Small intestinal glucoamylase deficiency and starch malabsorption: a newly recognized alpha-glucosidase deficiency in children.

To determine the prevalence of short polymers of glucose and starch malabsorption caused by small intestinal glucoamylase deficiency in children with chronic diarrhea, we studied small bowel biopsy specimens from 511 children (aged 1 month to 9 years) with chronic diarrhea evaluated at 54 medical centers. Glucoamylase and disaccharidase (lactase, sucrase, maltase, and palatinase) enzyme assays were performed. Of the 511 children, 15 had glucoamylase deficiency. Six who had significant small intestinal mucosal injury and disaccharidase deficiencies were defined as having secondary glucoamylase

Formula allergy and intolerance.

There are two major types of adverse reactions in infant formulas: (1) formula allergy/hypersensitivity, which is an immunologic response, and (2) formula intolerance, which is a nonimmunologic response. Formula intolerance can occur in infants with an underlying congenital or acquired enzyme deficiency (disaccharidase deficiency, galactosemia, hereditary fructose intolerance). The clinical presentation, diagnosis, and treatment of both reactions are reviewed in this article. The appropriateness of the use of a variety of infant formulas is discussed. Guidelines for the prevention of allergic

Stool chemistries in patients with unexplained diarrhea.

In approaching the differential diagnosis of unexplained diarrheal disorders, separation of osmotic and secretory causes is important. The osmotic gap between measured stool osmolality and osmolality calculated from stool sodium and potassium concentrations is low in secretory diarrhea and high in osmotic diarrhea. Cholera, villous adenoma and bile salts are among the causes of secretory diarrhea. Causes of osmotic diarrhea include monosaccharidase deficiency, disaccharidase deficiency, excessive use of sugar-free gum and excessive ingestion of magnesium antacids.

Morphological and biochemical changes in the gastric mucosa of A/HEJ mice injected with a xenogeneic stomach antigen.

Subcutaneous injection of W/Fu rat stomach antigen emulsified with an equal volume of Freund's complete adjuvant at 1 week interval for a total of 2 administrations induced atrophic gastritis in A/HeJ mice. From the second week after the last injection the parietal cell mass decreased. At the same time, Alcian blue-PAS positive cells and Paneth-like cells appeared. Furthermore, the activity of alkaline phosphatase and disaccharidase of the small intestinal mucosa was biochemically demonstrated in the atrophic glandular stomach.

Role of CD8+ and CD4+ T lymphocytes in jejunal mucosal injury during murine giardiasis.

T-cell-mediated pathogenesis has been documented in various idiopathic and microbially induced intestinal disorders. Diffuse microvillous shortening seen in giardiasis is responsible for disaccharidase insufficiencies and malabsorption of electrolytes, nutrients, and water. Other mucosal changes include crypt hyperplasia and increased numbers of intraepithelial lymphocytes (IEL). A recent report using an athymic mouse model of infection showed that these epithelial injuries were dependent on T cells. The aim of the present study was to identify which subset of superior mesenteric lymph node (S

Use of breath hydrogen testing to detect experimentally induced disaccharide malabsorption in healthy adult dogs.

OBJECTIVE: To develop a noninvasive method to detect disaccharide malabsorption in dogs by measuring hydrogen concentration ([H2]) in exhaled breath before and after experimentally induced disaccharide malabsorption. ANIMALS: 8 healthy mixed-breed dogs. PROCEDURE: [H2] was measured every 30 minutes for 8 hours after administration of disaccharide solutions (lactose, 0.5 g/kg of body weight; lactose, 1.0 g/kg; sucrose, 2.0 g/kg; maltose, 1.5 g/kg; and lactose [0.5 g/kg] and sucrose [2.0 g/kg]) to determine reference ranges of [H2] for each solution, which were compared with [H2] in dogs with

[Hereditary deficiency of isomaltase and saccharase responsible for a malabsorption syndrone (author's transl)].

Among 135 infants and children with a supposed malabsorption syndrome, a deficiency of isomaltase-saccharase of the duodenal mucosa was detected in 5 cases by measuring the disaccharidases directly in the mucosa homogenate. In one instance a deficiency of lactase was found in addition. In all patients the villi were of normal length, with an increased cell infiltration of the stroma detected in two cases. The loading tests with xylose-sucrose yielded a diminuished rise in the blood glucose level. Three of the patients were dwarfish, but only one showed an increased growth after the reduction o…

Small intestinal changes in enterocolitis complicating Hirschsprung's disease.

Hirschsprung's disease is one of the more common causes of childhood bowel obstruction. The disease can cause enterocolitis which, untreated, may result in considerable mortality. We describe our evaluation of two infants who suffered from intractable diarrhea of infancy secondary to Hirschsprung's disease. We found that mucosal damage to the small bowel and disaccharidase deficiency are among the most important mechanisms producing this complications. Therefore, management of enterocolitis including parenteral alimentation should proceed as soon as the complication is observed.

Demystifying Carbohydrate Maldigestion: A Clinical Review.

