Mortality, diagnosis, and etiology of disseminated intravascular coagulation-a systematic review and meta-analysis: communication from the ISTH SSC subcommittee on disseminated intravascular coagulation.

BACKGROUND: Establishing global standards for diagnosis and treatment of disseminated intravascular coagulation (DIC) requires a comprehensive evaluation of its global epidemiology. However, obtaining epidemiologic evidence on DIC from a single cohort study is challenging. OBJECTIVES: We aimed to evaluate global epidemiology and mortality of DIC by conducting a systematic literature review. METHODS: We conducted proportional meta-analyses of observational studies on DIC patients. We searched MEDLINE, Scopus, and the Cochrane Central Register of Controlled Trials and selected studies reportin…

Chronic disseminated intravascular coagulation in a patient with Takayasu arteritis.

Takayasu arteritis (TA) is a large-vessel vasculitis that predominantly affects the aorta and although it usually causes stenosis, aneurysms have been reported in ∼10-25% of the patients. Here, we present a male TA patient with chronic disseminated intravascular coagulation (DIC) as a rare complication of endovascular grafted aortic aneurysm (AA). Chronic DIC usually presents as mild or even hidden clinical symptoms, and early diagnosis of this condition can only be possible with laboratory tests. Due to the silent clinical scenario, this complication should be kept in mind among AA patients w…

[Extensive cardiac thombosis in a patient with disseminated intravascular coagulation].

Disseminated intravascular coagulation is a syndrome characterized by thrombin and fibrin generation which is associated with organ failure and death. Intracardiac thrombus may occur and further deteriorate prognosis. We report the case of a patient with massive intraventricular thrombus revealed by an acute ischemia of the upper left limb in a context of pulmonary adenocarcinoma complicated by a disseminated intravascular coagulation. We describe the diagnostic modalities and the fatal evolution.

Risk factors and treatments for disseminated intravascular coagulation in neonates.

BACKGROUND: Although disseminated intravascular coagulation (DIC) is a critical disease, there is few gold standard interventions in neonatal medicine. The aim of this study is to reveal factors affecting neonatal DIC at birth and to assess the effectiveness of rTM and FFP for DIC in neonates at birth. METHODS: We retrospectively evaluated DIC score on the first day of life in neonates with underlying conditions associated with DIC. DIC in neonates was diagnosed according to Japan Society of Obstetrical, Gynecological & Neonatal Hematology 2016 neonatal DIC criteria. RESULTS: Comparing neona…

Disseminated intravascular coagulation: treat the cause, not the lab values.

Disseminated intravascular coagulation (DIC) is a manifestation of an underlying pathologic process such as cancer, infection, trauma, or obstetric catastrophe. It can manifest as thrombosis, bleeding, or both. To succeed, treatment must address the underlying cause.

Controlled study of treatment for disseminated intravascular coagulation in the neonate.

Thirty-three neonates with disseminated intravascular coagulation were assigned randomly to one of three treatment groups: (1) exchange transfusion, (2) administration of fresh-frozen plasma and platelets, and (3) control (no therapy directed specifically at the coagulopathy). The three groups were comparable for degree of abnormality in initial coagulation studies and underlying pathologic processes. Shock was a common accompaniment of DIC and occurred in 85% of all infants. In all cases, underlying disease states and shock were treated aggressively. Resolution of DIC and survival were not di…

A Curious Case of Autoimmunity, Pancytopenia, and Disseminated Intravascular Coagulation.

HISTORY AND EXAMINATION: A 21-year-old female patient presented to us with severe low back pain for 4 months. On examination, patient was af … al edema with chronic subdural hematoma, and a concomitant coagulopathy workup revealed evidence of disseminated intravascular coagulation (DIC). DISCUSSION: Taking the whole picture into consideration, a malignant process in the body was suspected, and serum tumor markers carcinoembryonic antigen (CEA), carbohydrate antigen 19-9 (CA19-9), and cancer antigen 125 (CA-125) were sent, al …

[Cutting edge of diagnosis and treatment of disseminated intravascular coagulation].

During the treatment of disseminated intravascular coagulation (DIC) associated with hematopoietic malignancies (particularly, acute leukemia), fatal bleeding, such as cerebral, alveolar, and gastrointestinal hemorrhages at diagnosis or immediately after initiating treatment, determines the patient's prognosis. DIC should always be suspected in such cases, and the diagnosis should be made on the basis of the former Ministry of Health and Welfare DIC diagnostic criteria or the 2017 Japanese Society on Thrombosis and Hemostasis DIC diagnostic criteria. This treatment requires the use of appropri…

Avascular necrosis of the midface secondary to disseminated intravascular coagulation.

To the authors' knowledge, avascular necrosis of the midface secondary to disseminated intravascular coagulation has yet to be described following a hypoxic syncopal episode secondary to 'heat stroke'. A slow, progressive loss of anterior maxillary bone and the collapse of the nasal dorsum in a healthy young man with no other known medical co-morbidities led to the diagnosis. Following debridement, a staged reconstruction of the maxilla-nasal complex was successfully performed.

[Case of emphysematous pyelonephritis with sepsis and disseminated intravascular coagulation].

We present a case of emphysematous pyelonephritis (EPN) with sepsis and disseminated intravascular coagulation (DIC). An 87-year-old man with a history of uncontrolled diabetes mellitus (DM) for more than 25 years was admitted to our hospital for altered mental status and high fever. The initial diagnosis was acute pyelonephritis based on the findings of pyuria and right costovertebral angle knock pain. DIC developed rapidly even though empirical antimicrobial therapy had been started immediately. The abdominal CT revealed the presence of gas in the right renal parenchyma; the definitive diagn…

[Prostate cancer and disseminated intravascular coagulation: review of the literature].

Disseminated intravascular coagulation is a rare complication of prostatic cancer evolution. Occurring on hormonorefractory phase, it remains most often infraclinic. Cases of acute, severe intravascular disseminated coagulation at first presentation are life-threatening because of hemorragic and thrombotic complications, justifying emergency medical treatment. In the light of a review of literature, we insist on epidemiological features, physiopathology and therapeutics of intravascular disseminated coagulation. In spite of a pejorative prognosis, this could help to achieve a period of remissi…

Diffuse thoracic lymphangiomatosis with disseminated intravascular coagulation in a child.

An 8.5-year-old girl who presented with chronic cough and hemoptysis underwent a CT scan of the chest showing diffuse mediastinal and parenchymal infiltration and pleural effusion, and laboratory findings showed disseminated intravascular coagulation. Disseminated lymphangiomatosis was diagnosed after an open-lung biopsy. She was treated by systemic steroids, interferon, tamoxifen, chemotherapy, and radiation but died of respiratory failure and disseminated intravascular coagulation 2 years after the diagnosis. This patient represents a rare presentation of diffuse pulmonary lymphangiomatosis …
