Laparoscopic Repair of Congenital Diaphragmatic Hernia in Adults.

Background, Aims, and Objectives. Congenital diaphragmatic hernia typically presents in childhood but in adults is extremely rare entity. Surgery is indicated for symptomatic and asymptomatic patients who are fit for surgery. It can be done by laparotomy, thoracotomy, thoracoscopy, or laparoscopy. With the advent of minimal access techniques, the open surgical repair for this hernia has decreased and results are comparable with early recovery and less hospital stay. The aim of this study is to establish that laparoscopic repair of congenital diaphragmatic hernia is a safe and effective modalit

Diaphragmatic hernias: a spectrum of radiographic appearances.

Diaphragmatic hernias are common, and although frequently incidental, recognition of both benign and life-threatening manifestations of diaphragmatic hernias is necessary to guide appropriate management. Congenital fetal diaphragmatic hernias, traumatic diaphragmatic rupture, and large symptomatic Bochdalek, Morgagni, and hiatal hernias are typically repaired surgically, while eventration, diaphragmatic slips, and small diaphragmatic hernias do not require intervention or imaging follow-up but should be recognized to avoid confusion with other diagnoses that require additional attention. Eventration of diaphragm in adults: eleven years experience.

BACKGROUND: Eventration of diaphragm is a congenital condition in which there is absence of muscle fibers in the diaphragm while maintaining all the anatomical attachments normally. Surgical treatment is warranted in symptomatic patients so as to reduce the abnormal ascent of diaphragm. The present study was conducted to analyse the perioperative outcome of thoracotomy in adult patients with diaphragmatic eventration. METHODS: This descriptive case series was carried out in Cardiothoracic Surgery-Unit, Postgraduate Medical Institute, Lady Reading Hospital, Peshawar, Pakistan. Medical records …

[Effects of diaphragmatic plication in the antireflux barrier in rats].

INTRODUCTION: There is increasing evidence of frequent occurrence of gastroesophageal reflux (GER) in patients surviving operations for congenital diaphragmatic hernia (CDH) and diaphragmatic eventration (DE). The murine model allows to study the behavior of the components of antireflux barrier. AIM: To study the changes introduced in esophageal-gastric junction by DE due to cervical transection of the left phrenic nerve and subsequent plication of the paralyzed diaphragm. MATERIAL AND METHODS: Adult male Wistar rats were divided into two groups: in one we measured the pressure conditions be…

Lower Thoracic Epidural Block in Diaphragmatic Plication: A Case Report.

Acquired diaphragmatic eventration (DE) is an uncommon condition characterized by all or a portion of the diaphragmatic muscle replaced by a thin fibroelastic sheath, resulting in an abnormal elevation of the hemidiaphragm. Currently, the treatment of choice for symptomatic DE is a minimally invasive diaphragmatic plication via a video-assisted thoracoscopic or robotic approach. Surgical diaphragmatic plication is often associated with increased intra-abdominal pressure (IAP) and associated complications. This report describes the usefulness of lower thoracic epidural block in the management o…

Intrathoracic Kidney after Blunt Abdominal Trauma: A Case Report and Review of the Literature.

Abdominal trauma is responsible for most genitourinary injuries. The incidence of renal artery injury and intrathoracic kidney is quite low in patients who present with blunt trauma experiencing damage. There are four defined etiologies for intrathoracic kidney, which include real intrathoracic ectopic kidney, eventration of the diaphragm, congenital diaphragmatic herniation, and traumatic diaphragmatic rupture. The traumatic intrathoracic kidney is an extremely rare case. We presented intrathoracic kidney case after traumatic posterior diaphragmatic rupture.

Collagen-Vicryl scaffolds for reconstruction of the diaphragm in a large animal model.

Current methods for closure of congenital diaphragmatic hernia using patches are unsatisfactory, and novel collagen-based scaffolds have been developed, and successfully applied in a rat model. However, for translation to the human situation constructs must be evaluated in larger animal models. We developed collagen scaffolds enforced with Vicryl, loaded either with or without the muscle stimulatory growth factor insulin-like growth factor 1 (IGF1). We describe our steps to a surgical method to implant these scaffolds into a diaphragmatic defect in 1.5–3 week old lambs, and evaluate the scaffo

New technique of diaphragmatic plication by means of uniportal video-assisted thoracoscopic surgery.

