Direct access of ascending aorta for endograft delivery in the descending thoracic aorta.

First-line therapy for aneurysm, dissection, or rupture of the descending thoracic aorta is now by the endovascular approach. Retrograde insertion of the endograft, through access from the femoral arteries, is the preferred approach. This case presents a new, innovative technique for delivery of an endoprosthesis into the descending thoracic aorta when hostile anatomy prevents delivery from the femoral arteries, iliac arteries, or infrarenal abdominal aorta in a patient not suitable for open repair.

Extended repair of thoracic aorta through left thoracotomy after aortic root replacement.

A patent distal false lumen after repair of type A aortic dissection often poses serious late complications. We present a successful repair of dissecting aortic aneurysm through left thoracotomy, extending from the ascending to descending thoracic aorta after composite graft replacement of the aortic root. Although staged operations, including the elephant trunk procedure, are usually chosen for remaining extensive aortic disease after replacement of the aortic root or the ascending aorta, a single-stage repair from the ascending to the descending thoracic aortas through left thoracotomy can b

Evaluation of Marfan patients status post valve-sparing aortic root replacement with 4D flow.

BACKGROUND: Over the past two decades elective valve-sparing aortic root replacement (V-SARR) has become more common in the treatment of patients with aortic root and ascending aortic aneurysms. Currently there are little data available to predict complications in the post-operative population. The study goal was to determine if altered flow patterns in the thoracic aorta, as measured by MRI, are associated with complications after V-SARR. METHODS: Time-resolved three-dimensional phase-contrast MRI (4D flow) was used to image 12 patients with Marfan syndrome after V-SARR. The patients were fo

Type B aortic dissection with rupture of the left common iliac artery: a case report.

Type B aortic dissection involves the appearance of a false lumen distal to the left subclavian artery and extending distally into the descending thoracic aorta and into the abdominal aorta. Complications of the dissection include rupture of the thoracic aorta, leg ischemia, visceral ischemia, and renal failure. A 37-year-old man presented with complaints of sudden onset of chest pain, left leg pain, and numbness. Examination revealed no femoral, popliteal, or distal pulses with decreased sensory and motor function on the left lower extremity. A CT scan revealed an aortic dissection at the pro

Successful remodeling and endovascular repair of a ruptured type B chronic aortic dissection 12 years after primary surgery.

A 70-year-old woman with a medical history of descending aorta replacement for chronic type B aortic dissection 12 years prior was admitted to our hospital with sudden back pain and hemoptysis. The patient was diagnosed with ruptured residual dissected thoracic aortic aneurysm and underwent emergent endovascular treatment. Two TAG thoracic endoprosthesis of different sizes were used to accommodate the discrepancy in size of the true lumen, resulting in a successful closure of the entry tear and hemostasis, without any damage to the intima. Computed tomography performed 3 months after surgery r

Clinical utility of magnetic resonance imaging in the follow-up of chronic aortic type B dissection.

Several imaging modalities are utilised in the assessment of disease progression in chronic aortic dissection. We present the case of a 66 year-old male who underwent ascending aorta repair for Stanford type A aortic dissection. On follow-up the persisting dissection of the descending thoracic aorta was observed to regress on magnetic resonance imaging (MRI). MRI has several advantages over computed tomography (CT) scanning and echocardiography in the follow-up phase of this disease.

Echocardiography in the diagnosis of thoracic aortic pathology.

Transthoracic two-dimensional and Doppler echocardiography has been well established as a useful technique for evaluating many pathologic processes affecting the thoracic aorta. However, the distance of the aortic arch and descending thoracic aorta from the chest wall and the interposition of highly attenuating lung and highly reflective mediastinal structures between the transducer and the aorta present unavoidable limitations. Transesophageal echocardiography is a relatively new technology that overcomes many of the inherent limitations with transthoracic imaging. Complete echocardiographic

Aortic remodeling after thoracic endovascular aortic repair and Provisional ExTension To Induce COmplete ATtachment to treat complicated type B aortic dissection.

OBJECTIVE: Aortic remodeling after thoracic endovascular aortic repair (TEVAR) for complicated type B aortic dissection remains insufficiently characterized, particularly when the Provisional ExTension To Induce COmplete ATtachment (PETTICOAT) technique is applied. This study evaluated early geometric remodeling of the true and false lumens and the whole aorta following TEVAR with PETTICOAT using patient-specific computational modeling. METHODS: A single-center retrospective review of 20 patients (16 men; age, 65 ± 11 years; nine acute, seven subacute, and four chronic) with type B aortic dis

Single-stage repair of acute type A aortic dissection associated with aortic coarctation, perimembranous ventricular septal defect, and bicuspid aortic valve.

A 17-year-old man with coarctation of the thoracic aorta, bicuspid aortic valve, perimembranous ventricular septal defect, and acute type A dissection of the ascending aorta underwent emergency repair for dissection by means of single-stage replacement of not only the aortic valve and ascending aorta, but also simultaneous repair of the coarctation by graft interposition. These combined conditions comprised some difficulties that included decisions on the optimal timing and sequence of surgical repair, technique, and circulatory support.

Spontaneous isolated dissection at renal upper abdominal aortic: a rare case report.

Arterial dissection is defined as a cleavage of the arterial wall caused by intramural hematoma. Isolated extra-aortic arterial dissection has been reported in renal and carotid arteries in few literatures but suprarenal aorta dissection associated with retrograde formation of a giant descending thoracic aneurysm is considered very rare. We present a quite unusual case of suprarenal aorta dissection associated with retrograde formation of a giant descending thoracic aneurysm sparing both renal and mesenteric vessels, without any branch vessel involvement or visceral damage. Because of the pati

Gross anatomical observations on two cases of right-sided arch of the aorta with the left subclavian artery as its last branch.