Carbohydrate intolerance is a poorly understood and potentially overlooked cause of unexplained gastrointestinal symptoms, particularly among patients with disorders of gut-brain interaction. Symptoms related to carbohydrate intolerance arise from bacterial fermentation of unabsorbed carbohydrates leading to increased gases and osmotic load within the gastrointestinal tract. This mechanism is shared across various carbohydrates, including lactose, sucrose, maltose, fructose, and fermentable oligosaccharides, disaccharides, monosaccharides, and polyols. Primary forms of carbohydrate maldigestio

The role of eicosanoids in the process of adaptation following massive bowel resection in the rat.

BACKGROUND: Long chain polyunsaturated fatty acids (LCPUFAs) such as arachidonic acid (AA) and eicosapentaenoic acid (EPA) stimulate intestinal adaptation. Prostaglandins also enhance intestinal adaptation. The purpose of this study was to determine by which eicosanoid pathway dietary arachidonic acid enhances intestinal adaptation. Cyclo-oxygenase or lipoxygenase were selectively inhibited to determine whether either of them enhanced or inhibited adaptation. METHODS: Sixty Sprague-Dawley rats were divided into 2 groups, one receiving an 80% small bowel resection and the other receiving a sha

Effect of intraamniotic dexamethasone administration on intestinal absorption in a rabbit gastroschisis model.

Infants with gastroschisis experience delayed intestinal motility and absorption for several weeks after birth. This intestinal dysfunction is believed to occur primarily in the third trimester and to be largely caused by the prolonged exposure of the intestine to amniotic fluid. Previous studies have shown that prenatal steroid administration will enhance mucosal disaccharidase activity and nutrient uptake. The present study evaluates the effects of dexamethasone on intestinal function in a rabbit fetal gastroschisis model. Thirty-four fetuses from 10 New Zealand white rabbits were divided in

Two-step procedure of whole orthotopic intestinal transplantation in rats: considerations of techniques and graft functional adaptation.

Intestinal transplantation may eventually become the definitive therapeutic modality for irreversible intestinal failure. To date, immunological, functional, and technical problems still hamper clinical success. In this paper, the technical aspects of a two-stage procedure for a whole orthotopic intestinal transplantation model of rats with systemic drainage are illustrated. Graft mucosal Na(+)/K(+)-ATPase and disaccharidase revealed recovery of graft function. From this study, we conclude that the present model may provide an excellent tool to further investigate the physiology and immunology

Brush border enzymes in coeliac disease: histochemical evaluation.

Two hundred and ninety four duodenal and jejunal mucosal biopsy specimens from patients with coeliac disease, treated and untreated, and other conditions were examined histologically and by histochemical staining for five peptidase and three disaccharidase enzymes to determine profiles of activity. Suppression of activity paralleled the histology with the following enzymes: lactase, trehalase, brush border endopeptidase, dipeptidyl peptidase II and isomaltase. Lactase, trehalase, and brush border endopeptidase were specifically suppressed in untreated coeliac disease and were diagnostically us

High carbohydrates consumption and inflammatory bowel diseases.

High consumption of potatoes, soft drinks, cornflakes, chocolates, mushrooms and other common examples of complex carbohydrates in the western side of the globe is significant. Similarly, Inflammatory Bowel Diseases (IBD) (Crohn's disease and ulcerative colitis) are also prevalent in these regions. Evidence exists to support that factors, such as lack of zinc (it is common in patients with IBD, in particular Crohn's patients) may significantly affect the activity of some enzymes, such as, disaccharidases and other digesting enzymes of carbohydrates and that would lead to the recruitment of inc…

Altered small bowel propulsion associated with parasitism.

Experiments were designed to test the hypothesis that alterations of intestinal motility accompany enteric helminth infections. Motility was measured in rats during the intestinal phase of infection with the nematode Trichinella spiralis by following the transit of radioactive chromium through the gut. Intraduodenal catheters were surgically implanted in rats. One week after the operation one group of animals was infected with 8 x 10(3) T. spiralis larvae and a second group was infected with 16 x10(3) larvae. A third group of uninfected rats served as controls. Three, four, or five days postin

Seasonal Changes and Phenotypic Flexibility in the Digestive System of the Hibernating Marsupial Dromiciops gliroides.

During hibernation, reduced metabolic activity enables to withstand periods of limited resources and harsh weather. Therefore, animals exhibit reduced activity and decreased feeding, which involves energy savings. Thus, a reduction in both physiological and morphological digestive capacities is expected due to the prolonged metabolic depression of hibernation, which involves phenotypic flexibility of the digestive system at several levels. We studied Dromiciops gliroides a heterothermic marsupial endemic of the temperate rainforests of southern South America, by comparing the morphology and ph

Using a cutoff of <10 ppm for breath hydrogen testing: a review of five years' experience.

To assess the clinical use of the breath hydrogen test in a large community hospital using a <10 ppm cutoff, we reviewed 222 tests performed over a 5-year period to evaluate patients for disaccharidase deficiency or bacterial overgrowth of the small intestine. Of these, the vast majority (195) were for lactose malabsorption, although fructose (17), sucrose (8) and lactulose (2) were also occasionally administered. One hundred eleven tests (50 percent) were positive, with an increase of at least 10 ppm hydrogen above the fasting level and a maximum value most commonly observed (42.3 percent of