Diaphragmatic plication has been a well employed method for the treatment of diaphragmatic eventration and/or paralysis. Uniportal thoracoscopic procedures seem to offer substantial benefit in terms of postoperative pain and cosmesis with similar outcomes as other surgical techniques. A 60-year-old female patient was referred to our clinic for the surgical treatment of diaphragmatic eventration of an unknown cause. The patient was referred to our institution from another hospital, where she had undergone a femoro-popliteal and crural bypass which was unsuccessful, requiring amputation of her l…

Spondylothoracic dysplasia with diaphragmatic defect: a case report with literature review.

Spondylothoracic dysplasia (Jarcho-Levin syndrome) is a syndrome of unknown etiology. We describe a new case with diaphragmatic eventration. Literature review for cases of Jarcho-Levin syndrome with diaphragmatic defects, which were six cases, revealed that renal affection increased when diaphragmatic defects associate the syndrome with pulmonary hypoplasia. Thus, the subgroup of spondylothoracic dysplasia with diaphragmatic defect is a more severe subgroup of the syndrome rather than the other forms of this syndrome. Relating the described anomalies in this case and that of the literature cas…

Hepatic heterotopia in congenital diaphragmatic anomaly.

An antenatally diagnosed case of a left congenital diaphragmatic hernia is reported. The diaphragmatic eventration hernia sac consisted of a sheet of ectopic liver tissue in continuity with a hypoplastic left lobe which formed the medial anterior and posterior walls of the hernia is presented. The operative management of this combination of defects has not previously been described in English literature. Embryological considerations, limitations of accurate preoperative diagnosis, technical challenge in the operative repair of the defect and need for drainage is discussed.

[Predictive value of prenatal MRI in the diagnosis of thoracic congenital malformations].

INTRODUCTION: Magnetic imaging (MI) has an increasing value in the prenatal diagnosis of thoracic malformations. MATERIAL AND METHOUS: We compare in this work the prenatal diagnoses with the prenatal sonographic diagnoses and postnatal imaging, surgical or postmortem findings. RESULTS: Prenatal sonography diagnosed 5 diaphragmatic hernias (CDH) and 3 cystic adenomatoid malformations (CAM). MI confirmed left side CDH in 4 cases, in two of them showing also herniation of the left hepatic lobe and the spleen. In the 5th case, MI suggested diaphragmatic eventration with partial occupation of the …

Novel features of PIK3CA-Related Overgrowth Spectrum: Lesson from an aborted fetus presenting a de novo constitutional PIK3CA mutation.

PIK3CA-Related Overgrowth Spectrum (PROS) encompass a group of disorders which are mainly characterized by segmental overgrowth of several t … mnios. Fetal autopsy contributed to better characterize the phenotype, showing megalencephaly, left diaphragmatic eventration, facial dysmorphism (hypertelorism, abnormal hair line implantation) and duplication of distal portion of the small bowel. Clinical exome sequencing identified a de novo constitutional variant c.1030G>A p.(Val344Met) in PIK3CA. Although this mutatio …

Management of Diaphragm Paralysis and Eventration.

An elevated diaphragm may be due to eventration or paralysis. Diaphragm elevation is often asymptomatic and found incidentally on imaging. Fluoroscopic testing can be used to differentiate eventration (no paradoxic motion) from paralysis (paradoxic motion). Regardless of etiology, a diaphragm plication is indicated in all symptomatic patients with an elevated diaphragm. Plication can be approached either from a thoracic or abdominal approach, though most thoracic surgeons perform minimally invasive thoracoscopic plication. The goal of plication is to improve lung volumes and decrease paradoxic

[Diaphragmatic palsy and dysfunction: from physiology to surgery].

The clinical presentations of diaphragm dysfunctions vary according to etiologies and unilateral or bilateral diseases. Elevation of the hemidiaphragm from peripheral origins, the most frequent situation, requires a surgical treatment only in case of major functional impact. Complete morphological and functional analyses of the neuromuscular chain and respiratory tests allow the best selection of patients to be operated. The surgical procedure may be proposed only when the diaphragm dysfunction is permanent and irreversible. Diaphragm plication for eventration through a short lateral thoracoto