Two cases of Adachi-Williams-Nakagawa type N (Krause's type II-2-B) right-sided arch of the aorta were observed gross anatomically. We discovered the first in a 67-year-old female corpse during dissection practice. The second was found in an old male patient through radiography. After his death at 87 years, angiography revealed the anomaly to be of type N. Case 2 (and case 1 after the discovery of the anomaly) were dissected outside of regular dissection practice. Neither belonged to the "circumflexus" type and in each case the ligamentum arteriosum was located on the left, forming a vascular

Intimo-intimal intussusception: a rare clinical form of aortic dissection.

A 33-year-old hypertensive man presented with epigastric pain radiating to the back. Transoesophageal echocardiography (TOE) revealed an intimal flap on the aortic arch and descending aorta. No intimal flap of the ascending aorta was detected on TOE or CT. The diagnosis was made on opening the ascending aorta: complete circumferential dissection of the ascending aorta flush with the coronary ostia, with no residual intimal flap, and intimo-intimal glove-finger intussusception of the internal channel into the descending thoracic aorta. Aortic intussusception is a very rare form of Type I dissec

Review article: Accuracy of emergency physician performed point-of-care ultrasound of the thoracic aorta: A systematic review and narrative synthesis of the literature.

Point-of-care ultrasound (POCUS) is becoming ubiquitous in emergency medicine. POCUS for abdominal aortic aneurysm is well established in practice. The thoracic aorta can also be assessed by POCUS for dissection and aneurysm and transthoracic echocardiography is endorsed by international guidelines as an initial test for thoracic aortic pathologies. A systematic search of Ovid Medline, PubMed, EMBASE, SCOPUS and Web of Science from January 2000 to August 2022 identified four studies evaluating diagnostic accuracy of emergency physician POCUS for thoracic aortic dissection (TAD) and five studie

[The frozen elephant trunk technique for the treatment of complex lesions of the thoracic aorta].

The treatment of patients with extensive disease of the thoracic aorta represents a formidable challenge for the cardiovascular surgeon. The beginning of thoracic aortic endovascular aortic arch repair has promoted the development of different hybrid approaches as the frozen elephant trunk (FET) strategy, which includes classic arch replacement and antegrade stenting of the descending thoracic aorta. It represents an interesting approach for patients with extensive disease of the thoracic aorta, and its application has significantly increased over recent years. Our indications for this techniq

Single-Stage Repair of Thoracic Aortic Aneurysm through a Median Sternotomy in a Patient with Pseudocoarctation of the Aorta and Severe Aortic Valve Stenosis.

Pseudocoarctation of the aorta is a rare anomaly and considered a benign condition. Pseudocoarctation of the aorta has been associated with aneurysm formation in the thoracic aorta, which may cause sudden rupture or dissection. Thus, the presence of an aneurysm in combination with pseudocoarctation of the aorta is thought to be an indication for surgery. We present a case of pseudocoarctation of the aorta associated with thoracic aortic aneurysm and severe aortic valve stenosis with a bicuspid aortic valve. In our case, single-stage repair was performed through a median sternotomy using our "p

[Acute aortic dissection with leg ischemia].

From January of 1987 to July of 1994, 83 patients with acute aortic dissection were treated at our institution. Of these, 7 patients (8%) sustained acute leg ischemia. Angiography showed that one patient had arterial occlusion at the abdominal aorta, three had occlusion at the right common iliac artery, and one had severe right common iliac artery stenosis. Four patients with acute type A dissection underwent emergency replacement of the aortic arch and/or ascending aorta. Three of them were discharged, but one patient died due to renal failure and multiple organ failure. In three patients wit

The PETTICOAT technique is associated with abdominal aortic remodeling in sub-acute, uncomplicated type B aortic dissection.

OBJECTIVE: It remains unclear whether the provisional extension to induce complete attachment (PETTICOAT) technique is superior to standard thoracic endovascular aortic repair for type B aortic dissection. This study evaluated the efficacy of the PETTICOAT technique for type B and postoperative residual type B aortic dissection in the subacute phase. METHODS: This single-center retrospective cohort study evaluated sequential aortic morphological changes in consecutive patients with uncomplicated type B and residual postoperative type B aortic dissection treated using the PETTICOAT technique i

A case of severe aortic valve regurgitation caused by an ascending aortic aneurysm in a young patient with autosomal dominant polycystic kidney disease and normal renal function.

Aortic aneurysm is one several well-known cardiovascular complications in patients with autosomal dominant polycystic kidney disease (ADPCKD). Commonly affected site of aortic aneurysm and its related dissection in ADPCKD is abdominal aorta. Long standing hypertension, haemodialysis, old age are closely related with discovering of aortic aneurysm and dissection in ADPCKD. However, thoracic aortic aneurysms and its related severe aortic regurgitations (ARs) are rare in younger patients suffering from ADPCKD, especially ones who have normal renal function. Here, we report a case involving a 27-y

Misdiagnosed mid-back pain by Stanford type B aortic dissection masquerading as musculoskeletal origin.

Aortic dissection is a life-threatening condition that often presents with acute chest pain; Misdiagnosis can lead to delayed management and potentially fatal outcomes. This case report aims to highlight a rare instance involving a 40-year-old male patient who initially sought care at a spine clinic for his severe mid-back pain (MBP). His condition had worsened over the past 24 h, ultimately leading to a diagnosis of Stanford type B aortic dissection. The initial evaluation included physical examination and imaging studies focused on the musculoskeletal origin of the pain. However, as the pati
