Optimal Vessel Sizing and Understanding Dissections in Infrapopliteal Interventions: Data From the iDissection Below the Knee Study.

PURPOSE: To investigate if imaging with intravascular ultrasound (IVUS) yields a more accurate estimate of vessel diameter and the presence of dissections than angiography after intervention in the infrapopliteal arteries. MATERIALS AND METHODS: A prospective, single-center study enrolled 20 consecutive patients (mean age 74.1±12.4 years; 12 women) with infrapopliteal disease who were treated with percutaneous transluminal angioplasty (PTA; n=10) or orbital atherectomy (OA) followed by PTA (n=10). The majority of patients were hypertensive and half were diabetic. The overall lesion length was

Early Intraluminal Frozen Elephant Trunk Stent Graft Thrombosis After Aortic Arch Surgery.

BACKGROUND: Partial intraluminal thrombosis of the frozen elephant trunk (FET) stent graft is a poorly described but not infrequent complication after aortic arch surgery. This study aims to describe and analyze the occurrence of early FET stent graft thrombosis. METHODS: Retrospective single-center analysis including patients who underwent aortic arch replacement with FET technique between 2006 and 2020. Stent graft thrombosis was diagnosed through computed tomography scan. Several computed tomography scan parameters and clinical variables were analyzed as predictors of this event. RESULTS:

Changing Management of Type B Aortic Dissections.

The purpose of this manuscript is to review recent trends in the management of acute type B aortic dissection. Due to its efficacy and low morbidity, thoracic endografting has rapidly been adopted as the treatment of choice for most patients with malperfusion or rupture as a consequence of acute aortic dissection. This technology is increasingly applied to patients without rupture or malperfusion, so-called "uncomplicated" dissections, to reduce the incidence of late aneurysmal degeneration in the ungrafted segments of the thoracoabdominal aorta. A variety of techniques have been proposed, inc

Distal Aortic Malperfusion Exacerbated by Antegrade Stent-Graft Placement During Hybrid Repair of Acute DeBakey Type I Aortic Dissection.

We present the case of an acute DeBakey type I aortic dissection with malperfusion. The patient underwent valve resuspension, ascending aortic and partial arch replacement, debranching of the innominate artery, and placement of a small-diameter stent within the left common carotid artery, after which antegrade deployment of a stent-graft into the proximal descending thoracic aorta was performed to expand the true lumen. Distal malperfusion was exacerbated by the stent-graft's traversal into the false lumen, necessitating further endovascular repair to reestablish flow to the distal aorta. Miti

Resternotomy aortic root and arch replacement following previous complex type A aortic dissection requiring endovascular repair for malperfusion.

A redo sternotomy, aortic root, and arch replacement in a patient following previous complex surgical and endovascular type A aortic dissection repair is presented in this video case report. Shortly after having the initial type A aortic dissection repair with replacement of the ascending aorta, the patient developed severe visceral malperfusion due to a compressed distal true lumen and underwent emergency endovascular repair with ascending arch and descending thoracic aorta stents and chimney grafts for the aortic arch vessels as well as fenestration of the intimal flap of the abdominal aorta

Multi-stage open surgical and endovascular treatment of progressive aortic degeneration in a patient with Marfan syndrome.

Aortic interventions in patients with underlying connective tissues disorders present a unique challenge. The inevitable degeneration of the native aorta can lead to the need for multiple staged interventions with significant risk of complications associated with reoperative aortic procedures. We present a challenging case of progressive aortic degeneration in a patient with Marfan syndrome treated with multi-staged open surgical and endovascular procedures.

Favorable remodeling after hybrid arch debranching and modified provisional extension to induce complete attachment technique in type a aortic dissection: A case report.

RATIONALE: Type A aortic dissection (TAAD) usually requires emergency open repair of the ascending aorta. In cases of diffuse dissection that spreads along the descending and abdominal aorta (type I, DeBakey classification), the risk of aneurysmal degeneration varies between 30% and 50% during 5 years and increases even higher during a longer follow-up. Those patients might require complex intervention to prevent aortic rupture. A combination of hybrid arch debranching and the extended provisional extension to induce complete attachment (e-PETTICOAT) technique might be an available alternative

Treatment of Complex Thoracoabdominal Aortic Disease.

