used superficia

Dermatophytosis is an infection of the hair, skin, or nails caused by a dermatophyte, which is most commonly of the Trichophyton genus and less commonly of the Microsporum or Epidermophyton genera. Tinea capitis, tinea pedis, and onychomycosis are common dermatologic diseases that may result from such an infection. The treatment of fungal infections caused by a dermatophyte has been successful when treated with oral or topical antifungal agents. Terbinafine, itraconazole, and fluconazole are oral antimycotics that are effective in the treatment of superficial mycoses, although, depending on th

The treatment of disorders of hyperpigmentation including melasma, photoaging-related dyschromia, and postinflammatory hyperpigmentation pose numerous challenges, especially in patients with higher Fitzpatrick skin types (skin of color). Given limitations in efficacy as well as safety and cost considerations of available therapies, a "magic bullet" single treatment modality for hyperpigmentation is currently lacking. Successful treatment typically involves a combination of topical agents with or without in-office procedures, exploiting the different mechanisms of action of each agent or treatm

UNLABELLED: A 36-year-old woman with chronic psoriasis developed blurring of vision and intraepithelial infiltrates within the flap in the left eye concurrent with the appearance of acute psoriatic generalized skin lesions 14 days after bilateral laser in situ keratomileusis. After treatment with topical antibiotic and steroid agents and topical artificial tears, the intraepithelial infiltrates within the flap completely resolved. The final uncorrected distance visual acuity was 1.0 in both eyes, along with complete resolution of the dermatologic psoriatic lesions.

Evidence-based dermatology (EBD) is the application of the principles of evidence-based medicine to the diagnosis and treatment of skin disorders. EBD does not discount the individual dermatologist's clinical judgment. In fact, EBD is based on the interaction of external evidence, the physician's clinical experience, and the patient's experience. Randomized controlled trials constitute one of the highest levels of evidence and are the gold standard for validating a therapeutic intervention. For the treatment of rosacea, oral tetracycline, topical metronidazole, topical azelaic acid, and topica

BACKGROUND: Psoriasis vulgaris is one of the most common dermatologic diseases in Germany with a prevalence of about 2%. Along with efficacy and safety, costs are an important criterion for selecting appropriate therapy for this chronic disease. The majority of the patients with mild to moderate psoriasis vulgaris can be successfully treated with topical therapy alone. The most important agents are vitamin D(3) analogs and corticosteroids either as single agents or in a fixed combination (Daivobet).
METHODS: For the evaluation of the cost-effectiveness of different topical psoriasis treatment

Adenosine triphosphate-binding cassette subfamily B member 1 (ABCB1), also known as permeability glycoprotein, multidrug-resistant protein 1, or cluster of differentiation 243 (CD243), is a crucial protein for purging foreign substances from cells. The functions of ABCB1 have been investigated extensively for their roles in cancer, stem cells, and drug resistance. Abundant pharmacogenetic studies have been conducted on ABCB1 and its association with treatment responsiveness to various agents, particularly chemotherapeutic and immunomodulatory agents. However, its functions in the skin and impl

Allergic contact dermatitis associated with topical antimicrobial agents is an increasing problem in the postoperative wound care period. We reviewed the topical antimicrobial agents most commonly used postoperatively in North America and Europe, examined the incidence of allergic contact dermatitis from each agent, and provided guidelines for the use of topical antimicrobials on closed and open wounds in the postoperative period. Neomycin was the most common cause of allergic contact dermatitis both in the general patch-tested population (11%) and in the postsurgical population. Bacitracin wa

BACKGROUND: Natural biomedicines (NBMs) are frequently used to manage immune-mediated and inflammatory dermatologic diseases (IMIDDs). This systematic review evaluates the efficacy, safety, and clinical relevance of NBMs in IMIDDs, providing an evidence-based analysis to guide dermatologic practice.
METHODS: Following PRISMA guidelines, a systematic search was conducted in PubMed, Embase, Medline, and Google Scholar for randomized controlled trials (RCTs) investigating NBMs in IMIDDs from 1990 to 2023. Studies were included if they met predefined eligibility criteria: RCT design, relevant IMI

Etanercept, an inhibitor of TNF alpha, has been found effective in the treatment of psoriatic arthritis and psoriasis vulgaris. Etanercept is a fusion protein made up of domains of the soluble, fully human p75-TNF alpha receptor and the F(c) portion of human IgG(1). The drug is a protein which must be administered subcutaneously. Several controlled studies have highlighted its efficacy for both skin symptoms and joint involvement. The usual dose is 25 mg s.c. twice weekly. Higher dosages of 50 mg twice weekly may be used in severe cases. Before starting the therapy with etanercept, infections

The use of aesthetic dermatologic procedures in patients with underlying allergic and inflammatory skin diseases is on the rise, but specific safety considerations are often overlooked. Individuals with hypersensitivity to injectables, chronic inducible urticaria, allergic contact dermatitis, atopic dermatitis, or hereditary angioedema may face an increased risk of adverse reactions triggered by mechanical trauma, injected substances, or disruption of the skin barrier. Adverse reactions to local anesthetics are predominantly non-IgE-mediated. Thus, preventive strategies should focus on selecti

PURPOSE OF REVIEW: Preexposure prophylaxis (PrEP) represents a cornerstone in HIV prevention, yet its broader clinical implications are less frequently explored. This review summarises current evidence on dermatological manifestations associated with PrEP use, including drug-induced reactions, sexually transmitted and contact-acquired infections, and cutaneous effects related to recreational substance use and chemsex practices.
RECENT FINDINGS: Antiretroviral agents approved for PrEP exhibit excellent dermatologic tolerability, with cutaneous adverse drug reactions rarely reported in clinical

BACKGROUND: Antibiotics are often prescribed in perioperative settings, including dermatologic surgery. Given the continued evolution in the breadth and complexity of cutaneous procedures performed and inevitable localized or distant infections that occasionally occur, it seems prudent to periodically evaluate findings and recommendations from the literature regarding the use of antibiotics in cutaneous surgery.
MATERIALS AND METHODS: Literature review from English-language sources from the past 30 years, especially focusing on sources from the past 5 to 10 years. Data were examined for a var