Thoracoabdominal aortic aneurysms are increasing in incidence. Rupture is associated with a high rate of morbidity and mortality. The historic gold standard of open repair can be performed with low rates of complications at centers of excellence. However, these results are not universally achievable, with significantly higher rates of mortality reported from statewide studies. With the advent of endovascular therapy, techniques to mitigate the physiologic stress of open surgery have been developed. Hybrid open/endovascular operations are being undertaken with total visceral debranching followe

Temporary extracorporeal brachio-femoral vascular prosthesis shunt for ischemia prevention in an operation for abdominal aortic and iliac aneurysms in a patient with Marfan's syndrome.

In open vascular repair, when prolonged infrarenal aortic clamping can be expected, and collateral perfusion is reduced, the use of a temporary shunt may reduce the risk of ischemic complications. In a patient with Marfan's syndrome and aortic dissection who had developed infrarenal aneurysms, segmental arteries had been occluded by prior aortic surgery and collateral arteries in the anterior torso could have been damaged by previous pectus excavatum, muscle flap, sternotomy, and ventral hernia operations. The axillary artery was dilated. For the prevention of ischemia during open repair with

Retrograde trans-posterior communicating artery snare-assisted rescue of lost access to a foreshortened pipeline embolization device: complication management.

BACKGROUND AND IMPORTANCE: The Pipeline embolization device (PED; Covidien Vascular Therapies, Mansfield, Massachusetts) is a promising, yet experimental, vascular reconstruction device for the treatment of complex intracranial aneurysms. We present a PED-related complication and describe a salvage strategy. CLINICAL PRESENTATION: A 64-year-old woman underwent PED-assisted parent vessel reconstruction for her giant cavernous internal carotid artery (ICA) aneurysm. During placement of the first PED, the proximal part of the PED foreshortened and was displaced into the aneurysm sac. Multiple su

A combination of preductal aortic coarctation and type B dissection: report of a case.

We present herein the case of a 39-year-old man found to have significant coarctation of the preductal aorta combined with a type B dissection, associated with Marfan's syndrome. This extremely rare pathological combination could be defined preoperatively only by a three-dimensional computed tomography scan. By means of selective cerebral perfusion, the coarctation was resected, and the total aortic arch was replaced with a Dacron graft.

Left thoracotomy removal of a fenestrated stent graft and arch/descending aortic replacement for chronic aortic dissection.

A 58-year-old woman with chronic aortic dissection developed rapid aneurysmal enlargement of the distal aortic arch and descending aorta following fenestrated thoracic endovascular aortic repair with left subclavian artery stenting. Computed tomography revealed a proximal type I endoleak associated with progressive aneurysmal dilatation. The patient underwent urgent open surgical repair via a left thoracotomy, including complete removal of the fenestrated stent graft and replacement of the distal aortic arch and descending aorta. The post-operative course was uneventful, and follow-up imaging

Zone 1 total arch replacement with frozen elephant trunk for type A dissection: early-to-mid-term outcomes.

BACKGROUND: Total arch replacement with frozen elephant trunk is widely used to treat acute Stanford type A aortic dissection. Zone 1 proximalization, which requires concomitant extra-anatomical left subclavian artery bypass, may provide a practical balance between safety and technical complexity. Although the early outcomes of this procedure have previously been reported, comprehensive mid-term data, particularly regarding left subclavian artery bypass durability, remain limited. This study aimed to evaluate the early and mid-term outcomes of zone 1 proximalization in total arch replacement w

A case of dissection of intracranial cerebral arteries with segmental mediolytic "arteritis".

The intravital diagnosis of intracranial arterial dissection is not always possible due to atypic and non-specific clinical and radiological presentations. The postmortem pathological examination of cerebral blood vessels is therefore necessary to establish or confirm the presence of a dissecting aneurysm of intracranial arteries. Most of the described cases showed no significant underlying vascular pathology. Here we present the case of a 24-year-old women who died 5 days after admission to the hospital for a rapidly developing right-sided hemisyndrome. Neuroradiological examination had revea
